Generally, injuries to the head with brain matter extrusion are difficult to treat; bones appearing greenish in color are also difficult to treat. If flesh and bone are shattered elsewhere on the body, immediately apply Kongtong San externally and internally administer Shufeng Liqi Tang (Wind-Dispersing Qi-Regulating Decoction) for five to six doses. Once the wound has healed, subsequently administer Buxue Shunqi Tang (Blood-Nourishing Qi-Smooth Decoction). If tetanus occurs, presenting symptoms of trismus (lockjaw) and opisthotonos (arched back), urgently administer Feilong Duoming Tang (Flying Dragon Snatching Life Decoction).
If the eye is injured, apply Shouzhu San externally. Dip the tip of a silver hairpin into well water (Jinghua Shui) to moisten the medicine and dab it onto the blood vessels. Subsequently, wash and rub the area with a blue cloth or old blue silk soaked in decoction. Then, take Huanhun Tang (Soul-Returning Decoction) for two doses. After stabilization, take Shengxue Mingmu Yin (Blood-Generating Eye-Brightening Drink).
If the nasal bone is fractured, first apply Jiegu San externally, then apply Shengji San mixed with rapeseed oil topically, followed by taking Huoxue Zhitong San (Blood-Invigorating Pain-Relieving Powder).
For lacerations or defects of the lips, first apply Daitong San. Cover the area with down from quail tails for protection, suture using mulberry branch oil thread, apply Shengji San again, and take Huoxue Zhitong Yin (Blood-Invigorating Pain-Relieving Drink). In cases where a knife was held in the mouth, severing the tongue but it has not yet fallen off, wrap the severed tongue in egg white membrane (Danbaoyi). Mix Xuedan (Blood Cinnabar) with honey and apply to the lesion. Further mix honey and beeswax, apply over the egg membrane, utilizing its softness to facilitate medicinal penetration. Since medicines in the mouth disperse easily, frequent application is advisable. If rapid effect is not achieved, treat with Jinchuang Yao (Metal Wound Medicine). For tongues bitten by others, apply crushed fresh crab externally, which will also lead to recovery.
In cases of mandibular joint dislocation, first fumigate and wash with Kuanjin San decoction. Wrap cotton around the thumb, insert it into the mouth to support the lower part, while supporting externally with the palm. Gently massage and push until the joint reduces. Subsequently, take Bushen Heqi Tang (Kidney-Nourishing Qi-Harmonizing Decoction). For bilateral acromial injuries from falls, which are difficult to bandage, if swelling is observed, first administer Chuanqi Tang (Panting-Qi Decoction) to align the joints, then apply Jiegu San externally, wrap in cotton, and apply obliquely across the chest and back. Follow with Huoxue Tang (Blood-Invigorating Decoction).
Shoulder dislocations are similar to knee dislocations, though reducing the knee is easier than the shoulder, both are generally reducible. Press one hand on the shoulder, support the arm with the other, gently shake to relax tendons and promote circulation. Have the patient sit low, have an assistant hold the body, while the physician pinches the shoulders,夹s the arms between knees, and exerts upward force to reduce. Wrap in cotton, apply external bone-setting medicine, and internally administer Shengxue Busui Tang (Blood-Generating Marrow-Nourishing Decoction).
For elbow dislocation, lift the wrist with one hand above, press the hand below, and brace against the ankle internally. Exert force to extend and reduce. Apply Jiegu San, wrap in cotton, and internally administer Shengxue Busui Tang.
For wrist dislocation, press the glenoid fossa with one hand above, support the palm and fingers with the other, and extend forcefully to reduce. This method suits the Huimai pulse. Take Kuanjin Huoxue San (Tendon-Relaxing Blood-Invigorating Powder), apply Jiegu San externally, wrap in cotton, and secure with a wide board and four 2-inch fir boards for seven days to achieve healing.
Fingers have three joint sockets; only the middle phalanx dislocation presents this issue, but it is easy to reduce. Use the two-hand traction method to reduce, and take Kuanjin Huoxue Tang (Tendon-Relaxing Blood-Invigorating Decoction). In cases of elbow dislocation protruding into the abdomen, have another person lie on their side to expose muscle. Guide the limb outwards and upwards, pressing against the waist, holding the wrist, and bracing with the knee. Pull left-to-right or right-to-left, extending to reduce. Take Shengxue Busui Tang.
Legs are prone to injury, often resulting in double fractures. First fumigate and wash with Kuanjin San. Place the patient on their affected side, apply Jiegu San externally to both legs, wrap in cotton, and secure with eight 4-inch fir boards bound by three cotton ropes. Take Huoxue Zhitong San for three to four doses, then Zhuangjin Xugu Dan (Tendon-Strengthening Bone-Continuing Pill).
For patellar dislocation, bundle cotton clothes into a large pad. Have the patient lie supine with the pad under the knee. Lift the ankle; if the bone shifts left, guide it down left, and vice versa. Stabilize the pad, grasp the knee with the upper hand, press the foot with the lower hand, align the joint socket, and forcefully lever it back into place. Apply Jiegu San, cover with padded cloth, press with the pad, take Shengxue Busui Tang for five doses, then Zhuangjin Xugu Tang (Tendon-Strengthening Bone-Continuing Decoction).
The lower leg has two bones; a single fracture is easy to treat, while both being fractured is difficult. Fractures resembling "lotus root segments" (seven-headed break pattern) are easier to treat. Flat breaks are difficult. Penetrating wounds through skin and flesh are particularly difficult. Treatment follows the same method as thigh fractures. If bone pierces the skin, realign fragments without washing with water, and apply Shengji San. If skin is intact, apply Jiegu San. Ankle joint dislocations are easy to dislocate but hard to reduce. Lift the foot with heels up, pull toes, adjust direction based on displacement, lift heel down, do not simply extend. Internally take Kuanjin Huoxue San. For sprains causing tendon spasm and pain limiting movement, wash with Kuanjin San decoction.
For stiff neck (Shizhen), have the patient sit low. Grasp the chin with one hand and slowly straighten the neck.
For stab wounds from knives or spears affecting only skin and flesh, apply Jinchuang Yao. Injuries penetrating internal membranes or intestines are incurable. Head/forehead cuts from axes/knives may cause chills and fever; fine pulse indicates treatability, large pulse indicates difficulty. Treat bone first, then flesh. Apply Jinchuang Yao externally, take Hufeng Tuoli San (Wind-Protection Interior-Supporting Powder) internally. For self-inflicted esophageal severance, first apply fine powder of Banxia (Pinellia Rhizome), then apply mixture of quail down and Ginseng powder, suture with mulberry bark oil thread, and bind with soft silk. If quail down is unavailable, use softened cattail flower stamens. Administer Zijin Dan (Purple Gold Pill) mixed with fetal bone powder and wine via spoon, followed by Hufeng Tuoli San. For abdominal rupture with evisceration, remove patient's fingernails first. If intestines are exposed and food can be ingested, quickly position a spinning wheel near the wound to prevent wind exposure. Wash with warm water to reduce intestines, suture with mulberry bark thread, apply Jinchuang Yao externally, and take Tongchang Huoxue Tang (Intestine-Unblocking Blood-Invigorating Decoction).
For torture by wooden clamps, upon release, soak feet in a basin of hot child's urine. If urine cools, heat red-hot bricks and quench them in the urine until a white oily film floats on the surface, preventing ulceration. Mash soap into mud, mix with egg white, apply to wound, wrap in grass paper, and bind tightly with a foot strap. Leave undisturbed overnight for effect. Internal herbal remedies are listed later.
