The *Zhen Jing* (Needle Classic) states: When cold pathogen lodges in the epiglottis, sudden aphonia occurs. This is due to cold pathogen and phlegm-fluids condensing between the throat and larynx; it is appropriate to treat with sweet, acrid, and warm herbs. Cold-natured drugs must be strictly avoided; otherwise, the pathogen will remain stagnant and unresolved, leading to chronic disease.
*Mother Ginger Wine* from *Qian Jin Fang*:
Fresh ginger juice (1 sheng), butter, ox marrow (each 1 sheng), Cinnamomi Cortex (Guixin), Zanthoxyli Pericarpium (Qinjiao) (each 1 liang), Saposhnikoviae Radix (Fangfeng) (1.5 liang), Ligustici Chuanxiong Rhizoma (Xiongnu), Notopterygii Rhizoma et Radix (Duhuo) (each 1 liang 6 zhu).
Grind the above into powder, mix with ginger juice, and decoct until the ingredients are thoroughly moistened. Add butter and marrow, mix well, and simmer gently over low heat using the "three up, three down" method. In the morning, take 2 he of the paste with 1 sheng of warm clear wine, swallowing slowly. Take three times daily and once at night.
*Banxia Guigan Tang* (Pinellia, Cinnamon, and Licorice Decoction): Treats cold syndromes with absence of Yang, sore throat, and throat closure.
Cinnamomi Ramulus (Lagui), Glycyrrhizae Radix et Rhizoma (Gancao) (roasted), Pinelliae Tuber (Banxia) (processed).
Take equal parts of the above, cut into pieces. For each dose, use 3 qian, add 1 da zhen of water, decoct to half a zhen, and sip slowly. (From *Huo Ren Shu*)
The "latent qi disease" refers to an abrupt cold pathogen striking the body out of season, lying dormant in the Shaoyin meridian. Initially unnoticed, it manifests after ten days, starting with sore throat followed inevitably by diarrhea. Ancient formulas call this "Kidney Cold Damage," and *Banxia Guigan Tang* is indicated.
Sore throat accompanied by loss of voice is caused by wind-heat and phlegm-fluids obstructing the throat gate. It may also result from Yin deficiency and lung damage. Since the Lung corresponds to Metal and governs voice, if Metal is broken, it does not sound; if Metal is full/excess, it also does not sound. To differentiate: excess patterns present with obstruction preventing sound emission, while deficiency patterns present with hoarse, broken voice.
*Hezi Tang* (Chebula Decoction) from *Xuan Ming Lun Fang*:
Terminalia Chebula Retz. Fructus (Hezi) (4 fruits, half raw, half roasted), Platycodonis Radix (Jiegeng) (1 liang, half roasted, half raw), Glycyrrhizae Radix et Rhizoma (Gancao) (2 cun, half roasted, half raw).
Grind into fine powder. For each dose, take 2 qian, decoct with one zhen of urine from a healthy boy and one zhen of water for five or six boils. Take warm. Severe cases resolve within three doses.
*Haizang Fasheng San* (Haizang Voice-Generating Powder): Treats sore throat and inability to speak.
Trichosanthis Fructus (Gualou), Bombyx Batryticatus (Bai Jiangcan) (head removed, stir-fried, 0.5 liang), Glycyrrhizae Radix et Rhizoma (Gancao) (roasted, 2 qian).
Grind into fine powder. For each dose, take 3 qian, mix with warm wine or fresh ginger juice. Take 0.5 qian, wrap in cotton, hold in mouth to dissolve slowly, swallowing saliva is acceptable. Take two to three times daily. *Bao Jian* adds Platycodonis Radix (7.5 qian, stir-fried and powdered), mixed with 1 qian-bi of Natrii Sulfas (Puxiao), held in the mouth to swallow saliva.
