Dyspepsia with acid regurgitation is caused by damp-heat in the stomach, which stagnates and suppresses Liver Fire. Dampness stagnation generates heat, and heat stagnation produces sourness; since sourness is the flavor associated with Wood of the Liver, the pathology is attributed to Liver Fire. The appropriate treatment involves modifying Xiaoyao San (Free and Easy Wanderer Powder) combined with Zuojin Wan (Left Metal Pill). In cases where acid regurgitation is due to food stagnation or vomiting of acid due to phlegm-fluid retention, the primary treatment should focus on resolving food stagnation and transforming phlegm, also combined with Zuojin Wan. If elderly or weak individuals suffer from chronic acid regurgitation, their body fluids will be depleted, gradually evolving into a condition of dysphagia and obstruction (Ge Zheng). Additionally, for elderly people with deficient Stomach Qi who continuously vomit acid water, the main treatment should be to tonify Stomach Qi.
Zaot (Restlessness in the epigastrium), commonly known as "heart restlessness," belongs to Fire syndromes, with Phlegm being secondary. Those suffering from Zaot throughout the year are destined to die prematurely, as nothing is more intense than Fire among all things. If an elderly person suffers from persistent Zaot, it is also a precursor to Ge Zheng. Zhongchun attributed both acid regurgitation and Zaot to Stomach Fire. The treatment should prioritize clearing heat and descending fire, with slight addition of methods to resolve stagnation.
Zuojin Wan [See Hypochondriac Pain]. Alternatively, combine with modified Xiaoyao San. For elderly patients with deficient Stomach Qi and continuous vomiting of acid water, combine this formula with Yigong San. If acid regurgitation is due to food stagnation, combine with Baohe Tang. If vomiting of acid is due to phlegm-fluid retention, combine with Erchen Tang, adding Cang Zhu (Atractylodis Rhizoma) and Ze Xie (Alismatis Rhizoma). This is a miraculous formula for treating acid regurgitation. In all treatments for acidity, a small amount of Wu Zhu Yu (Evodiae Fructus) must be added as a counter-agent (fan zuo), utilizing its nature to disperse the pathogen.
Qingwei Decoction [Self-made]. Shi Hu (Dendrobii Caulis), Mai Dong (Ophiopogonis Radix), Lu Gen Zhi (Reed Rhizome Juice) [to clear Stomach heat], Huang Lian (Coptidis Rhizoma) [stir-fried with Wu Zhu Yu], Lian Qiao (Forsythiae Fructus) [to descend fire], Shan Zha (Crataegi Fructus), Mai Dong (Ophiopogonis Radix), Ju Hong (Citri Reticulatae Pericarpium Rubrum) [to resolve stagnation]. Add Zhu Ye (Bambusae Folium); if heat is severe, add more Shi Gao (Gypsum Fibrosum) and Qing Dai (Indigo Naturalis). Bad breath or insipid taste [both belong to Stomach Heat] can also be treated with this formula, removing Shan Zha, Ya (Hordei Fructus Germinatus), and Ju Hong. This is a formula that clears Stomach Heat while resolving stagnation, following Zhongchun's method to treat acid regurgitation and Zaot. Mr. Liu stated that Zaot should be treated by distinguishing between excess and deficiency. For Zaot caused by deficient Fire of the Spleen and Stomach, which stops upon eating, Zisheng Wan is appropriate. If there is excess Qi, long-term stagnation transforms into Fire, Fire stagnation generates Phlegm, and Phlegm moves due to Fire causing Zaot, use Erchen Tang added with Huang Lian (Coptidis Rhizoma) and Shan Zhi (Gardeniae Fructus). This prescription is particularly suitable for obese individuals with damp-phlegm.
