Whenever wind and cold are sought for cooling during pregnancy, the uterine collaterals (blood vessels connecting to the uterus) become cold. Upon delivery, the amount of blood discharged will necessarily be scant. Or, if one seeks wind and cold after childbirth, all these cause wind-cold to contend with the blood, resulting in the blood's failure to circulate and dissipate normally. Accumulated within, this leads to postpartum lochia dripping continuously without clearing.
Excessive exposure to wind and cold during pregnancy causes coldness in the uterine collaterals; near delivery, the blood discharge is scant. Or, if lochia has not fully cleared after childbirth and one seeks wind and cold, wind-cold contends with the blood. The blood becomes stagnant and fails to circulate and dissipate, accumulating inside. With cold qi inside contending with the stasis, pain ensues. Severe cases may transform into a blood mass (fixed abdominal lump), also causing amenorrhea.
Postpartum qi deficiency combined with pre-existing cold causes the cold to contend with the blood, leading to blood coagulation and failure to dissipate. If qi rebels upward, blood follows it up to rush against the heart, causing chest pain. Generally, after childbirth, residual blood that remains un-discharged congeals upon encountering cold, contending with qi to cause pain. If exposed to cold again, the blood congealment becomes more severe, transforming into a blood mass, and also causing menstrual obstruction and stagnation.
"Half-birth" refers to the situation where the fetus's bones, joints, and viscera have gradually developed, but birth occurs before the full term. This is mostly caused by overwork or fright/disturbance, or by violating taboos.
After recent childbirth, pain caused by the contention between blood and qi is called "Zha pain" (mass-related pain). "Zha" implies "false," meaning the pain floats and lacks a fixed location. This arises from pre-existing wind-cold and disharmony of blood and qi; due to scant blood discharge at delivery, this condition occurs. If not treated promptly, it often forms accumulations, interfering with menstruation: mild cases cause stagnation and涩 (roughness/stagnation), while severe cases lead to complete obstruction.
Childbirth damages blood and labors qi, making the interstices (pores and muscle spaces) deficient, allowing wind pathogen to invade. Pathogen contending with qi, unable to be excreted, causes false swelling. If the swelling is light and floating like blown air, it is pathogen contending with qi, constituting Qi Edema; if the skin is thin like a ripe plum, it transforms into Water Edema. Qi Edema resolves with sweating; Water Edema improves with promoting urination.
Postpartum visceral deficiency, or pre-existing wind-cold, or new exposure to cold, contends with qi, causing abdominal pain. If qi rebels upward, it also causes heart pain and hypochondriac pain. Over time, this may transform into Shan Zha (hernia-like masses).
