Case 2: Wife of Yan XX, suffering from a warm disease, severe heat toxin. Rash appeared after six days, spots clustered densely. Head and face swollen like "Big Head Plague" (Da Tou Wen), sore throat and swelling, unable to swallow food, unconsciousness and convulsions. At the time of diagnosis, it was the tenth day (fourth day since rash appeared). Due to inability to swallow medicine because of throat pain, imitating the intention of Wu Zhi Yin (Five Juice Drink), prescribed Pear Juice 30g, Water Chestnut Juice 30g, Sugarcane Juice 30g, Qing Hao Lu (Qinghao Dew, Artemisia Annua Dew) and Yin Hua Lu (Yinhua Dew, Honeysuckle Dew) 15g each, swallowed slowly. Subsequently administered Liu Shen Wan (Six Divine Pills) five pills, moistened with warm water, placed on the tongue surface, held until dissolved and swallowed with saliva. After swallowing the pills twice, continued frequently consuming the five juices. Next day's follow-up: Sore throat and swelling alleviated, breathing勉强 possible. Although head and face rash spots are deep red, there is a sign of recovery in vital qi. Plan to first dissolve and administer one pill of Zhi Bao Dan (Treasure of Ultimate Efficacy Pill). Another prescription: Sheng Di Zhi (Rehmanniae Radix Preparata, Prepared Rehmannia Root Juice) 30g, Da Huang (Rhei Radix et Rhizoma, Rhubarb Root) 3g, Xi Jiao (Cornu Rhinocerotis, Rhino Horn) and Ling Yang Jiao (Cornu Saigae Tataricae, Saiga Antelope Horn) 1.5g each ground into juice, Xian Pu Gong Ying (Taraxaci Herba Recens, Fresh Dandelion Herb) 60g crushed into juice, Zhu Li (Nodus Bambusae in Taedia, Bamboo Sap) 24g mixed evenly, warmed slightly, divided into three doses. In between, still use the previous five juices as tea. Two days later follow-up: Throat swelling gradually subsided, consciousness gradually cleared. Still using the previous formula minus Zhi Bao Dan, increase Sheng Di to 45g, increase Da Huang to 4.5g. After medication, had a smooth bowel movement once. Two days later follow-up: Upper body rash spots gradually resolving, spirit clear and calm, able to consume thin congee. Use Gui Di Yang Ying Tang (Tang Gui Di Yang Ying Decoction, Decoction for Nourishing Blood and Yin with Angelica and Rehmannia) combined with Zhu Ye Shi Gao Tang (Decoction of Bamboo Leaves and Gypsum) removing Ren Shen (Ginseng Root) and Ban Xia (Pinelliae Rhizoma, Pinellia Tuber), Mai Mai Gan Jiao Tang (Decoction for Restoring the Pulse) removing Sheng Jiang (Zingiberis Rhizoma Recens, Fresh Ginger) and Rou Gui (Cinnamomi Cortex, Cinnamon Bark) to conclude treatment and achieve cure. (From *Ran Xuefeng Medical Cases*)
[Commentary]: Hou Zhen (Throat Rash), also known as Yi Hou Zhen (Epidemic Throat Rash), refers to a syndrome caused by epidemic toxic pathogens entering through the mouth and nose, where heat toxin attacks the throat due to the interaction of accumulated heat in the Lung and Stomach. Treatment should employ agents that disperse, detoxify, clear heat, and generate fluids. In Case 1, at the initial consultation, the condition was changing rapidly; quickly administering exterior-dispersing and heat-clearing agents allowed the pathogenic heat to retreat, followed by clearing heat and nourishing yin to conclude treatment. In Case 2, the heat toxin was severe, and medicine could not be swallowed. Ye Tianshi said, "When heat is extreme and rejects medicine, merely using decoctions is useless." This case began by imitating Wu Zhi Yin (Five Juice Drink), administering sweet, moistening, cool, and clear agents, akin to receiving jade liquid from heaven, thereby easing the pathological mechanism. Furthermore, since the rash spots appeared late, if heavy interior-clearing agents were used abruptly, internal attack would surely cause complications; physicians must be cautious. Subsequent use of methods such as clearing heat and detoxifying, benefiting the throat and reducing swelling, and nourying yin led to recovery.

