A question was asked: "What is the nature of a carbuncle developing at the back of the neck and below the head?" The answer was: "This is what is called 'Duikou Chuang' (Carbuncle facing the mouth), which belongs to the Du Meridian (Governing Vessel). When Yang Qi alone is excessive and Qi is surplus, Fire flares upward, causing a carbuncle. One should urgently administer Wujin San (Black Gold Powder) or Shengjin Dan (Victory Golden Pill) to induce sweating. For those with robust constitutions, Yili Jin Dan (One-Pill Golden Pill) may be used to purge. If treatment is delayed until it ulcerates and discharges pus, and wind pathogen triggers spasms, it is difficult to treat. If toxic pathogens are excessively congested, leading to sudden massive hemorrhage and death, or if toxins attack the heart and abdomen causing distension and delirium, death ensues. In this condition, if there is redness, swelling, and pain, it is treatable; however, if the base is broad and the spirit is confused and lethargic, it is difficult to treat. One must assess the exterior-interior and deficiency-excess patterns, employing methods such as diaphoresis, purgation, interior support, or interior resolution. Externally, apply a strong encircling plaster to confine the lesion, and puncture the center with Liu Hui Gao (Six-Gray Ointment) to break and drain the toxicity, ensuring the root does not scatter. If the toxin threatens to spread to both shoulders and the spine, it is incurable. Some suggest first taking Zhuiting Duoming Tang (Tiphoid-Expelling Life-Snatching Decoction) for two doses, followed by Huoxue Huadu Tang (Blood-Invigorating and Toxin-Transforming Decoction)."
An effective prescription for treating Duikou Chuang: Use seven fresh eggplant stems, weigh them, then use fresh He Shou Wu (Polygonum multiflorum Rhizome), weighing the same amount as the eggplant stems. Decoct in one and a half cups of water and drink. After one dose, pus will discharge; after another dose, the wound will close. If fresh eggplant stems or He Shou Wu are unavailable, dried ones may be used, though they are less potent than fresh ones. This is a mild formula and cannot treat severe diseases; readers should not adhere rigidly to it.
A question was asked: "What is the nature of extreme itching penetrating to the bone above the Tianzhu bone, accompanied by nausea, vomiting, and stiffness/tension in the shoulder and back?" The answer was: "This is named Tianzhu Ju (Tianzhu Carbuncle). Urgently apply moxibustion through garlic slices until the itching stops. If garlic is unavailable, direct moxibustion can also be used. Administer Miding Yin (Life-Saving Decoction) added with Qiang Huo (Notopterygium Rhizome) and Jie Geng (Platycodon Root), or select Wujin San or Shengjin Dan. The appearance of blisters after moxibustion is auspicious; their absence is ominous. Sweating after administering diaphoretic agents indicates treatability; lack of sweating indicates difficulty. If it ulcerates with coma, retching, nausea, and uncontrollable bleeding, death occurs."
A question was asked: "What is the nature of a carbuncle developing on the upper neck and behind both ears?" The answer was: "This is named Zhu Ju (Zhu Carbuncle). Initially, swelling is not severe, but pain and itching occur intermittently with clear fluid discharge. It gradually enlarges, resembling the spots of a hawksbill turtle; it is also called Huagu Chuang (Bone-Melting Sore). Urgently administer Bazhen San (Eight-Army Powder), Duoming Dan (Life-Snatching Pill), or Miding Yin added with Jie Geng and Chai Hu (Bupleurum Root). If treatment is delayed and it ulcerates for a long time, exposing bone, it is a highly malignant condition. This disease often arises in individuals with chronic emotional depression, frequently presenting the 'Seven Evils' syndromes. Those who violate dietary or behavioral taboos are incurable."
