A person suffering from extreme heat causing mania, climbing high and shouting, discarding clothes and running, with panting and sweating profusely like rain, this is due to the fire of the Yangming Stomach Meridian. Why can the fire of Yangming cause one to climb high and shout? Because the nature of fire is to flare upwards; when internal fire is blazing, the body naturally feels restless and soaring. Heat stagnates in the chest, and shouting allows Qi to dissipate. Clothes are meant to cover the body; when internal heat is intense, covering the body with clothes feels like being burned, so discarding them brings relief, why care about anything else? Fire punishes Metal (Lung), naturally causing severe panting; when panting reaches its extreme, Lung Metal is injured and cannot protect the skin and hair, pores open, Yin fails to control Yang, forcing fluids out with an unstoppable momentum. Once sweat is exhausted, the Heart loses nourishment, and Spirit is about to fly away and scatter, how can one not become manic? The prescription used is Modified Baihu Tang (White Tiger Decoction) to save the condition.
Ren Shen (Ginseng Radix et Rhizoma) (2 liang), Shi Gao (Gypsum Fibrosum) (3 liang), Zhi Mu (Anemarrhenae Rhizoma) (5 qian), Fu Ling (Poria) (5 qian), Mai Dong (Ophiopogonis Radix) (3 liang), Gan Cao (Licorice Root) (1 qian), Ban Xia (Pinelliae Tuber) (3 qian), Zhu Ye (Bambusae Folium) (200 leaves), Nuo Mi (Glutinous Rice) (a pinch).
Decoct in water and take orally. After one dose, mania stabilizes; after two doses, heat stops. Do not use three doses.
This syndrome requires Baihu Tang to urgently clear Stomach fire; otherwise, Kidney Water will be boiled dry immediately, turning the body into black charcoal. However, a prairie fire cannot be extinguished by a cup of water; it requires torrential rain to completely put out flames across mountains and fields.
This syndrome can also be treated with Kan Shui Tang.
Shi Gao (Gypsum Fibrosum) (1 liang), Xuan Shen (Scrophulariae Radix) (2 liang), Gan Cao (Licorice Root) (1 qian), Tian Hua Fen (Trichosanthis Radix) (3 qian), Chao Zhi Zi (Gardeniae Fructus, stir-fried) (3 qian), Che Qian Zi (Plantaginis Semen) (2 qian).
Decoct in water and take orally. One dose stabilizes mania; two doses result in full recovery.
Another person suffers from mania caused by fire, with abdominal fullness preventing lying down, red face, heart heat, hallucinations, and delirious speech as if seeing ghosts. This is also due to excessive fire in the Yangming Stomach.
However, Stomach fire is a Yang syndrome, while hallucinations and seeing ghosts resemble Yin syndromes. Why? Excessive Yangming fire stems from excessive Pericardium fire.
Yangming belongs to Yang, while the Pericardium belongs to Yin. When fire in both the Pericardium and Yangming moves together, abdominal fullness and inability to lie down occur. If only Stomach fire moves without Pericardium fire, despite thirst and fullness, lying down is still possible. Only when the Pericardium assists Stomach fire does Heart Spirit float outward, allowing Yin Qi to invade, creating illusions and delirious speech, resembling seeing ghosts though none exist. The treatment should clear Stomach fire but need not clear Pericardium fire. Since the Stomach is the "Son" (element generated by) of the Pericardium, and the Pericardium is the "Mother" (element generating), when the Mother is excess, the Son becomes prosperous; yet if the Son declines, the Mother weakens too. Clearing Stomach fire is effectively clearing Pericardium fire. The prescription is Xie Zi Tang (Draining Son Decoction):
Xuan Shen (Scrophulariae Radix) (3 liang), Gan Ju Hua (Chrysanthemi Flos) (1 liang), Zhi Mu (Anemarrhenae Rhizoma) (3 qian), Tian Hua Fen (Trichosanthis Radix) (3 qian).
Decoct in water and take orally. One dose calms Stomach fire; two doses calm Pericardium fire. With both fires calmed, mania heals itself.
