In cases where postpartum dryness of the mouth and throat with thirst is accompanied by difficult urination, it is due to blood depletion causing excessive sweating, combined with fatigue and overwork damaging the Spleen. The Spleen fails to transport fluids for the Stomach, resulting in impaired circulation of Yuan Qi (Original Qi) generated from digestion and failure of the diffusing and descending functions. Consequently, there is a lack of fluid circulation in the upper body, leading to symptoms of dry throat and thirst; while in the lower body, stagnation of the Stomach and Kidney leads to obstruction of the water passages. Treatment should focus on assisting the Spleen and benefiting the Lung, raising Blood and Qi. Once Blood and Qi circulate freely, Yang rises and Yin descends, naturally generating fluids and facilitating smooth excretion. If one mistakenly attributes thirst to Fire and uses Huang Qin (Scutellaria Root), Huang Lian (Coptis Rhizome), Zhi Mu (Anemarrhena Rhizome), and Huang Bo (Phellodendron Bark) to clear heat downward, or treats urinary retention as Water stagnation using Wu Ling San (Five-Ingredient Powder with Poria) to promote diuresis, such approaches are not only ineffective but harmful.
Zhu Pi Yi Fei Tang (Assist Spleen Benefit Lung Decoction): Huang Qi (Astragalus Root) 1.5 qian, Ren Shen (Ginseng Radix et Rhizoma) and Mai Dong (Ophiopogon Tuber) each 2 qian, Wu Wei Zi (Schisandra Fruit) 10 berries, Dang Gui (Angelica Sinensis Radix) 2 qian, Fu Ling (Poria) 1.5 qian, Gan Ge (Puerariae Radix) 1 qian, Sheng Ma (Cimicifugae Rhizoma) 4 fen, Zhi Gan Cao (Honey-fried Licorice Root) 5 fen. For excessive sweating, add Zao Ren (Ziziphus Seed) 2 qian and Ma Huang Gen (Ephedrae Herba Cum Caulice) 1 qian. For severe thirst, use Sheng Mai San (Generate the Pulse Powder) as tea substitute. For constipation, add Rong Ren (Cistanche Stem) 2 qian. If the parturient is robust but suffers from intense heat and difficult urination, temporarily add Zhi Mu (Anemarrhena Rhizome) and Hua Shi (Talcum) each 1 qian.
In cases of frequent urination after childbirth, it is caused by pre-existing cold qi in the abdomen manifesting due to deficiency. Sang Piao Xiao San (Mantis Egg-case Powder) is appropriate, or one may grind Yi Zhi Ren (Alpinia Oxyphylla Fruit) into powder and take 2 qian with rice water. If dysuria with pain occurs, it is also due to maternal weakness allowing cold qi to lodge in the abdomen; Bai Mao Gen Tang (Imperata Rhizome Decoction) should be used.
Sang Piao Xiao San: Sang Piao Xiao (Mantis Egg-case) 12 pieces, Ren Shen (Ginseng Radix et Rhizoma) 3 liang, Huang Qi (Astragalus Root) 3 liang, Lu Rong (Deer Antler without Horn) and Mu Li (Oyster Shell) and Chi Shi Zhi (Red Halloysite) each 2 liang. Grind all into fine powder. Take orally with rice water on an empty stomach.
Bai Mao Gen Tang (Imperata Rhizome Decoction): Bai Mao Gen (Imperata Rhizome), Qu Mai (Dianthus Herba), Fu Ling (Poria), Che Qian Zi (Plantago Seed), Ren Shen (Ginseng Radix et Rhizoma), Hua Shi (Talcum), Tong Cao (Tetrapanacis Medulla), Mai Dong (Ophiopogon Tuber), Zhi Gan Cao (Honey-fried Licorice Root). Add a few stems of Deng Xin Cao (Junci Medulla) and decoct for administration.
Additionally, an Imperial Secret Recipe: Use thick wash water from aged rice, clarify it, and drink it hot; urination will immediately flow smoothly.
Note: For treatment of Xue Lin (Blood Strangury), add Huai Niu Xi (Achyranthes Bidentata Root).
