A patient suffered from blood stasis flowing and accumulating (blood stasis circulating along the meridians and stagnating), with the lumbar spine and both feet turning black. After drinking urine of a young boy mixed with wine, bi stone therapy was used to pierce the skin and release the stagnant blood and necrotic flesh. Then, Xiao Chai Hu Tang (Minor Bupleurum Decoction) was administered, with Ban Xia (Pinelliae Rhizoma) removed, and Shan Zhi (Gardeniae Fructus), Huang Qin (Scutellariae Radix), and Lian Zi (Coptidis Rhizoma) added to clear Liver Fire. Ba Zhen Fu Ling San (Eight-Treasure Poria Powder) was used to strengthen the Spleen and Stomach. The necrotic tissue sloughed off while new flesh grew; later, the wound ulcerated again but became calm. After more than a year of regulation and treatment, the patient recovered.
Another patient had blood stasis attacking and accumulating in the scrotum, causing it to ulcerate into a fistula with thin and watery pus. All medications taken were cold and cooling in nature. Examination of the Liver pulse revealed it to be short and choppy, while other pulses were floating and weak. This indicated that Liver Wood was constrained by Lung Metal, and Yuan Qi (Original Qi) was deficient and unable to consolidate. Consequently, formulas to strengthen the Spleen and Stomach and generate Qi and Blood were employed, leading to a slight recovery of Yuan Qi. However, the patient ultimately died on a day when Metal energy was at its peak.
For severe injuries accompanied by confusion and unconsciousness, Du Shen Tang (Single Ginseng Decoction) should be urgently infused. Even if there is internal blood stasis, purgatives must absolutely not be used. Instead, Hua Rui Shi San (Flower Coral Stone Powder) should be urgently administered internally to resolve the stasis, as purgation may lead to the loss of Yin. In cases of deficient Yuan Qi, purgatives are especially contraindicated; the aforementioned powder should also be used to resolve the stasis. Whenever blood stasis remains inside the body causing obstruction of urination and defecation, if the coagulated blood does not descend, Mu Xiang (Aucklandiae Radix) and Rou Gui (Cinnamomi Cortex) powder, two or three qian, should be urgently mixed with hot wine and administered. Life can be saved only after the blood descends. For individuals with weak constitutions, when using Xiao Ming (Natrii Sulfas) and Da Huang (Rhei Radix et Rhizoma), Rou Gui and Mu Xiang must be added for decoction, utilizing their heat to facilitate the action of the cold medicines.
A woman injured her foot in early winter, resulting in blood loss, sweating on the head, internal heat with thirst, shortness of breath, irritability, and intermittent confusion. Her pulse was surging and large but weak upon deep pressure. This indicated deficiency of Yin Blood below and solitary Yang flaring above, causing Jue syndrome (cold extremities due to Yang deficiency or blockage) and head sweating. One dose of Si Wu Tang (Four-Substance Decoction) combined with Xiao Chai Hu Tang stopped the sweating. Subsequently, Si Wu Tang was used with Chuan Xiong (Ligustici Chuanxiong Rhizoma) removed, and Ren Shen (Ginseng Radix), Bai Zhu (Atractylodis Macrocephalae Rhizoma), Mai Men Dong (Ophiopogonis Tuber), Wu Wei Zi (Schisandrae Fructus), and Zhi Gan Cao (Prepared Licorice Root) added, with a small amount of Rou Gui. After four doses, all symptoms disappeared. After another thirty-plus doses, Qi and Blood were restored, and she recovered.
A man fractured his leg in early summer, losing excessive blood. Initially, he experienced dizziness and blurred vision, followed by confusion. This was caused by damage to Yin Blood, blazing Yang Fire, inability of Lung Metal to control Liver Wood, and Liver Wood exuberance generating Wind. Urgent administration of young boy's urine was given, followed by a formula containing Ren Shen and Dang Gui (each 5 qian), Jing Jie (Schizonepetae Spica), Chuan Xiong, Chai Hu (Bupleuri Radix), Shao Yao (Paeoniae Radix), and Bai Zhu (each 2 qian), Shan Zhi, Huang Bo (Phellodendri Cortex), Huang Qin, and Jie Geng (Platycodonis Radix) (each 1 qian), and Gan Cao (0.5 qian). He felt refreshed after taking it. Later, Si Wu Tang was used with Ren Shen and Bai Zhu (each 3 qian), Sheng Di Huang (Rehmanniae Radix), and Chai Hu (each 1 qian). After four doses, irritability completely subsided.