Theoretically, Baihu Tang could be used, but I dislike its overly harsh nature, so I use Xie Zi Tang instead. Since the Pericardium belongs to Yin, using Baihu Tang to drain Yang heat inevitably damages Yin. Xie Zi Tang drains Yang without harming Yin, which is superior. Some ask: If Mother excess leads to Son prosperity, shouldn't we drain Pericardium fire? Why drain Stomach fire (the Son)? It is unknown that among all organs, Stomach fire is the most fierce. Once Stomach fire blazes, Kidney Water dries instantly. Thus, saving Stomach fire first causes Pericardium fire to subside. If one drains Pericardium fire first, cold-natured herbs must pass through the Stomach to reach the Pericardium ("borrowing a path to destroy Guo"), which might anger Stomach fire further. Directly draining Stomach fire controls both Yang and Yin, achieving mutual benefit.
Er Shi Tang (Two Stones Decoction) is also miraculous for this syndrome.
Ren Shen (Ginseng Radix et Rhizoma) (5 qian), Shi Gao (Gypsum Fibrosum) (5 qian), Han Shui Shi (Alumen) (2 qian), Fu Ling (Poria) (3 qian), Ban Xia (Pinelliae Tuber) (2 qian), Dan Pi (Moutan Cortex) (5 qian).
Decoct in water and take orally. One dose stabilizes mania; two doses result in full recovery.
A person easily laughs and smiles, speaks deliriously and wildly, with scattered Heart Spirit and visual hallucinations. People suspect Stomach heat, but this is actually Heart heat. Heart heat causes mania; why do the San Jiao (Triple Burner) and Pericardium fail to guard? Because the Heart is excessively hot, the Pericardium and San Jiao dare not command on behalf of the Heart, obeying the Heart's autonomy leading to uncontrolled laughter. Like a tyrannical ruler where ministers dare not advise, sycophants surround him, bringing pleasure and amusement, inevitably leading to excessive joy and laughter. Orders issued disrupt governance; when orders fail, chaos ensues like a ghost realm. Similarly, Heart heat causes Spirit to flee. Why not die immediately? Heart heat mania differs from Stomach heat mania. Stomach heat invades the Heart from outside; Heart heat arises from internal disorder. Stomach mania risks sudden death, while Heart mania allows lingering survival. Treatment must focus on clearing the Heart; once the Heart is clear, mania settles. The prescription is Qing Xin Dan (Clear Heart Pill):
Huang Lian (Coptidis Rhizoma) (3 qian), Fu Shen (Poria with Hostwood) (5 qian), Sheng Suan Zao Ren (Ziziphi Spinosae Semen, raw) (5 qian), Ren Shen (Ginseng Radix et Rhizoma) (3 qian), Mai Dong (Ophiopogonis Radix) (1 liang), Xuan Shen (Scrophulariae Radix) (1 liang), Dan Shen (Salviae Miltiorrhizae Radix et Rhizoma) (3 qian).
Decoct in water and take orally. One dose calms Spirit; two doses stabilize mania. No third dose needed.
Huang Lian clears Heart fire, but using it alone may aggravate dryness due to its hot nature. Therefore, add Ren Shen, Dan Shen, and Mai Dong to moisten and balance. Fire excess implies Qi deficiency; tonifying Qi to drain fire protects the Heart Monarch, keeping it quiet.
Jie Wang Tang (Resolve Delusion Decoction) is also effective.
Ren Shen (Ginseng Radix et Rhizoma) (1 liang), Huang Lian (Coptidis Rhizoma), Fu Shen (Poria with Hostwood), Bai Zi Ren (Biota Seed), Xuan Shen (Scrophulariae Radix), Dan Shen (Salviae Miltiorrhizae Radix et Rhizoma) (each 3 qian), Sheng Suan Zao Ren (Ziziphi Spinosae Semen, raw) (5 qian), Gan Cao (Licorice Root) (1 qian), Rou Gui (Cinnamon Bark) (2 fen).
Decoct in water and take orally. One dose stabilizes mania; two doses result in full recovery.
