The differentiation of true and false cases of hematemesis (vomiting blood) and epistaxis (nosebleed) has been mentioned in the preceding text, yet the distinction between true and false has not been fully elaborated. "True" does not refer to the debate on cold and heat syndromes; "false" does not refer to the debate on deficiency and excess syndromes.
There are cases where sudden epistaxis flows like a gushing spring due to falls or collisions; this is false epistaxis, not caused by internal injury. There are cases of hematemesis resulting from beatings, with blood flowing profusely like an overturned basin; this is also false hematemesis, not true internal injury hematemesis. This differentiation of true and false differs from the previous discussion on yin-yang and deficiency-excess. Although both conditions are identical in that they stem from external trauma and their treatment principles should be similar, they cannot be treated identically. This is because nasal injury from falling involves pathology in the upper jiao, whereas hematemesis from beating involves the upper, middle, and lower jiao. I have now formulated two prescriptions: one for treating epistaxis caused by nasal trauma from falls, named Bujin Dan (Tonify Metal Pill), which stops bleeding after one dose and prevents recurrence after a second dose. The efficacy of this prescription lies in its entry into the Lung meridian and its ability to ascend to the nasal orifices, allowing injured tissues to heal and ruptured orifices to close again. It also generates and tonifies blood to greatly benefit Lung Metal (the Lung organ in Five Element theory), thereby achieving miraculous results. For hematemesis caused by beating, the prescription is named Husun Qidan (Extraordinary Pill for Protecting Injuries). This prescription uses Danggui (Angelica Sinensis Root) and Baishao (White Peony Root) to generate blood, and Dahuang (Rhubarb Rhizome) to dispel blood stasis. Once the stasis is removed, new blood is generated; once new blood is generated, the stasis ceases. This indeed possesses miraculous power. Both prescriptions are excellent for rescuing traumatic injuries, not merely for these two specific conditions.

Mania (Frenzy) also has true and false forms. Although deficiency and excess can encompass them, true and false are not synonymous with deficiency and excess. Some individuals experience sudden confusion, delirious speech, and hallucinations of seeing ghosts; this is Phlegm confusing the Heart orifices, not Fire-Toxin entering the Heart. Is this not false mania? If treated as true mania, the patient will die. Furthermore, some individuals who are frustrated may initially engage in chaotic discussions, distorting right and wrong, and criticizing others, originally intending to vent their pent-up resentment. Over time, this develops into mania, where they curse their wives and children upon seeing them, accuse officials when meeting them, and even run naked year-round without stopping. This is a condition where false mania evolves into true mania, which cannot be compared to acute episodes of climbing high and singing, discarding clothes and running, or jumping into water. Such diseases can only be treated by addressing the mania itself and must not be treated by purging Fire. If treated as Fire-driven mania, the patient will die instantly. Therefore, I have established a separate category to discuss this extensively. I specifically transmit one prescription that treats both conditions, named Shikuang Dan (Release Mania Pill). If the patient refuses to take the medicine, two people must hold his hands, one person must embrace his body, one person must pry open his teeth, and another must administer the medicine. After ingestion, the patient will inevitably curse loudly. After a long while, he will become physically exhausted; after a longer while, he will lie down. Allow him to sleep naturally; do not startle him awake. The previous symptoms will vanish instantly. He will then ask for half the dosage of the medicine. Administer two more doses, and recovery will be complete. Truly miraculous.
