Haizang stated: Zhang Zhongjing's various Xiexin Decoctions (Bitter Cold Decoction to Drain the Heart), such as those mentioned, target disorders of the Hand-Shaoyin Heart Meridian. Because the symptom manifests as epigastric fullness and stuffiness (Pi), it is categorized under Yangming disease patterns.
Dahuang Huanglian Xiexin Tang (Rhubarb and Coptis Bitter Cold Decoction to Drain the Heart): Treats Taiyang disease where, after a physician erroneously induced sweating, fever and aversion to cold ensued; subsequently, purgation was incorrectly applied, resulting in epigastric fullness and stuffiness. At this stage, both exterior and interior are deficient, and Yin-Yang and Qi-Blood are exhausted. The absence of Yang leads to the dominance of Yin. If moxibustion with burning needles is additionally applied, chest irritability ensues. A cyan-yellow complexion indicates a difficult-to-treat condition. If the complexion is slightly yellow and the hands and feet are warm, recovery is easy. For cases of epigastric fullness and stuffiness that are soft upon palpation, with a floating pulse at the Guan position, this formula is indicated.
Ingredients: Dahuang (Rhei Radix et Rhizoma) (2 liang), Huanglian (Coptidis Rhizoma) (1 liang). Note: Adding Huangqin (Scutellariae Radix) creates Dongyuan's Sanhuang Tang (Three Yellows Decoction).
Preparation and Administration: Chop the herbs into sizes similar to hemp seeds. Soak in 2 sheng of boiling water for a short while, then strain to remove dregs. Divide into two doses and take warm.
Fuzi Xiexin Tang (Aconite Bitter Cold Decoction to Drain the Heart): Treats epigastric fullness and stuffiness accompanied by aversion to cold and sweating. This condition arises from erroneous purgation causing epigastric stuffiness. If the stuffiness does not resolve with Xiexin Tang, and the patient presents with thirst, dry mouth, vexation, and dysuria, Wuling San (Five-Ingredient Powder with Poria) should be used primarily.
Ingredients: Dahuang (Rhei Radix et Rhizoma), Huanglian (Coptidis Rhizoma), Huangqin (Scutellariae Radix) (each 1 liang); Fuzi (Aconiti Lateralis Radix Praeparata) (1 piece, processed, peeled, sliced, decocted separately to extract juice).
Preparation and Administration: Chop the first three herbs. Soak in 2 sheng of boiling hemp-seed decoction (Mafei Tang), strain to remove dregs, add the extracted Fuzi juice, divide into two doses, and take warm.
Shengjiang Xiexin Tang (Fresh Ginger Bitter Cold Decoction to Drain the Heart): Treats post-Taiyang disease where, after sweating resolved the exterior syndrome, stomach disharmony persists, manifesting as hard epigastric fullness and stuffiness, dry belching with food odor, fluid retention under the hypochondrium, borborygmus, and diarrhea.
Ingredients: Shengjiang (Zingiberis Rhizoma Recens), Banxia (Pinelliae Rhizoma Preparatum) (washed, each 2 liang); Gancao (Glycyrrhizae Radix et Rhizoma) (honey-fried); Huangqin (Scutellariae Radix); Renshen (Panacis Ginseng Radix) (each 1.5 liang); Ganjiang (Zingiberis Rhizoma); Huanglian (Coptidis Rhizoma) (each 0.5 liang); Dazao (Jujubae Fructus) (6 pieces, split).
Preparation and Administration: Decoct the eight herbs in 5 sheng of water to obtain 3 sheng. Strain, then further decoct to obtain 1.5 sheng. Take 0.5 sheng warm per dose.
Li Dongyuan Gancao Xiexin Tang (Licorice Bitter Cold Decoction to Drain the Heart): Treats Shanghan or Fengzhong where erroneous purgation caused frequent diarrhea (dozens of times daily) with undigested food, borborygmus, hard and full epigastric stuffiness, dry vomiting, and restlessness. Seeing the epigastric stuffiness, the physician mistakenly believed pathogenic factors remained and purged again, worsening the stuffiness. This is not heat accumulation but deficiency of Stomach Qi with rebellious hostile Qi rising, causing hardness. This formula is suitable.
Ingredients: Gancao (Glycyrrhizae Radix et Rhizoma) (2 liang); Banxia (Pinelliae Rhizoma Preparatum) (1 liang); Huangqin (Scutellariae Radix), Ganjiang (Zingiberis Rhizoma) (each 3.5 liang); Huanglian (Coptidis Rhizoma), Renshen (Panacis Ginseng Radix) (each 0.5 liang); Dazao (Jujubae Fructus) (6 pieces).
Preparation and Administration: Decoct the seven herbs in 5 sheng of water to obtain 3 sheng. Strain, then further decoct to obtain 1.5 sheng. Take 0.5 sheng warm per dose, divided into three servings.
Banxia Xiexin Tang (Pinellia Bitter Cold Decoction to Drain the Heart): Treats epigastric fullness without pain, which is defined as Pi (stuffiness); if painful, it is Jie Xiong (Chest Binding).
Ingredients: Banxia (Pinelliae Rhizoma Preparatum) (0.5 sheng, soaked); Huangqin (Scutellariae Radix), Ganjiang (Zingiberis Rhizoma), Renshen (Panacis Ginseng Radix) (each 3 liang); Huanglian (Coptidis Rhizoma) (1 liang); Gancao (Glycyrrhizae Radix et Rhizoma) (honey-fried, 2 liang); Dazao (Jujubae Fructus) (12 pieces).
