After a new recovery from Cold Damage disease (Shanghan Bing), the body fluids have not yet recovered, and Qi and Blood remain deficient. If physical exertion or sexual activity occurs too early, causing the disease to recur, it is termed "Labor Recurrence" (Lao Fu). Mental exertion such as excessive talking or thinking damages Shen (Spirit); physical exertion such as combing hair or bathing consumes strength. Exertion generates heat; this pathogenic heat, taking advantage of the deficiency, re-enters the meridians, causing the disease to recur. If the pulse is deep and tight, purgation is indicated.

Following a new recovery from Cold Damage disease, or after a major illness, the Spleen and Stomach remain weak, and Gu Qi (Grain Qi) has not yet recovered. Consuming pork, intestines, blood products, fatty fish, or greasy foods will inevitably cause severe diarrhea, which is difficult for physicians to treat and may lead to death. Consuming wheat cakes, broomcorn millet, malt sugar, dates, chestnuts, and other dried fruits, as well as hard-to-digest foods, will cause internal heat accumulation due to the weak Stomach Qi's inability to digest them. If purgatives are then administered, the Stomach Qi becomes cold and deficient, leading to uncontrollable severe diarrhea. Without purgation, death is certain; with purgation, the condition remains critical and difficult to save. Many die from this after major illnesses; careful nursing is essential. After a new recovery, only thin rice gruel should be eaten. It is better to eat little and feel hungry than to overeat. Do not consume other foods, even if desired. After some time, when gradually improving, mutton gruel or soup can be introduced, but pork and dog meat must strictly be avoided.

After Cold Damage disease, recurrence often results from unregulated exertion or excessive eating, causing the disease to return. This is named "Recurrence" (Fu). "Fu" means the disease returns as it was initially. This occurs because the meridians are still weak and Qi and Blood are not yet substantial, leading to renewed illness. Preemptive medication and preventive measures are employed to prevent this, hence termed "preventing recurrence."

The disease known as "Yin-Yang Exchange" (Yin Yang Yi) refers to an illness contracted by a man or woman who has newly recovered from Cold Damage disease but whose body has not yet fully stabilized, through sexual intercourse with the opposite sex. If a man recovers and a woman contracts the disease through intercourse, it is called "Yang Exchange" (Yang Yi). If a woman recovers and a man contracts the disease through intercourse, it is called "Yin Exchange" (Yin Yi). No exchange occurs between two men or two women. It is called "Exchange" because Yin and Yang interact, stirring up pathogenic toxins that transfer to the other person, akin to changing clothes. Symptoms include heavy body, urgency and pain in the lower abdomen, sometimes drawing into spasms in the genitalia, hot qi rushing to the chest, heavy head unable to be lifted, visual obscurations, limb stiffness and cramping, lower abdominal pain, and curled hands and feet. These symptoms typically lead to immediate death. In cases where death is not immediate, the patient suffers from lower abdominal urgency, hot qi rushing to the chest, heavy head unwilling to lift, loose joints, weak meridians, deficient Qi and Blood, empty marrow, mental confusion, weak breathing, declining strength, inability to move while bedridden, requiring assistance for daily activities, and death may occur after months.
