Liu Hejian stated: Wind syndromes are characterized by their mobility and rapid changes; invading the Zang-Fu organs is extremely fast, with life or death determined in an instant. Therefore, it is imperative to urgently differentiate between exterior/interior and deficiency/excess to guide treatment. When pathogenic factors are at the exterior, symptoms include tendon and vessel stiffness, occasional chills and fever, muscle twitching and spasms, and a floating, wiry pulse. Use Qianghuo Fangfeng Tang (Notopterygium and Saposhnikovia Decoction) to disperse them. When pathogenic factors are at the half-exterior/half-interior, symptoms include slight sweating on the head but no sweating on the body. Use Qianghuo Tang (Notopterygium Decoction) to harmonize. When pathogenic factors have entered the interior, symptoms include a stiff tongue, locked jaw, opisthotonos (backward arching of the neck and back), muscle twitching and spasms, copious phlegm-saliva, chest and abdominal fullness and oppression, constipation and dark urine, occasional sweating, and a surging, rapid, and wiry pulse. Use Da Xionghuang Tang (Major Ligusticum and Rhubarb Decoction) to guide out and purge. If sweating persists after purgation, this indicates exterior deficiency; use Baizhu Fangfeng Tang (Atractylodes Macrocephala and Saposhnikovia Decoction) to tonify. It is beneficial to occasionally administer porridge drinks. The aforementioned methods apply when Qi deficiency has not yet severely damaged the constitution. If pus and blood loss are excessive, causing Yang to dissipate with Yin, resulting in both Qi and Blood deficiency with symptoms resembling the above, one must vigorously tonify the Spleen and Stomach, strictly avoiding wind-dispelling herbs. For details, see Volume 5 of Miscellaneous Diseases.
One patient injured his hand in mid-summer, presenting with opisthotonos and locked jaw, and a floating, scattered pulse. This was an exterior syndrome, so he was administered one dose of Qianghuo Fangfeng Tang (Notopterygium and Saposhnikovia Decoction), which resolved the condition immediately. If this syndrome occurs during autumn or winter when the interstices are dense, herbs like Mahuang (Ephedrae Herba) must be used to induce sweating. This is the treatment method for sudden injury without significant damage to Qi and Blood.
Another patient suffered a minor break from a beating and developed this condition, presenting with a surging, large, and excess pulse. This was an interior syndrome. One dose of Da Xionghuang Tang (Major Ligusticum and Rhubarb Decoction) resulted in slight bowel movement, after which all symptoms subsided. Administering exterior-releasing drugs would have been fatal. Urgent differentiation of exterior/interior and deficiency/excess is essential to avoid error.
Another patient presented with chills, fever, and dry mouth. Four doses of Siwu Tang (Four Substances Decoction) modified with Renshen (Ginseng Radix), Huangqi (Astragali Radix), Baizhu (Atractylodis Macrocephalae Rhizoma), Ruan Chaihu (Bupleuri Radix), Chao Huangqin (Scutellariae Radix), Maimendong (Ophiopogonis Radix), and Wuweizi (Schisandrae Fructus) reduced symptoms slightly. Xue Ji proposed using Bian stone (stone lancet) to remove stasis, but the patient refused. Later, the patient developed palpitations, insomnia, poor appetite, tight jaw, headache and eye pain, chills, and fever. This indicated internal accumulation of pus/toxins. Upon applying Bian stone therapy to drain the pus, the patient felt relieved. Twenty doses of Bazhen Tang (Eight Treasures Decoction) modified with Suanzaoren (Ziziphi Spinosomi Semen), Maimendong (Ophiopogonis Radix), and Wuweizi (Schisandrae Fructus) gradually cured the previous symptoms. Subsequently, the previous prescription plus Dushen Tang (Single Ginseng Decoction) helped necrotic tissue slough off. Due to overexertion, the patient later experienced sleeplessness and night sweats, which were cured using Guipi Tang (Restore the Spleen Decoction) modified with Maimendong (Ophiopogonis Radix), Wuweizi (Schisandrae Fructus), and Yuanzhi (Polygalae Radix). Later, the patient had a distended and stuffy jaw and reddened face and eyes. Suspecting tetanus again, Xue Ji still considered it a deficiency syndrome and treated with Bazhen Tang (Eight Treasures Decoction) and other herbs, resulting in stability.
Another patient presented with abdominal pain, panting, thirst, chills, and fever, with erosion of the buttocks and thighs mixed with dead blood, resembling a skin bag filled with paste. He took a decoction of Tongbian (Child's Urine) boiled with Siwu Tang (Four Substances Decoction) plus Taoren (Persicae Semen), Honghua (Carthami Flos), Chaihu (Bupleuri Radix), Huangqin (Scutellariae Radix), Maimendong (Ophiopogonis Radix), and Huafen (Trichosanthis Radix), which rapidly reduced symptoms. However, he applied black sheepskin to the area, worsening the condition. After Bian stone therapy drained much pus and blood, his breath became faint, lips moved slightly, jaw tightened, and the lesion turned dark. Some suggested using tetanus medicines. Xue Ji said: This is a complication due to Qi and Blood deficiency. Use Renshen (Ginseng Radix), Huangqi (Astragali Radix), Chuanxiong (Ligustici Chuanxiong Rhizoma), Danggui (Angelicae Sinensis Radix), and Baizhu (Atractylodis Macrocephalae Rhizoma), along with Dushen Tang (Single Ginseng Decoction) and Renru (Human Placenta Milk). As Yuanqi (Original Qi) recovered, symptoms subsided. Then, Shiquan Dabu Tang (Comprehensive Great Tonic Decoction) was used for regulation and recovery. If pus and stasis are retained internally, needle puncture is appropriate. If after ulceration there is locked jaw and enuresis resembling tetanus, it indicates extreme Qi and Blood deficiency; urgently use major tonic formulas. If the patient usually has phlegm and suffers from wind syndromes, clear phlegm and descend fire. If wind syndromes appear due to anger, clear the Liver and descend fire. If the patient neglects sexual restraint and suddenly develops these symptoms, it is due to insufficient Kidney Water and blazing Heart Fire; prioritize nourishing Yin and tonifying Qi and Blood. Misdiagnosing this as a wind syndrome will lead to death.
