Some cases arise from phlegm generated by heat, while others involve heat generated by phlegm; the two entangle each other and are difficult to resolve, mutually exacerbating symptoms resembling malaria (alternating chills and fever). Therefore, to eliminate phlegm, one must first clear the heat. In the past, it was held that when phlegm reverses upward due to excessive fire, treatment should prioritize clearing the fire. The manifestations include dryness of the throat, or a sensation of blockage and congestion, heaviness and dizziness of the head and eyes, coughing up thick and sticky sputum, and redness and heat in the face and eyes.

When the lung is deficient and yin fluids are exhausted, dryness increases daily; qi fails to transform normally, generating fire, and body fluids condense to form phlegm.
