The manifestation of toxic swelling is indistinguishable from wind-induced swelling, often causing high fever (Zhuang Re). If the pathogenic toxin is severe and invades the abdomen, it can be fatal.
This condition is similar to the previously described toxic swelling, merely indicating that the swelling and heat gradually intensify due to the invasion of toxins into the abdomen. The manifestation of toxin invading the abdomen first causes chills (Chi Se) and aversion to cold, mental restlessness, vomiting and regurgitation, rapid breathing, and abdominal fullness; such cases are fatal.
Ehe (Malignant Nodules) refers to sudden nodules appearing within the muscle layers, linked together like plums or peaches, small as bean grains, with dry and painful skin and flesh, wandering left and right within the body, arising abruptly. This is formed by wind pathogens carrying toxins. It also resembles the toxicity of "She Gong" (a disease caused by water-dwelling insects). Initially, the location is irregular, accompanied by sharp pain. If not treated promptly, the toxin invades the abdomen, causing restlessness and aversion to cold, leading to death. If not cured for a long time, it may transform into fistulas or ulcers.
Generally, if swelling is complicated by wind-cold, it will not resolve but instead form nodules.
Qi swelling presents similarly to an abscess but lacks a head, showing hollow swelling without color change. The skin is tense and painful upon slight touch. This is caused by the struggle between wind pathogens and Qi.
Sudden pain in one part of the body, unbearable as if beaten; or pain wandering through the body at specific times. During pain attacks, there is mild fever; when pain subsides, the area feels cold as if covered by ice and frost, hence called Qi pain. This is also due to deficiency of the body allowing wind pathogens to invade, which are then constrained by cold qi, preventing the expulsion of pathogens.
E Mai (Malignant Veins) refers to the sudden appearance of red collateral vessels within the body, protruding and clustered like dead earthworms; they appear to contain fluid, varying in length according to the course of the collaterals. This arises from contracting evil wind during spring and winter, entering the collaterals and causing blood stasis. If untreated for a long time, the collateral constriction may lead to further complications.
E Rou (Malignant Flesh) refers to sudden protrusions of flesh resembling small beans, thin and long, shaped like cow or horse milk, or chicken combs, without itching or pain. If not treated for a long time, they continue to grow. This is caused by evil wind injuring the body in spring, entering the muscles, and accumulating blood stasis.
When swelling is accompanied by intense heat and consolidation, blood transforms into pus. If pus is not drained early, it will erode tendons and rot bones, making the condition incurable.
Once an abscess has ulcerated, pus must be completely drained in a timely manner. If pus remains, it will re-consolidate into swelling, identical to the initial state, making it more difficult to treat.