A person has body heat and mania, speaking lewd words, desiring pleasure. Any contradiction triggers sudden wild rage, seeing spirits and ghosts. People think it is extreme Heart heat, but it is actually Pericardium heat. The Pericardium is the deputy of the Heart Monarch. If the Heart is quiet, why allow the Pericardium to be so rebellious? This is because the Ruler is weak and the Minister strong; the Heart is extremely cold and lacks autonomy. Like a weak ruler delegating power to a corrupt minister who manipulates life and death, praising compliance and punishing dissent, reversing right and wrong, causing suspicion and hallucinations. This resembles Pericardium heat mania. Treatment should drain Pericardium fire. However, treating only the Pericardium while the Heart remains cold risks shock. One must tonify the Heart to strengthen Qi before calling upon external forces to clear the "rebel." Simply draining Pericardium fire risks rebellion against the Heart. The prescription is Wei Zhu Tang (Guard the Master Decoction):
Ren Shen (Ginseng Radix et Rhizoma) (1 liang), Fu Ling (Poria) (5 qian), Xuan Shen (Scrophulariae Radix) (1 liang), Tian Hua Fen (Trichosanthis Radix) (3 qian), Mai Dong (Ophiopogonis Radix) (5 qian), Sheng Di Huang (Rehmanniae Radix) (5 qian), Dan Pi (Moutan Cortex) (3 qian).
Decoct in water and take orally.
Xuan Shen, Sheng Di Huang, and Dan Pi clear Pericardium fire, while Ren Shen, Fu Ling, and Mai Dong tonify the Heart. A strong Heart makes Pericardium fire weak. Although these herbs drain Pericardium fire, they also tonify the Heart, turning chaos into order. Some argue: If the Heart is cold-deficient, Ren Shen is appropriate, but cooling herbs like Xuan Shen and Dan Pi might increase Heart cold, suggesting hot herbs are needed. Alas, cold Heart uses hot herbs normally. But with Pericardium fire excess, hot herbs must pass through the Pericardium, potentially fueling its flame rather than benefiting the Heart. Ren Shen helps the Heart and is acceptable to the Pericardium. Combined with Xuan Shen, it guards the Heart, directing other herbs to clear heat, transforming cold to warmth without needing hot herbs.
Zheng Xin Tang (Rectify Heart Decoction) is also miraculous.
Ren Shen (Ginseng Radix et Rhizoma), Shu Di Huang (Rehmanniae Radix Praeparata) (each 1 liang), Xuan Shen (Scrophulariae Radix), Mai Dong (Ophiopogonis Radix) (each 2 liang), Chang Pu (Acori Tatarinowii Rhizoma) (1 qian), Bai Jie Zi (Sinapis Semen) (3 qian).
Decoct in water and take orally. One dose lightens symptoms; two doses cure.
A person insulted by bullies feels resentful and develops Heart mania, holding knives, jumping over walls, screaming with disheveled hair. People attribute this to Yangming Stomach fire, but it is actually Yangming Earth deficiency. Yangming fire excess usually stems from Heart fire excess. Heart fire generates Stomach fire (Fire generates Earth); if Heart fire declines but Stomach fire rises, it indicates Earth failing under Fire. Fire generating Earth keeps Stomach stable; Earth failing Fire leads to Stomach pathology. Though resembling true fire excess, the root is gone, leading to collapse. Mania is generally heat, but I consider this deficient heat, not excess heat. Visceral excess heat is cooled; deficient heat requires warming guidance. However, Yangming deficient heat cannot be fully warmed. Warm middle jiao with slight cold herbs is best. Yangming deficient heat is internal injury, not external invasion. Resentment generates heat differently than pathogenic invasion. Pathogenic heat is excess; righteous heat is deficient. The prescription is Ping Re Tang (Calming Heat Decoction):
Ren Shen (Ginseng Radix et Rhizoma) (5 qian), Huang Qi (Astragali Radix) (1 liang), Gan Cao (Licorice Root) (1 qian), Mai Dong (Ophiopogonis Radix) (1 liang), Huang Qin (Scutellariae Radix) (1 qian), Qing Pi (Citri Reticulatae Pericarpium Viride) (5 fen), Zhu Li (Bambusae Sap) (1 he), Bai Shao (Paeoniae Radix Alba) (5 qian), Fu Ling (Poria) (3 qian), Zao Ren (Ziziphi Spinosae Semen) (3 qian), Chao Zhi Zi (Gardeniae Fructus, stir-fried) (5 fen), Tian Hua Fen (Trichosanthis Radix) (3 qian), Chai Hu (Bupleuri Radix) (5 fen).
