Lower abdominal carbuncle (Shaofu Yong) syndromes include those caused by Qi and Blood stagnation (Qi xue ning zhi), those where Damp-Heat flows into the Small Intestine (Shi re liu zhu Xiao chang), and those formed by Cold-Dampness constraint (Han shi yu jie). The illness began last summer; the lower abdomen was stiff and hard, with attacking and rushing pain. The lower abdomen is the region of the Large Intestine (Guang chang) and the pathway of the Liver Meridian circulation. Liver Qi depression combined with Cold Evil invasion causes the Qi and Fire of the Stomach and Intestines to malfunction. Blood obstruction follows Qi stagnation; over time, deficiency of Healthy Qi (Zheng Qi) and condensation of Pathogenic factors worsen. From winter to spring, the swelling became higher and more pronounced, red in color and soft, indicating internal pus formation, necessitating external rupture. However, since the intestinal membrane is injured, there is fear of fecal matter discharging alongside pus. Furthermore, poor dietary intake, urination without defecation, internal heat with a dry tongue, and a rapid pulse indicate Yin injury with Heat constraint. If, after draining large amounts of pus, Stomach Qi fails to recover and Yuan Qi (Original Qi) cannot sustain itself, this is deeply concerning. Regarding surgical treatment principles, one should typically use tonifying and expelling-toxin formulas. However, in cases of extreme deficiency, tonification is poorly tolerated, and clearing Heat may hinder the Spleen. Therefore, treatment should abandon external approaches and focus on internal regulation. The proposal is to regulate the Stomach and nourish Yin; once Yin is sufficient, bowel movements will naturally unblock, and when the Stomach harmonizes, appetite will return.

The intestinal carbuncle has ruptured externally, yielding slight pus, and foul matter is discharging from the orifice, allowing Turbid Qi to escape externally. This is highly inappropriate. The pulse is difficult to harmonize, and food intake is difficult to gradually increase. The primary concern is the simultaneous discharge of Healthy Qi and Turbid Qi, risking a collapse of the upper and lower Jiao. Urgently, add Baishao (Paeoniae Radix Alba), Renshen Xu (Ginseng Radix et Rhizoma Filum), and Shudi (Rehmanniae Radix Preparata) to the original formula.
Fu Qi (Intestinal Qi) has now unblocked. Add Dangshen (Codonopsis Radix) and Shihu (Dendrobii Caulis) to the original formula, and remove Baiziren and Raw Shouwu.
After the intestinal carbuncle rupture, pus is scanty while Qi is exhausted. Swelling has reduced by half, the pulse is calm, the body is cool, and the patient slept peacefully through the night; this is a favorable sign. At dawn, however, external symptoms worsened with swelling and pain. At Mao hour (5–7 AM), Qi deficiency leads to stagnation in the Large Intestine. Pathogenic factors and Healthy Qi clash at the intestinal membrane, making complete healing difficult. Treatment should tonify both Yin and Yang and simultaneously protect the membrane to prevent further deterioration.

One month into the intestinal carbuncle, the lower abdomen remains internally hard with cramping pain, and urine is turbid like pus. The approach should be to resolve stasis and promote dampness removal.

Qi and Blood stagnation causes lower abdominal hardness and pain, difficult urination, chills, and fever, trending toward an intestinal carbuncle. Urgent treatment should move Qi and transform phlegm.

One year of intestinal carbuncle: the inner membrane is injured, the constitution is emaciated, and the pulse is weak; this is a difficult-to-treat syndrome.
Administer Shiquan Dabu Wan (Decem Totorum Tonifying Pills), and also administer Hupo Lafan Wan (Ambari et Vitrioli Pilula).
The intestinal carbuncle has ruptured externally, foul matter discharges from the orifice, and the intestinal membrane is perforated; wound closure is extremely difficult. Use modified Shiquan Dabu as the main treatment. Add Muxiang (Aucklandiae Radix), Yurou (Corni Fructus), and Huang Sijuan (Serici Flavus Fibra).
The internal hardness of the intestinal carbuncle has loosened, and the pus is clearer. Continue modifying the previous method.
