During pregnancy, if the bladder is displaced causing inability to urinate, the symptoms vary by cause. Some are due to weak constitution, some to excessive worry and depression, some to irritable temperament, some to rich and strong dietary flavors, and some to holding urine for too long. All these conditions can be resolved using diuretic or promoting urination medications. Upon reflection, the bladder does not displace itself; it is compressed by the fetus, shifting to one side, causing the ligaments connecting the bladder to become twisted and obstructed. When standing, urine is excreted; when squatting, it cannot. If the fetus rises centrally, the bladder ligaments naturally loosen, and the water passage becomes unobstructed. Therefore, this condition where the fetus presses upon the urinary tract causing obstruction is named "Zhuanbao" (Displaced Bladder). If urine leaks continuously, it is named "Lou Tai" (Leaking Fetus), each having its own treatment formula. If unable to urinate accompanied by abdominal distension, it is named "Zhuanbao", suitable for treatment with Ba Wei Wan (Eight-Ingredient Pill) or Danxi Shen Zhu Yin (Danxi Ginseng and Atractylodes Decoction); after ingestion, induce vomiting by stimulating the throat with a finger, expel the medicinal juice, wait until the qi stabilizes, then administer again in the same manner. For those who can urinate while standing but not while squatting, Wu Ling San (Five-Ingredient Powder with Poria) is suitable. For those whose urine retention results from holding it for too long, Talcum Powder (Hua Shi Mo) mixed with scallion decoction can be taken orally, or fumigation and washing of the external genitalia with a decoction of Perilla Leaf (Zi Su Ye), Scallion White (Cong Bai), and Galangal (Liang Jiang) is also effective.
During pregnancy, if there is inexplicable continuous weeping and sadness, resembling spirit possession, it is called "Zang Zao Shang Bei" (Visceral Dryness and Sorrow). It is suitable to use Zhu Ru Tang (Bamboo Shavings Decoction) or Gan Mai Da Zao Tang (Licorice, Wheat, and Jujube Decoction), or to burn red jujubes to charcoal retaining their properties and swallow them with rice water.
[Zhu Ru Tang] Bamboo Shavings (Zhu Ru), Ginseng (Ren Shen), Ophiopogon Root (Mai Dong), Pinellia Tuber (Ban Xia), Poria (Fu Ling), Licorice Root (Gan Cao), Fresh Ginger (Sheng Jiang)
[Gan Mai Da Zao Tang] Licorice Root (Gan Cao), Wheat (Xiao Mai), Jujube (Da Zao)
An Tai (Calming the Fetus) and Gu Tai (Securing the Fetus) are different: Tonifying when blood deficiency leads to threatened miscarriage is called "Gu Tai"; regulating and smoothing when qi disharmony occurs is called "An Tai". An Tai focuses on smoothing qi and cooling blood, while Gu Tai focuses on generating blood and tonifying qi.
During pregnancy, although menstruation is scanty or occurs once a month, it is named "Lou Tai" (Leaking Fetus). Causes include qi deficiency, blood deficiency, heat, wind invasion, excessive blood volume, qi stirring blood, and bleeding due to physical exertion; the causes are not singular.
For those with qi and blood deficiency, Jiao Ai Si Wu Tang (Gelatin and Mugwort Four Substances Decoction) with added Chinese Plum (Wu Mei), or Si Wu Tang (Four Substances Decoction) with added Ginseng (Ren Shen), Atractylodes Macrocephala (Bai Zhu), and Cimicifuga Rhizome (Sheng Ma) should be used.
For deficiency complicated by heat, Zi Su Yin (Perilla Decoction) with added Scutellaria Root (Huang Qin), Atractylodes Macrocephala (Bai Zhu), Donkey-hide Gelatin (E Jiao), and Amomum Fruit (Sha Ren) should be used.
For bleeding due to physical exertion, Fo Shou San (Buddha's Hand Powder) with added Bitter Orange (Zhi Ke), Scutellaria Root (Huang Qin), and Atractylodes Macrocephala (Bai Zhu) should be used.
For bleeding stirred by qi or occurring naturally due to abundant blood, medication is unnecessary, and the fetus will remain safe.