Decoct in water and take orally. Two doses lighten mania; four doses stabilize it; one month of use ensures peaceful sleep.
This formula modifies Zhuye Shigao Tang (Bamboo Leaf and Gypsum Decoction) to treat Yangming deficient heat. Sweet-warm herbs reduce great heat, assisted by sweet-cold herbs to harmonize Yangming fire, strengthening Earth within the "fire house" and eliminating smoke. With rooted Earth, fire subsides, and mania disappears. Using Zhuye Shigao Tang for excess heat would be fatal.
Shu Fen Tang (Soothe Resentment Decoction) is also miraculous.
Bai Shao (Paeoniae Radix Alba) (2 liang), Chao Zhi Zi (Gardeniae Fructus, stir-fried) (5 qian), Xuan Shen (Scrophulariae Radix) (1 liang), Tian Hua Fen (Trichosanthis Radix) (3 qian), Chai Hu (Bupleuri Radix) (1 qian).
Decoct in water and take orally. One dose stabilizes mania; two doses cure; three doses ensure full recovery.
A person starving and overworking suddenly becomes manic, disheveled and naked, unaware of shame. People think it is loss of mind, but it is Stomach injury stirring fire. Stomach belongs to Yangming; Yangming fire movement is hard to stop. Worldly wisdom says Stomach fire should be drained, not tonified. Excess Stomach can be drained; deficient Stomach cannot. Classics state: Diseases of Er Yang (Stomach) originate in Heart and Spleen. Stomach is the Sea of Water and Grain, capable of containing food. Food digests in Stomach, nourishing it; food nourishes Stomach so fire rests; Stomach lacking food causes fire to move. When fire moves and Stomach Earth collapses, it seeks help from Heart and Spleen. Heart sees boiling Stomach fire, feeling pain, becoming disturbed. Spleen sees burning Stomach fire, fearing neighbor's disaster, becoming chaotic and losing grounding, leading to mania. Treatment need not calm Heart Spirit or stabilize Spleen Intent directly, but must save Stomach Qi. Saving Stomach Qi requires saving Stomach Earth; if Stomach fire is not suppressed, Stomach Qi cannot survive alone. The prescription is Jiu Fen Liao Wei Tang (Save from Burning and Heal Stomach Decoction):
Ren Shen (Ginseng Radix et Rhizoma) (1 liang), Xuan Shen (Scrophulariae Radix) (1 liang), Zhu Li (Bambusae Sap) (1 he), Chen Pi (Citri Reticulatae Pericarpium) (3 fen), Shen Qu (Massa Medicata Fermentata) (5 fen), Shan Yao (Dioscoreae Rhizoma) (5 qian), Bai He (Lilii Bulbus) (5 qian).
Decoct in water and take orally. One dose stabilizes mania; two doses stop it; three doses cure.
This formula heavily uses Ren Shen to save Stomach Earth, combined with Xuan Shen to suppress Stomach fire, and other herbs to mediate between Heart, Lung, Spleen, and Kidney, preventing Liver from attacking Stomach Earth, facilitating Stomach Qi recovery. Once Stomach Qi turns, Stomach injury heals. With no Stomach injury, Heart Spirit and Spleen Intent remain undisturbed, making mania easy to settle.
E Huo Tang (Extinguish Fire Decoction) is also miraculous.
Ren Shen (Ginseng Radix et Rhizoma), Bai Zhu (Atractylodis Macrocephalae Rhizoma), Sheng Di Huang (Rehmanniae Radix) (each 5 qian), Xuan Shen (Scrophulariae Radix) (1 liang), Gan Cao (Licorice Root) (1 qian), Zhi Mu (Anemarrhenae Rhizoma) (1 qian), Tian Hua Fen (Trichosanthis Radix) (2 qian), Chen Pi (Citri Reticulatae Pericarpium) (5 fen), Shen Qu (Massa Medicata Fermentata) (1 qian), Dan Pi (Moutan Cortex) (5 qian).
Decoct in water and take orally. One dose stabilizes mania; two doses cure.
