The deficient Yang fire on the left side attacks upward. Unilateral tonsillitis (Ru E) is severe, accompanied by wind pathogenic factors in the throat sides. Lesions located at the Guan Qian (anterior/shallow part of the throat) are easy to treat but require urgency; those at the Guan Hou (posterior/deep part) require different treatments.
This condition arises beside the Di Zhong (central deep part of the throat, near Tian Tu acupoint), resembling a lotus seed. The left side corresponds to the Heart, and the right side to the Lung. The treatment method is the same as for bilateral tonsillitis, and knife incision must absolutely be avoided.
Shu Fu said: This condition has distinctions in location and differences between deficiency/excess, wind-heat, and Qi stagnation. Those arising on both sides of the Di Zhong are unilateral or bilateral tonsillitis, belonging to excess patterns at the Guan Qian, which are easy to treat. If it originates inside the deep throat, it is named "Throat Tumor" (Hou Liu), belonging to Qi stagnation and deficiency patterns at the Guan Hou, which are difficult to treat. Contemporary physicians fail to recognize this and generally treat it as tonsillitis; how can they expect efficacy? However, Throat Tumors arise from heat stagnation in the Liver and Lung meridians, compounded by excessive speech damaging Qi and an irritable temperament. The shape resembles a longan fruit, wrapped with red blood vessels, appearing either bilaterally or unilaterally, growing beside the inner throat, sometimes with a large head and small stalk. Initially, there is slight pain in the throat without aversion to cold or fever. After a long time, the color becomes white and slightly hard, causing no pain unless touched. It may be aggravated by strong alcohol, fried/grilled foods, or anger and shouting, leading to sharp pain. Needle and knife therapies are contraindicated. Blow powder Xiao Liu Bi Yu San onto the lesion. Internal administration of Jia Wei Xiao Yao San or Yi Qi Qing Jin Tang is suitable. Alternatively, decoct Xia Ku Cao (Prunella vulgaris) together with Yu Jin (Curcuma aromatica Rhizome) and drink it as tea. Over time, it will naturally subside. For those with weak constitutions who develop this disease due to depression and dissatisfaction, Gui Pi Tang added with Chai Hu (Bupleuri Radix), Dan Pi (Moutan Cortex), and Shan Zhi (Gardeniae Fructus) is appropriate. Regarding modifications, one should apply wisdom and flexibility.
Yan Kou Feng (Swallow's Mouth Wind) arises beside the Di Zhong, making it impossible to swallow food or drink despite wanting to eat. If needle and knife therapies are applied correctly without error, rapid efficacy can be achieved easily.
This condition appears on both sides above the Di Zhong, affecting both left and right, presenting with redness and swelling that prevents swallowing. In severe cases, the swelling protrudes over the tongue, causing pain throughout the tongue. When using the knife method, gently puncture the swollen area carefully. Do not pierce deeply. Under no circumstances should the knife be used mistakenly in the middle of the upper region or the upper part of the Yan Kou shape. First, use Jiao Yao (horn medicine) to resolve phlegm, then use acupuncture to open the wind pathway, blow Hui Sheng Dan powder, and administer Zi Di Tang internally.
Shu Fu said: At the initial stage of this condition, blowing Chi Lin San is sufficient, which is far superior to using horn medicines and knives. Internally, continue administering Zi Di Tang plus Kai Guan San. If the fire pathogen is severe, add Shi Gao (Gypsum Fibrosum) according to the degree. This is a method I have repeatedly verified to be effective.
