Postpartum management should primarily focus on greatly tonifying Qi and Blood, as tonifying Qi and Blood is equivalent to tonifying Yin and Yang. However, postpartum treatment cannot simply involve blind tonification of Qi and Blood without distinguishing between conditions of Excess Yang with Deficient Yin or Excess Yin with Deficient Yang, nor can one indiscriminately use tonic formulas. For instance, if a patient presents with postpartum fever and fainting due to blood loss (Xue Yun), this is caused by Qi deficiency failing to generate Blood. In such cases, not only should Blood be nourished, but it is particularly appropriate to tonify Qi. Using Ren Shen (Ginseng Radix) as the Sovereign herb, assisted by Dang Gui (Angelicae Sinensis Radix), Chuan Xiong (Ligustici Chuanxiong Rhizoma), and Jing Jie (Schizonepetae Herba) yields excellent results. Similarly, if postpartum abdominal pain due to lochia stasis (Er Zhen Tong) causes tenderness upon touch accompanied by fainting due to blood loss, this indicates that while Blood and Qi are abundant, Yang Qi is deficient and unable to promote circulation. The aforementioned formula can be used with the addition of ten Hawthorn fruits (Shan Zha) to achieve efficacy. Only in cases of excessive blood loss where only faint Qi remains, or when alternating chills and fever occur, must one greatly tonify Blood with a lesser amount of Qi tonification being appropriate. The same decoction may be employed, using Dang Gui and Chuan Xiong as Sovereign and Minister herbs, and Ren Shen and Jing Jie as Assistant and Envoy herbs; it is not uncommon for one dose to bring about success. Postpartum diseases have their own specialized studies; I mention only the general principles because generally, those with Yin Deficiency feel heavy and lethargic at night, while those with Yang Deficiency feel weak during the day. Medication based on these characteristics is more suitable. You may further expand upon this.
The distinction between Yin and Yang in matters of fertility was established by the Heavenly Master and Zhang Zhongjing in ancient times, and later elucidated by Lei Gong, so there is no need for me to reiterate. However, why do I speak of it again? Because an imbalance of Yin and Yang ultimately hinders conception; merely taking warming and tonifying substances is of little benefit. One must understand that for Yang Deficiency, one tonifies Yang, and for Yin Deficiency, one tonifies Yin, thereby achieving balance between Yin and Yang, which is the path to having children. For example, experiencing abnormal sexual arousal upon seeing a person or color, weeping upon seeing a door, becoming aroused during intercourse but suddenly losing erection, or desiring another act but failing to maintain erection—these are undoubtedly signs of declining Yang Qi. The formula Fu Ruo Wan (Support Weakness Pill) is prescribed to assist, taken daily with wine, six qian or one liang per dose. After three days, sexual function will revitalize, no longer remaining as debilitated as before. However, during these three days, if the partner does not restrain sexual desire, the miraculous effect of my prescription will manifest; if one violates this prohibition, only half assistance can be expected, and the condition cannot be fundamentally changed. This is not due to the imperfection of the transmitted formula. Those with Yin Deficiency unable to conceive must not take this medicine. Yin Deficiency inevitably leads to Fire Hyperactivity; excess Fire depletes Water, scorching and drying up Yin Essence, resulting in scant ejaculation, or excessive heat disturbing the Uterus, or insufficient fluids making ejaculation difficult, or night sweats with bone-steaming fever leading to Yin collapse—all of these prevent conception. I now establish a formula, prepared according to method, taken throughout the day, which will surely enable conception. Taking five qian or one liang morning and evening, or even more, is beneficial. Within three months to half a year, there will be no failure to conceive. Both formulas possess profound logic and unique merits; the key lies in distinguishing Yin and Yang to serve as the "Jade-Planting Elixir."
