Waist and lower back pain due to cold-dampness arises from sitting or lying in damp and cold environments for prolonged periods, developing gradually over time. This corresponds to the "Kidney Fixation" (Shen Zhuo) described in the *Jin Gui Yao Lue*. The symptoms include pain accompanied by a sensation of coldness and heaviness, which worsens during cloudy weather or after prolonged sitting. The primary treatment is Shen Zhuo Tang.
Shen Zhuo Tang (Note: The text lists the name as Sheng Fu Tang, but describes a formula for Kidney Fixation)
Ingredients: Raw Aconite Root (Fuzi, raw, 1 fen), Atractylodes Rhizome (Cangzhu, stir-fried), Eucommia Bark (Duzhong, stir-fried with ginger juice, each 0.5 liang), Dried Ginger (Sheng Ganjiang), Atractylodes Macrocephala Rhizome (Baizhu), Poria (Fuling), Achyranthes Root (Niuxi, soaked in wine and baked), Magnolia Bark (Houpo, processed), Licorice Root (Gancao, honey-fried, each 2 liang).
Crush the ingredients. For each dose of 3 qian, add four slices of fresh ginger and two jujubes. Take before meals.
Qianniu Wan: Treats waist pain caused by the flow and accumulation of cold-dampness, rendering the waist unable to bend or extend.
Ingredients: Black Morning Glory Seed (Hei Qianniu), Corydalis Rhizome (Yanhusuo, slightly stir-fried), Psoralea Fruit (Buguzhi). These three herbs are ground into powder separately, pounded separately, and stir-fried separately; take the powder of each, 2 liang.
Mix with paste made from roasted garlic. Form pills the size of paulownia seeds. Take 50 pills per dose before meals, washed down with decoction of scallion, wine, and salt. (From *Yang Shi Jia Zang Fang*)
Zihe's Yugong San: Treats waist pain caused by water qi flowing and accumulating.
Ingredients: Black Morning Glory Seed (Hei Qianniu, 4 liang), Fennel Seed (Huixiang, stir-fried, 1 liang).
Grind into powder. Mix with ginger juice and take 1-2 qian per dose.
When Spleen damp-heat transmits to the Kidneys, it often results from excessive consumption of rich wine and strong-flavored foods, damaging the Middle Qi internally. This leads to the accumulation and stagnation of damp-heat, which flows into the Kidney meridians, causing heaviness and pain in the body. Symptoms flare up during cloudy weather or after prolonged sitting, and the pulse is typically slow (Huan).
Dongyuan Cangzhu Tang
Ingredients: Atractylodes Rhizome (Cangzhu, 5 qian, to dispel dampness and relieve pain), Bupleurum Root (Chaihu, 3 qian, to circulate the meridians), Saposhnikovia Root (Fangfeng, 1.5 qian, to dispel wind and overcome dampness), Phellodendron Bark (Huangbai, 1.5 qian, to clear heat and relieve pain).
Add two zhong of water, decoct until one zhong remains. Take on an empty stomach before meals.
Danxi’s Formula for Damp-Heat Waist and Leg Pain
Ingredients: Tortoise Shell (Guiban, wine-fried, 2 liang), Atractylodes Rhizome (Cangzhu), Phellodendron Bark (Huangbai, wine-stir-fried), Xanthium Fruit (Canger), Clematis Root (Weilingxian, wine-soaked, 1 liang), Biota Tops (Cebai, 0.5 liang).
Grind into powder. Use wine paste to form pills. For administration, decoct Siwu Decoction with black bean juice, adding Tangerine Peel (Chenpi), Licorice Root (Gancao), and Fresh Ginger (Shengjiang). Drink the decoction to wash down the pills. Another version includes White Peony Root (Baishao) and Anemarrhena Rhizome (Zhimu).