External blow: Hou Ke Niu Huang San (Throat Department Buffalo Calves Bile Powder), external application of Fu Rong Kan Gao (Confederate Rose Patch) on right jaw. After four doses of medicine, fever subsided but swelling did not disappear. Daily aspiration of pus, small amount, only about 7ml each time. October 16: Due to local high swelling and soft top not appearing, stool still dry and constipated, added Dan Pi 4.5g and Xuan Ming Fen (Natrii Sulfas, Mirabilite) 9g (dissolved) to original formula. October 17: Defecated twice, swelling and pain reduced, slight pus aspirated. Internal medicine formula only reduced Xuan Ming Fen to 4.5g, take another 2 doses. November 18-19: Throat swelling gradually receded, swallowing slightly easier, jaw lump also subsided, only right ear has slight distension sensation, situation continues to improve. Administered heat-clearing, swelling-reducing, throat-benefiting agents. Right throat abscess swelling diffuse and not retracting, although signs of suppuration exist, root is hard and not soft, pulse fine and rapid, coating greasy and rough. This is due to unclear internal heat, phlegm-stasis condensing and not transforming. Again use bitter descending agents to reduce swelling and transform phlegm-heat. (Zhang Zanchen treated, Chen Zhicai organized: Syndrome Differentiation and Treatment of Throat Abscess, *Chinese Journal of Traditional Chinese Medicine* 9:11, 1964)
[Commentary]: Hou Yong (Throat Abscess) is a general term for sores and ulcers occurring in the throat and nearby areas. Equivalent to diseases such as pharyngeal abscesses. Case 1 syndrome belongs to Stomach fire rising and phlegm-heat accumulating internally. Urgently drain heat and reduce swelling, "valuing resolution," but if resolution fails, promote early suppuration, puncture and drain pus, toxins expelled with pus leads to cure. Case 2 initially used heat-clearing, detoxifying, and swelling-reducing agents, combined with puncture and aspiration, but root remained hard and not soft, due to unclear internal heat and phlegm-stasis condensing and not transforming. Continued with bitter descending agents to reduce swelling and clear-transform phlegm-heat, leading to cure, and provided substitute tea formula to clear residual heat and prevent recurrence. Dr. Zhang's prescriptions are apt, and medicinal usage is distinctive.

Case 2: Sun XX, 20 years old. Suffering from Diphtheria, repeatedly treated, mostly modifying formulas from *Bai Hou Ji Biao Jue Wei* (Selecting Subtleties on Avoiding Exterior Release in Diphtheria), condition worsening daily. Physicians claimed untreatable, then invited me for consultation. Throat gate purely white, excessive sticky saliva, filling mouth instantly, must be spat out. Pulse fine, weak, and rapid; tongue fat, tender, pale red. Know this is deficiency of both Spleen and Kidney. Kidney deficiency failing to control Qi transformation, causing Yin fire to counterflow, phlegm-water flooding up; Spleen earth deficient, unable to control it, thus throat swelling, pain, and excessive sticky saliva. Administer Liu Wei Di Huang Tang (Six-Ingredient Pill with Rehmannia Decoction) to tonify Spleen and Kidney to clear deficient fire, additionally add Bai Zhu (Atractylodis Macrocephalae Rhizoma, Atractylodes Rhizome), add a little Su Zi (Perillae Fructus, Perilla Fruit) to restrain phlegm-water flooding. Prescription: Da Shu Di (Rehmanniae Radix Preparata, Prepared Rehmannia Root) 18g, Huai Shan Yao (Dioscoreae Rhizoma, Yam Rhizome) 12g (crushed raw), Shan Yu Rou (Corni Fructus, Cornelian Cherry Fruit) 6g, Yun Fu Ling (Poria, Poria Cocos) 9g, Fen Dan Pi (Moutan Cortex, Tree Peony Bark) 4.5g, Fu Ze Xie (Alismatis Rhizoma, Alisma Rhizome) 4.5g, Sheng Yu Zhu (Atractylodis Macrocephalae Rhizoma, Atractylodes Rhizome) 4.5g, Su Zi (Perillae Fructus, Perilla Fruit) 2.4g. Take continuously for 10 doses and cured. (From *Reprinted Compilation of Verified Cases by Famous Doctors Nationwide*, Case of Zhang Xichun)
[Commentary]: Bai Hou (Diphtheria) refers to "Epidemic Throat" capable of mutual transmission. *Chong Lou Yu Yue* states: "The symptom of white rot appearing in the throat is extremely dangerous... Once mistreated, it becomes incurable." This disease is caused by invasion of pestilential evil and epidemic toxins, combined with prevailing dry air of the season, internally resulting in Lung-Kidney Yin deficiency and accumulation of heat in intestines and stomach. Case 1 Lung Yin severely injured, Stomach heat blazing. Urgently administer large dose of Yang Yin Qing Fei Tang (Decoction for Nourishing Yin and Clearing the Lung) to generate fluids and moisten lungs, assist with clearing heat and promoting bowel movement, allowing accumulated heat in Lung and Stomach to descend, then add detoxification to clear remaining pathogens, finally tonify origin and consolidate foundation to conclude treatment. Additionally, Diphtheria caused by dry-heat is common, and those triggered by exterior pathogens are also frequent. Case 2 repeatedly treated, showing signs of Spleen-Kidney deficiency and counterflow of Yin fire. Method uses tonifying Spleen and Kidney, clearing deficient fire, restraining phlegm-water to achieve cure. Indeed, excess fire is easy to clear but deficient fire is difficult to pacify; failing to distinguish deficiency and excess leads to error.
