For patients with excess conditions and internal heat, needle insertion should be shallow with light rubbing; excessive rubbing may generate wind pathogens and worsen the condition. For patients with excess conditions and internal cold, rubbing must be deep and thorough to ensure smooth Qi flow and harmonize the disease.
In deficiency conditions, ten turns of the needle combined with following (Xun) manipulation are used; if Qi does not arrive, insert another two cun and turn again, as Qi arrival will be slow. Insert another three cun and add three turns; if Qi still fails to arrive, it is a fatal case difficult to treat.
In excess conditions, observe the needle sensation: if the needle feels loose, no turning is needed. If the needle feels tight after insertion, turning is required. Ignoring this and continuing to turn blindly may cause violent Qi, damaging Ying and Wei (Nutritive and Defensive Qi) harmony.
When Qi arrives, advance by three fen to enhance stimulation and promote blood-Qi circulation for therapeutic effect. In deficiency cases, retain the needle at depths ranging from one to three cun, allowing blood-Qi to circulate freely with respiration.
For emergency situations, press and withdraw slowly, withdrawing three fen at a time. Rapidly lift five fen, then advance three fen. Withdraw until bleeding stops at five fen depth, then tap the needle handle three times to stop bleeding spontaneously.
Rotate the needle gently in a clockwise direction only when Qi flow is stagnant. This method is often used in emetic therapies but should not be used in routine mild treatments.
Rotate the needle horizontally against the meridian direction, specifically opposing Yang meridians for emetic therapies. This should not be used in routine treatments, as it may harm Yang Qi.
To raise Yin, one must first raise Yang. Carefully examine the balance of blood and Qi. Raising Yang on days of abundant Yang or promoting blood prosperity on days of abundant Yin follows specific principles. On Yang days, rotate Yang meridians counter-clockwise; on Yin days, nourish Yin meridians and Zang-Fu organs. Using raising-Yang methods on Yin days and lowering-Yang methods on Yang days can benefit hundred diseases.
Lifting manipulation is primarily used on Yang meridians, while pressing is used on Yin meridians to balance blood. For deficient patients, combine lifting with pressing, followed by following (Xun) to facilitate Qi arrival. Lifting skin by one cun on Yang meridians indicates they are not deficient. Using raising-Yang methods on Yin days and lowering-Yang methods on Yang days eliminates hundred diseases.
Scraping and trembling manipulations involve upward scraping for Yang meridians and downward scraping for Yin meridians to promote spontaneous blood flow. For severe Yang conditions, scrape and tremble seven or four to five times; for severe Yin conditions, scraping should be light. Upward scraping on Yin meridians may drive pathogens inward against the flow. Scraping and trembling follow similar principles for both Yin and Yang to harmonize blood and Qi.
Tonification and sedation based on Mother-Child relationships utilize lifting, pressing, and rubbing. Combine high/low scraping and trembling techniques. Counter-clockwise rotation is tonification, clockwise is sedation. Upward scraping and downward trembling require rubbing. Add searching rotations one or two times, and abdominal coiling techniques to produce sounds. Three-nine cycles of lifting and rubbing are used for excess conditions; rubbing helps regulate blood-Qi in heat conditions.
For individuals around twenty years old, wait for thirty degrees of Qi arrival, applying tonification three times and following seven times. For those aged forty to fifty, wait for forty degrees, applying tonification five times and following ten times, though Qi deficiency may persist. For those aged sixty to seventy, wait for thirty-five degrees, applying tonification four times to support Old Yang, using seven diligent operations. Male and female respiratory rhythms are distinct, revealing true patterns of climate and deficiency/excess.
