Postpartum accumulation of blood stasis often leads to the formation of internal abscesses, a condition rarely recognized by people. When the pathogenic toxin flows into the thighs and legs and manifests externally, it is an external discharge of toxins. This falls under surgical treatment and, although treatable, poses little danger. However, when accumulation occurs internally leading to Large Intestine Abscess (Chang Yong) or Small Intestine Abscess (Xiao Chang Yong), it becomes a matter for Internal Medicine and Obstetrics/Gynecology, which must not be ignored. Often, the abscess may perforate through the umbilicus, or cause internal rotting and rupture of the intestines leading to death. People only know that the patient died from abdominal pain, unaware of the harm caused by intestinal abscess. Having cured many such cases, I have recorded them in this book to supplement what predecessors had not fully expounded, and to enable future practitioners in obstetrics and gynecology to understand this condition and save lives.
The symptoms can be diagnosed by pressing on the painful areas within the abdomen; if there is a hard mass near the umbilicus that causes increased pain upon pressure, this indicates an intestinal abscess. If the hard mass is firm, pus has not yet formed, and the Chi pulse (尺脉) will definitely not be rapid (Shu). If there is ceaseless day and night pain, use Mudanpi San (Cortex Moutan Powder). After taking several doses, the mass will surely dissipate. If the Chi pulse has become rapid, and the mass feels slightly soft upon pressure, with borborygmus (intestinal rumbling) inside, and urination and defecation not flowing smoothly, with intermittent cessation of pain followed by recurrence shortly after, this indicates that the abscess has matured and pus/toxin has no outlet. A Small Intestine Abscess will inevitably perforate through the umbilicus, indicating a critical condition. At this stage, a large dose of Renshen (Ginseng Root) should be used to tonify Qi; perhaps one out of ten patients can be saved. Even if life is preserved, fertility will be lost for life. If the treatment method is unknown, urine will flow from the umbilicus, leading to certain death with no rescue possible.
In the case of a Large Intestine Abscess, defecation is difficult. If treated improperly, or if the toxin is not expelled, the toxin will rot the intestines. Once the intestines are damaged, diarrhea will occur. The upper body will exhibit inability to eat, vomiting, and nausea, accompanied by a foul odor from the mouth. This indicates intestinal rotting and perforation, which is fatal. I have recorded verified cases, attached at the end of the volume.
If blood stasis flows to the thighs, buttocks, or waist, the pain is like needle pricks, and the area is too tender to touch. One must also guard against the development of carbuncles (Ju). It is advisable to take Tao Ren Tang (Peach Kernel Decoction) early to dissipate the stasis. In the initial stage of an intestinal abscess, there is lower abdominal pain, difficulty in urination, a slight and moderate pulse in all six positions, without chills or fever; this is mild. If the abscess has already formed, the lower abdomen is hard, swollen, and the six pulses are surging and rapid; this is dangerous. If only the Chi position pulse is rapid, it is still treatable. After ulceration, if there is a fishy/foul odor, inability to eat, and intolerance to tonification, it is incurable. Initially, administer Mudanpi San. If it does not subside, proceed with Tao Ren Tang or Yiyiren Tang (Coix Seed Decoction). If it still does not resolve, use Painong Tang (Drain Pus Decoction). If it has not perforated, Chenpi Kuigen Tang (Tangerine Peel and Hibiscus Root Decoction) will induce perforation. After pus drainage, if slight pain persists, my formulated Haifu Tang (Sea Floating Decoction) taken for three doses will expel remaining toxins or facilitate wound closure if toxins are exhausted. After three doses, one should take Siwu Tang (Four Substance Decoction), Bazhen Tang (Eight Treasures Decoction), or Shiquan Dabu Tang (Ten Complete Tonifying Decoction) to tonify. Otherwise, although the toxin is gone, the interior remains deficient, risking the emergence of deficiency signs such as poor appetite, sallow complexion, night sweats, weakness, mental fatigue, and insomnia. Sexual intercourse should be avoided for over a year, and both diet and emotional agitation must be handled with caution.
