For the treatment of malaria (Malaria) that begins on the first day of the lunar month, it should heal within fifteen days. If it does not improve (Chai), it should resolve by the end of the month. If it still does not resolve, it will form a mass or accumulation (Zheng Jia - abdominal masses). In such cases, urgent treatment is required, and this pill (Biejia Jian Wan) is indicated.
Bie Jia (Turtle Shell, 12 fen, calcined), Wu Shan (Iris Tectorii Rhizome/Shexian, 3 fen, burnt, i.e., She Gan), Huang Qin (Scutellariae Radix, 3 fen), Chai Hu (Bupleuri Radix, 6 fen), Shu Fu (Drynariae Rhizoma/possibly referring to a specific preparation involving fermentation or glue-like consistency in context, 4 fen), Gan Jiang (Zingiberis Rhizoma, 3 fen), Da Huang (Rhei Radix et Rhizoma, 3 fen), Shao Yao (Paeoniae Radix Alba, 5 fen), Gui Zhi (Cinnamomi Ramulus, 5 fen), Ting Li Zi (Lepidii Fructus, 1 fen, processed), Shi Wei (Pyrrosiae Folium, 3 fen, hairs removed), Hou Po (Magnoliae Officinalis Cortex, 3 fen), Dan Pi (Moutan Cortex, 5 fen), Qu Mai (Polygoni Avicularis Herba, 2 fen), Zi Wei (Camptothecae Fructus, 3 fen), Ban Xia (Pinelliae Rhizoma, 1 fen), Ren Shen (Ginseng Radix et Rhizoma, 1 fen), Xie Chong (Eupolyphaga/Steleophaga, 5 fen, processed), E Jiao (Colla Corii Asini, 3 fen, calcined), Qiang Lang (Catharsius Beetle, 6 fen, processed), Tao Ren (Persicae Semen, 2 fen, processed).
The above twenty-three ingredients are ground into fine powder. Take one dou of ash from the kitchen stove and one hu five sheng of clear wine. Soak the ash in the wine. When half of the wine has been consumed, place the Bie Jia into the mixture. Boil until the Bie Jia becomes soft and gelatinous like glue. Remove the Bie Jia, mix its residue/jelly with the powdered herbs, and make pills the size of tung tree seeds. Take seven pills at a time, three times daily.
The verse states: The alternation of cold, heat, excess, and deficiency relies entirely on the waxing and waning of Yin and Yang. The heavenly qi changes every half-month, and the human qi follows suit. Otherwise, if the heavenly and human qi do not coordinate, the pathogen will deepen. Thus, if the condition does not resolve, stubborn accumulations form, presenting a resistant pattern. Therefore, urgent treatment is necessary. Biejia Jian Wan contains many Qi-moving and blood-dispelling drugs, yet they are not considered too harsh; taking them three times a day is not considered too rapid. This is precisely what is meant by "attacking while the enemy is not yet fully assembled."
Wang Jin San stated: Biejia Jian Wan exclusively uses agile substances from different categories. Whether aquatic or terrestrial, flying or diving, ascending or descending, moving or hiding, all are included.
For the treatment of Warm Malaria (Wen Nue), the pulse resembles that of a healthy person. The body presents with fever only, without chills, accompanied by vexatious pain in the bone joints and occasional vomiting. This decoction is indicated.
Zhi Mu (Anemarrhenae Rhizoma, 6 liang), Shi Gao (Gypsum Fibrosum, 1 jin), Gan Cao (Glycyrrhizae Radix et Rhizoma, 2 liang, calcined), Jing Mi (Oryza Sativa, 6 he), Gui Zhi (Cinnamomi Ramulus, 3 liang).
The above five ingredients are boiled in ten dou of water until the rice is cooked and the decoction is ready. Strain the dregs. Take one sheng warm, three times daily.
The verse states: The discussion on the original Baihu Tang is detailed. Adding three liang of Gui Zhi creates a new formula. Fever without chills indicates Warm Malaria, accompanied by vexatious bone joint pain and vomiting. (The verse for Baihu Tang is omitted here.)
Wang Jin San stated: The *Neijing* discusses malaria, stating that malaria with heat preceding cold, where the pathogen hides in the marrow, is called Wen Nue and Dan Nue. Zhang Zhongjing considers that malaria with heat only and no chills, where the pathogen hides in the heart, is... [text incomplete].
Both belong to the Shaoyang channel patterns. Do not worry excessively.
