To treat Shaoyin disease (Shaoyin Syndrome) occurring within one or two days, where the mouth feels neutral (no bitter taste or dryness), and there is aversion to cold in the back, moxibustion should be applied, and this formula is indicated. It is also indicated for Shaoyin disease presenting with body pain, cold hands and feet, joint pain, and a deep pulse.
Fuzi (Aconiti Lateralis Radix Praeparata, raw, 2 pieces) Fuling (Poria, 3 liang) Renshen (Ginseng Radix, 2 liang) Baizhu (Atractylodis Macrocephalae Rhizoma, 4 liang) Shaoyao (Paeoniae Radix Alba, 3 liang)
Take these five herbs, add eight sheng of water, decoct to obtain three sheng of liquid, strain off the dregs, and take one sheng warm, three times daily.
Song says: Two pieces of raw Fuzi constitute the Fuzi Tang; Baizhu at four liang is the main herb of this formula. Shaoyao and Fuling are three liang each, Renshen is two liang; carefully distinguish symptoms such as cold back, deep pulse, and body pain.
Wei An (Chen Nianzu's commentary): The Treatise states that for Shaoyin disease acquired in one or two days, if the mouth is neutral and there is aversion to cold in the back, moxibustion should be used, and this decoction is suitable. This treats the deficiency of Yang in the Taiyang channel (Note: The text contrasts this with Shaoyin, but explicitly links it to Shaoyin Jun Fire deficiency below). This treats the internal deficiency of Shaoyin Jun Fire, where the spiritual mechanism fails to rotate. In the formula, raw Fuzi (2 pieces) serves as the sovereign drug, benefiting the generating Yang within the Water of the Lower Jiao to reach the Jun Fire of the Upper Jiao. Baizhu serves as the minister drug, because the Heart and Kidney rely on the Qi of the Middle Earth (Spleen/Stomach) to interact and combine. Renshen serves as an assistant drug, utilizing its sweet and moist nature to moderate the extreme acridity of raw Fuzi. Shaoyao also serves as an assistant drug, utilizing its bitter and descending nature to drain the severe toxicity of raw Fuzi. However, both Renshen and Shaoyao are Yin-natured drugs; although they can resolve the violent nature of Fuzi, they might restrain its efficacy. After Fuzi exerts its effect, these Yin drugs will exit through urination to a place of no use, leaving no residual Yin Qi to hinder the Yang drugs. Master Zhang Zhongjing uses this formula to treat Yang deficiency in one aspect, and Yin deficiency in another. Contemporary physicians speak of these two terms casually; do they understand that "Yang" refers to Taiyang, "Yin" refers to Shaoyin, and the principle of one formula governing treatment?
To treat Shaoyin disease with diarrhea accompanied by pus and blood in the stool, this decoction is indicated. It is also indicated for Shaoyin disease lasting two or three days, presenting with abdominal pain, difficulty in urination, and diarrhea without pus and blood.
Chishizhi (Halloysitum Rubrum, 1 jin; half used whole, half sifted into powder) Ganjiang (Zingiberis Rhizoma, 1 liang) Jingmi (Oryza Sativa, 1 sheng)
Take these three ingredients, add seven sheng of water, boil until the rice is cooked, strain off the dregs, add one cun bi of Chishizhi powder, and take seven ge warm, three times daily. If cured after one dose, do not take the remainder.
Song says: One sheng of Jingmi and one jin of Chishizhi; the method of grinding half the Chishizhi is also unique. Boil with one liang of Ganjiang and take; this is the good standard for treating Shaoyin pus and blood.
Zhanglingshao (Zhang Yinzhan) said: For Shaoyin disease with diarrhea and pus/blood, Taohua Tang is indicated. This is due to contracting the heat of Shaoyin Jun Fire, which does not affect the formless Qi transformation, but affects the有形 (form/structure). This leads to difficulty in urination, continuous diarrhea, and stools containing pus and blood. This refers to the period from the second or third day to the fourth or fifth day, when the Qi of Taiyin Spleen is dominant. If the Spleen collaterals are blocked, abdominal pain occurs; if the Spleen collaterals cannot transport, urination becomes difficult; if urination is difficult, water and grain are not separated, leading to continuous diarrhea; injury to the Yin collaterals results in pus and blood. Chishizhi is formed from the congealed blood vessels of the mountain, thus it treats diseases of the meridians. The following section mentions that pus and blood in stools can be treated with acupuncture, to clarify the meaning that the disease lies in the meridians.
