The generation of swelling arises from wind pathogens, cold-heat toxic qi invading the meridians, causing blood stasis and obstruction, leading to congestion and accumulation that form swelling. When caused by wind pathogens, the swelling has no head or root, floats on the skin surface, resembling a blown-up state; it is not red or painful, sometimes swollen and sometimes dispersed, not constantly swollen. When caused by cold qi battling with blood, there is a head and root, with red, painful, and swollen characteristics. When caused by heat toxicity, there is no definite head, but rather acute swelling that persists for a long time without subsiding; excessive accumulation of heat qi leads to pus formation. Since the manifestations are not uniform, they are referred to as various swellings (Zhu Zhong).

When a person suddenly develops swelling, whether on the limbs, chest, back, head, or neck, large as the rim of a water jar, with fluctuant, circular swelling resembling a blown-up state, without pain or redness: if located on the limbs, it may lead to motor impairment, causing irritability, fullness, shortness of breath, and constant coldness of the body. This is all due to encountering dampness in winter, allowing wind to enter the muscle interior; in spring, upon exposure to severe cold, the wind cannot exit, causing qi stagnation between the muscles, often unnoticed; in summer, seeking coolness and wind causes dampness to gather and fail to dissipate, forming swelling. If prolonged and unhealed, qi stagnation generates intense heat, transforming into pus and blood, leading to rotting and decay, which can be fatal.

A floating and deficient pulse at the Guan position on the right hand indicates swelling disease.

Sudden onset of swelling without pain or redness, moving unpredictably and accompanied by itching, occurs because previously there was no illness, but the interstices were vacuous, encountering wind pathogens.

Feng Du Zhong (Wind-Toxin Swelling) initially presents with redness, pain, and heat, with blister formation on the swelling, resembling burns from fire.
