During pregnancy, one must guard against miscarriage occurring in the third, fifth, and seventh months. This is because at three months of gestation, fetal fire begins to stir, often causing mental restlessness and chest oppression. At four or five months, the body receives the Ministerial Fire of Shaoyin (Lesser Yang) to nourish essence. At six or seven months, it receives the Executive Fire of Shaoyang (Greater Yang) to nourish qi. If the constitution has even slight deficiency of qi and blood, the fetus may descend and be lost. Chao Yuanfang stated: After conception in the abdomen, changes occur every seven days; by the third month, it belongs to the Heart; by the fifth month, to the Spleen; by the seventh month, to the Lung; all belonging to the Zang organs. Deficiency of Yin in the Zang organs makes miscarriage easy. For instance, if the first month belongs to the Liver, anger can cause miscarriage. Washing the lower body causes the orifices to open, which can also lead to miscarriage. Imprudent sexual intercourse can also cause miscarriage. This is what was previously referred to as "miscarrying immediately upon conception," which happens in most cases, though people are often unaware. Generally, abdominal pain due to fetal qi is common, but lower back pain requires strict vigilance. Medication should be taken immediately to tonify the Kidney and calm the fetus; this advice cannot be given too early. Once bleeding occurs, only three out of ten cases can be saved; it is appropriate to take Antai Yin (Calming Fetus Decoction), using a heavy dose of Shu Di Huang (Prepared Rehmannia Root), or adding Ren Shen (Ginseng Root) and Sang Ji Sheng (Taxillus Branch). However, spontaneous abortion (Xiao Chan) is more severe than term delivery. Term delivery is like a ripe melon falling from the vine, a natural process of heaven. Spontaneous abortion must have a cause; it descends due to disturbance, akin to forcibly breaking the stem. The fetal sac attaches to the Dai Mai (Belt Vessel), which encircles and ties to the waist; therefore, when there is lower back pain and the Dai Mai becomes lax, the fetal sac will descend. Taking Baotai Wan (Fetus-Protecting Pills) in advance prevents disaster.
After spontaneous abortion, treatment is similar to that for term delivery. If blood loss is significant, Ren Shen (Ginseng Root) and Shu Di Huang (Prepared Rehmannia Root) can be taken, while Dang Gui (Angelica Sinensis Root) should be used in heavy doses, reflecting the principle of removing stasis to generate new blood. In cases of severe deficiency-cold, Fu Zi (Aconite Lateral Root) and E Jiao (Donkey-hide Gelatin) should be added.
Zi Fan Syndrome (Pregnancy-related Restlessness) presents with mental restlessness, chest oppression, and indescribable distress. This is caused by the gathering of Ministerial Fire and Executive Fire to nourish the fetus; excessive fire leads to internal heat and restlessness. Dan Zhu Ye Tang (Lophatherum Decoction) is indicated as the primary treatment. If chest oppression is due to Qi stagnation and phlegm coagulation, Er Chen Tang (Two-Cured Decoction) with Bai Zhu (Atractylodes Macrocephala Rhizome), Huang Qin (Scutellaria Root), Su Geng (Perilla Stem), and Zhi Qiao (Aurantium Fructus) should be used. If there is Spleen deficiency and Stomach weakness with poor appetite and vomiting, Liu Jun Zi Tang (Six-Gentlemen Decoction) should be used, or Xiang Sha Liu Jun Zi Tang (Aucklandia and Amomum Six-Gentlemen Decoction) added to soothe the condition. However, if Zi Fan syndrome is not properly managed, it can lead to restless fetus and risk of spontaneous abortion. Furthermore, prolonged vomiting may cause Qi rebellion and severe depletion of Stomach Yuan Qi, preventing food intake, which risks Qi collapse in the mother. Therefore, the initial approach must be to clear fire, followed by calming the Stomach and promoting food intake; this is a fixed principle.
Dan Zhu Ye Tang (Lophatherum Decoction)
Dan Zhu Ye (Lophatheri Herba, 10 leaves, washed) Huang Qin (Scutellariae Radix, 1 qian) Zhi Mu (Anemarrhenae Rhizoma, 1 qian) Mai Dong (Ophiopogonis Tuber, 1.5 qian, heart removed) Fu Ling (Poria, 2 qian)
Decoct in water and take orally.
Er Chen Tang (Two-Cured Decoction) (See previous section on E Zu / Nausea of Pregnancy)
Liu Jun Zi Tang (Six-Gentlemen Decoction) (Same as seen previously)
If Xiang Fu (Cyperi Rhizoma, 1 qian) and Sha Ren (Amomi Fructus Rotundus, 0.5 qian) are added, it becomes Xiang Sha Liu Jun Zi Tang (Aucklandia and Amomum Six-Gentlemen Decoction).
