Arising from dryness and accumulation in the Heart, two days prior to onset, there is necessarily tightness of Qi in the chest and diaphragm, with rapid breathing. Suddenly, the throat becomes swollen and painful, the extremities become cold (Jue Ni), neck veins appear constricted as if bound by ropes, heat accumulates internally while swelling surrounds externally, accompanied by numbness and itching. Red filaments in the throat tighten and entangle, fingernails turn either white or cyan, the palms are intensely hot, the throat swelling is severe, and the sound of phlegm resembles sawing. This condition is extremely acute; if treated at the initial stage, it can be saved. However, if more than one day and night pass, and the patient presents with fixed staring eyes and a voice like thunder, it is incurable. If a lamp brought near the patient's mouth is immediately extinguished, it is also incurable. The appearance of panting and sweating on the forehead indicates imminent danger. This is caused by Heat in the Kidney Meridian leading to hyperactivity of Heart Fire. (Original note: It is advisable to add Realgar and Bezoar [Niuhuang] in treatment.) [Commentary by E: Chan Hou Feng (Entangling Wind Throat) is more fierce and dangerous than Sai Hou Feng (Blocked Wind Throat). The former is cured by opening the orifice and resolving phlegm; the latter must consider the Shaoyin channel, but initially, the priority is always to open and resolve phlegm.]
It is often due to unresolved chronic fatigue illness, with deficient Fire flaming upward and Ying Blood (Nutritive Blood) already exhausted. The symptoms include red spots on the hard palate, dense and resembling mosquito bites. This indicates a critical condition nearing death; do not treat it lightly to avoid blame and resentment.
Externally, there is redness and swelling; internally, the shape resembles an egg, with a bright red color and a mirror-like luster. There is fever and aversion to cold, with the eyeballs rolling upward. This is caused by accumulated Heat in the Heart and Spleen. Although the presentation appears ominous, prompt treatment leads to recovery. [Commentary by E: Hejian’s discussion on treating Hou Bi (Sore Throat) is akin to fighting a fire, reflecting the principle of "dispersing stagnant Fire" from the *Nei Jing*. "Dispersing" refers to inducing sweating; however, since the throat cannot induce sweating through perspiration, bleeding therapy serves as one method of dispersal.]
Located on both sides of the throat, occurring singly or bilaterally, varying in size from that of a round eye, with blood vessels entangled resembling tumors, hence the name. This results from Heat affecting the Lung Meridian, damage to Qi from excessive talking, shouting in anger, reading too hastily, or consuming excessive distilled spirits. Ingestion of roasted, fried, or hard foods causes pain upon contact, while avoidance prevents pain. One must calm the spirit, rest quietly, and treat with herbs that clear and descend, leading to recovery. [Commentary by E: This condition resembles Mei He Qi (Plum-pit Qi), caused by emotional stagnation leading to Phlegm condensation and Qi obstruction. Do not recklessly use knives or needles. It is suitable to apply Yu Bing Mei Dan powder, combined with Bai Jin San, and internally administer formulas such as Si Qi Tang, Yue Ju Wan, Xuan Fu Hua Decoction, or Cong Jiang Tang, hoping for gradual improvement.]
