(The etiologies of malaria include wind pathogen, summerheat pathogen, food stagnation, as well as chronic malaria (Lao Nue), malaria mass (Nue Mu), and phlegm-induced malaria.)
Chronic Malaria: This is caused by the invasion of wind and summerheat pathogens into the Yin aspect. For cases involving the Zang organs, blood-nourishing herbs are used: Chuanxiong Rhizoma (Chuanxiong), Xiongchuang Rhizoma (Fuxiong), Carthamus Flos (Honghua), Angelicae Sinensis Radix (Danggui), added with Atractylodis Macrocephalae Rhizoma (Cangzhu), Atractylodis Rhizoma (Baizhu), Angelicae Dahuricae Radix (Baizhi), Phellodendri Cortex (Huangbai), and Glycyrrhizae Radix et Rhizoma (Gancao) for decoction.
After decoction, leave it exposed overnight and take it warm the next morning. In cases without sweating, induce sweating to primarily disperse pathogens while accompanying with tonification; in cases with sweating, stop sweating to primarily support healthy Qi while accompanying with dispersion. If a malaria mass (abdominal lump formed after long-standing malaria) exists, use pill preparations for resolution and guidance, using Vinegar-processed Trionycis Carapax (Biejia) as the sovereign herb, with Sparganii Rhizoma (Sanleng), Curcumae Aestivae Rhizoma (Pengru), and Cyperi Rhizoma (Xiangfu) added or subtracted according to the syndrome.
Those who have an attack every three days have been sick for one year. Those with intermittent attacks have been sick for half a year. Those with daily attacks have been sick for one month. Those who attack for two consecutive days and then rest for one day indicate that both Qi and Blood are diseased. Those with attacks every other day should be treated with tonifying herbs accompanied by exterior-releasing herbs. After taking medicine, use "Malaria Intercepting Powder" (Nue Dan) to intercept the malaria. For diseases in the Yin aspect, use medication to bring them out to the Yang aspect before they can be intercepted.
Another prescription:
Tsaoko Fructus (Caoguo), Anemarrhenae Rhizoma (Zhimu), Arecae Semen (Binglang), Mume Fructus (Wumei), Dichroae Radix (Changshan), Roasted Glycyrrhizae Radix et Rhizoma (Zhi Gancao), and Processed Manis Squama (Pao Shanjia). Decoct with one large bowl of water and one large bowl of wine until half a bowl remains, leave exposed overnight. Take warm two hours before the onset of malaria. If vomiting occurs, allow it to vomit naturally.
Qinghao Pill for Intercepting Malaria:
Artemisiae Annuae Herba (Qinghao, 1 liang), Ilicis Aquifolii Folium (Dongqingye, 2 liang), Verbena Officinalis Herba (Mabiancao, 2 liang), Cinnamomi Ramulus (Guangui, 2 liang).
Dry the above three leaves thoroughly, weigh them, and grind into powder. Form pills the size of pepper seeds according to standard methods. Divide each liang of powder into four doses. Take all at once one hour before the expected onset of malaria.
General Principle: Summerheat and wind must be treated by inducing sweating. In summer, resting in cool, breezy places often causes sweat pores to close, preventing perspiration from being excreted. For cases caused by food stagnation, treat from the perspective of digestion.
For malaria patients with deficiency, first administer one or two doses of Codonopsis Radix (Renshen) and Atractylodis Rhizoma (Baizhu) to support the Qi and prevent sinking, then use other medications. The method is the same for treating internal injury complicated by external pathogens. Internally, phlegm is usually the primary factor; it must be resolved through sweating. Use Erchen Tang (Two-Aged Decoction) added with Dichroae Radix (Changshan), Bupleuri Radix (Chaihu), Scutellariae Radix (Huangqin), and Tsaoko Fructus (Caoguo).
For severe malaria presenting with alternating chills and fever, headache as if splitting, thirst with desire to drink water, and spontaneous sweating, treat with Ren Shen, [missing character], Baizhu, Poria (Fuling), Coptidis Rhizoma (Lian), Gardeniae Fructus (Zhizi), Chuanxiong Rhizoma (Chuanxiong), Cangzhu, and Pinelliae Rhizoma (Banxia).
For long-standing malaria, use Erchen Tang added with Chuanxiong Rhizoma (Chuanxiong), Cangzhu, Bupleuri Radix (Chaihu), Puerariae Radix (Gegen), and Atractylodis Rhizoma (Baizhu), combining tonification with dispersion.
