If the affected area appears pale, it is due to Spleen Qi deficiency; use Liu Jun Zi Tang (Six Gentlemen Decoction), Chuanxiong Rhizoma (Chuanxiong), and Angelicae Sinensis Radix (Danggui). If the affected area appears red, it is due to Yin Blood deficiency; use Si Wu Tang (Four Substance Decoction), Ginseng Radix et Rhizoma (Renshen), and Atractylodis Macrocephalae Rhizoma (Baizhu). If there is aversion to cold and fever, it is due to Qi and Blood deficiency; use Shi Quan Da Bu Tang (Ten Significant Great Tonifying Decoction). If the pus is thin and white with no new flesh growing, it is due to Spleen and Lung Qi deficiency; use Bu Zhong Yi Qi Tang (Tonify the Middle and Augment the Qi Decoction). If the pus is thin and red with no new flesh growing, it is due to Heart and Spleen Blood deficiency; use Dong Yuan Sheng Yu Tang (Dongyuan's Holy Salvation Decoction). If there are alternating chills and fever with no new flesh growing, it is due to Liver Fire stirring; use Jia Wei Xiao Yao San (Modified Free and Easy Wanderer Powder). If there is afternoon tidal fever with no new flesh growing, it is due to Liver Blood deficiency; use Ba Zhen Tang (Eight Treasure Decoction) plus Moutan Cortex (Mudanpi). If there is poor appetite and physical fatigue with no new flesh growing, it is due to Spleen and Stomach Qi deficiency; use Liu Jun Zi Tang (Six Gentlemen Decoction). If the pus is foul with no new flesh growing, it is due to Yin deficiency with pathogenic fire; use Liu Wei Di Huang Wan (Six-Ingredient Rehmannia Pill). If there is limb heaviness and fatigue, shortness of spirit, and no new flesh growing, it is due to internal injury of Yuan Qi (Primary Qi); use Bu Zhong Yi Qi Tang (Tonify the Middle and Augment the Qi Decoction). In summer, if diarrhea occurs, use Tiao Zhong Yi Qi Tang (Regulate the Middle and Augment the Qi Decoction); in heatstroke season, use Qing Shu Yi Qi Tang (Clear Summerheat and Augment the Qi Decoction); in autumn, if diarrhea occurs, use Qing Zao Tang (Clear Dryness Decoction).
A patient had an ulcer that failed to close. Because there was toxic heat internally, [the physician] intended to use cooling herbs. Xue Ji said: This is due to both Qi and Blood deficiency preventing closure. Without using Danggui, Baizhu, Renshen, etc., to cultivate the Spleen Earth, how can muscle tissue grow? How can one re-use cooling and draining herbs to further damage Qi and Blood! Thus, the aforementioned medicines were used, and the disease was cured.
If bleeding occurs from the affected area or various orifices, it is due to blazing Liver Fire causing blood heat to deviate from its vessels and flow recklessly; use Jia Wei Xiao Yao San (Modified Free and Easy Wanderer Powder) to clear heat and nourish blood. If it is due to weak Central Qi, with blood having no attachment and flowing recklessly, use Jia Wei Si Jun Zi Tang (Modified Four Gentlemen Decoction) to tonify and augment Central Qi. Or if Yuan Qi has collapsed internally and cannot control the blood, use Du Shen Tang (Single Ginseng Decoction) plus Pao Jiang (Prepared Ginger) to restore Yang. If there is no response, urgently add Aconiti Lateralis Radix Praeparata (Fuzi). Or if blood is stagnated internally causing hematemesis, use Si Wu Tang (Four Substance Decoction) plus Bupleuri Radix (Chaihu) and Scutellariae Radix (Huangqin). Any injury from falls, overwork, or anger, leading to abdominal distension and fullness, and mistakenly taking Rhubarb (Dahuang) and other drugs, damages the Yang collaterals resulting in hematemesis, epistaxis, hematochezia, or hematuria; damages the Yin collaterals resulting in blood stasis and clots, with muscles appearing dark blue. This is due to visceral deficiency and failure of the meridians' function; urgently tonifying Spleen and Lung may lead to recovery. Alas, it is a pity that patients do not understand this principle and do not adopt this method!
Zhang Diguan fell from a horse and injured his leg. After taking Caowu (Aconitum kusnezoffii) and other drugs, he suffered epistaxis, cough, arm pain, yellow eyes, thirst, toothache, and scanty urination. This was caused by drying agents damaging the Lung and Large Intestine. Xue Ji used Sheng Dihuang (Rehmanniae Radix), Huangqin (Scutellariae Radix), Huanglian (Coptidis Rhizoma), Huangbai (Phellodendri Cortex), Zhimu (Anemarrhenae Rhizoma), Shanzhi (Gardeniae Fructus), Shanyao (Dioscoreae Rhizoma), and Gancao (Licorice Root) to moisten Lung dryness and generate Kidney Water. Urination immediately increased, and all symptoms ceased. Then, using Shanyao, Wuweizi (Schisandrae Fructus), Maidong (Ophiopogonis Radix), Renshen, [missing character, likely Huangqi], Chuanxiong, Danggui, Huangbai, Huangqin, Zhimu, and Zhi Gancao (Honey-fried Licorice), he nourished Yin Blood and supported Yuan Qi, leading to wound healing.
Yu Jinshi had a fractured leg bone set for three months but still suffered fever and continuous bleeding. The Zhengti Tang physicians' treatment was ineffective. The pulse at the left Guan position was surging and rapid, indicating blazing Liver Fire causing blood to flow recklessly due to heat. He was administered Xiao Chai Hu Tang (Minor Bupleurum Decoction) plus Shanzhi, Shaoyao (Paeoniae Radix Alba), Sheng Dihuang, and Fangfeng (Saposhnikoviae Radix). Bleeding stopped and fever subsided. Subsequently, Ba Zhen Tang (Eight Treasure Decoction), Wuweizi, and Maidong were used, and the wound healed.
The nephew of Tian Zongbo fell due to anger in mid-autumn, suffering whole-body pain, fever, and epistaxis. The Liver pulse was wiry and surging. Xue Ji said: Long-term epistaxis with a wiry and surging pulse indicates excessive Liver Fire controlling Metal. By spring, when Liver Wood flourishes, it will self-consume or damage Spleen Earth. This is not an easy-to-treat condition; one should nourish Kidney Water to generate Liver Wood and tonify Spleen Earth to generate Lung Metal. However, mixed use of Liver-fire-draining herbs led to his death in early spring.
Dayin Liu Guoxin suffered bleeding from a metallic wound, fever, and irritability, attributed to Yin deficiency. Sheng Yu Tang (Holy Salvation Decoction) was used. The deficient fire subsided, and blood returned to its vessels. The nephew of Liang Gelao suffered swelling, pain, and continuous bleeding from a metallic wound, with chills, fever, and dry mouth. This was due to Qi deficiency causing blood to have no attachment and fail to return to its vessels. Bu Zhong Yi Qi Tang (Tonify the Middle and Augment the Qi Decoction) was used, with Wuweizi and Maidong as primary adjuncts. Yang Qi recovered, and he healed.
