The failure of the vulva to close is due to severe deficiency of Qi and Blood. First, administer Shenghua Tang (Transforming Stasis Decoction) for one or two doses, followed by Bazen Tang (Eight-Treasure Decoction) for tonification. If there is no response, use Shiquan Dabu Tang (Ten-Agent Great Tonifying Decoction) for warming and tonifying, after which the vulva will naturally close without abnormality.
In cases of difficult labor where the midwife's fingers cause trauma, resulting in damage and pain to the vulva, a small amount of Huarui Shi San (Flower Rhyolite Powder) can be applied to the wound, leading to gradual natural healing.
[Formula: Shenghua Tang]
Danggui (Radix Angelicae Sinensis, 4 qian) Chuanxiong (Rhizoma Chuanxiong, 1.5 qian) Heijiang (Prepared Zingiberis Rhizoma, 1 qian; use 0.5 qian in summer) Taoren (Semen Persicae, 10 grains, remove tips and crush; avoid double kernels) Yimucacao (Herba Leonuri, 2 qian)
Decoct with water and take warm. Fushen (Poria with Hostwood) and Gancao (Radix Glycyrrhizae) may be added according to the patient's condition. This is a celestial prescription. Based on the principle of generating and transforming life, it is a formula that must be used postpartum. For all new mothers, it should be taken for two or three doses starting the next day, naturally eliminating stasis and generating new blood, ensuring no illness postpartum. If other causes exist, treat according to specific symptoms. Secular people often claim that Shanzha (Fructus Crataegi) relieves pain, not knowing it is a digestive agent; it must absolutely not be used.
Shenghua Tang embodies the meaning of resolving stasis and generating newness. The combination of these five herbs each possesses the ability to generate newness and resolve stasis; it is not only Danggui that generates blood, but also Taoren and Yimucacao that attack stasis.
However, if one claims this is a mandatory formula for all postpartum cases, I dare not believe it firmly. Why? Because this formula consists entirely of acrid-warm substances. If applied to individuals with excessive Yang or heat constitution, would it not be like pouring oil on fire? (Comment by Xu)
Bazen Tang (See previous section on Pre-labor; adding Rougui [Cortex Cinnamomi] makes it Shiquan Dabu Tang)
Palpitations and spontaneous sweating postpartum are suitable for treatment with Guijiangzao Tang (Angelica, Ginger, and Jujube Decoction), combined with Vinegar Charcoal Fumigation.
[Formula: Guijiangzao Tang]
Danggui (Radix Angelicae Sinensis, 4 qian) Heipaogjiang (Prepared Blackened Zingiberis Rhizoma, 1 qian) Chao Zaoren (Fried Semen Ziziphi Spinosae, 2 qian)
Add Heizao (Blackened Jujubes) and decoct with water to take. If spontaneous sweating continues after administration, accompanied by palpitations and lack of mental control, fearing syncope or collapse, add Ren Shen (Radix Ginseng, 1 qian) and Shu Fuzi (Prepared Aconiti Lateralis Radix, 0.5 qian) to secure the root. Continue using vinegar charcoal fumigation to gather the spirit and stop sweating. This is an excellent method of simultaneous internal and external treatment. Although Ren Shen is generally inconvenient to use immediately after birth, it is indispensable when signs of collapse appear. As long as Danggui is heavily used, blood stasis will not remain, causing no harm. People hesitate to use it due to ignorance of preparation methods; improper use naturally leads to errors. In cases of Qi deficiency and weak Spleen, presenting with sweating, diarrhea, vomiting, and loss of appetite, what else could save them besides Ren Shen and Fuzi? The Classics state "in case of blood loss, tonify Qi"; extreme deficiency generates cold, requiring Shiquan Dabu Tang (Ten-Agent Great Tonifying Decoction) for success.
