Both Qi and Blood are deficient, Ying and Wei (Defensive and Nutritive Qi) are not secure, and the Spleen and Stomach have not yet recovered. External contraction of Wind-Cold or internal injury from diet can all lead to Malaria. There are also cases where Malaria was contracted before pregnancy and has not resolved postpartum. Postpartum Malaria is the most difficult to regulate; treatment should primarily focus on tonifying deficiency and supporting Righteous Qi. When Righteous Qi prevails, Pathogenic Qi declines daily. One must not lightly use antimalarial astringent drugs, as this would severely damage Righteous Qi and cause great harm. Use Zengsun Chaihu Siwu Decoction: Ta Dangshen (Codonopsis), Bei Chaihu (Bupleurum), Banxia (Pinellia), Zhigancao (Prepared Licorice), Guishen (Radix Angelicae Sinensis, wine-washed), Chuanxiong (Rhizoma Chuanxiong).
Qi and Blood are both deficient, Ying and Wei lose their control, daily life lacks regularity, and nursing is improper. Injury by Wind affects Wei, while injury by Cold affects Ying, resulting in Cold Damage (Shanghan). Treatment focuses solely on tonifying deficiency, treating other symptoms secondarily. Use Wuwu Decoction: Ta Dangshen, Chuanxiong, Guishen, Baishao (Radix Paeoniae Alba, wine-fried), Zhigancao.
Those with sweating are diagnosed as Wind Injury; add Guizhi (Cinnamon Twig) and Fangfeng (Saposhnikovia) to this formula. Those without sweating are diagnosed as Cold Damage; add Mahuang (Ephedra) and Ziye (Perilla Leaf) to this formula. Alternating chills and fever; add Chaihu (Bupleurum) to this formula. Headache; add Baizhi (Angelica Dahurica) and Xixin (Asarum) to this formula. Generalized body pain; add Qianghuo (Notopterygium) and Cangzhu (Atractylodes Lancea) to this formula. Fever only without aversion to cold; add Chaihu and Gegen (Pueraria) to this formula. Fever accompanied by thirst; add Zhimu (Anemarrhena) and Maidong (Ophiopogon) and Danzhuyeqian (Lophatherum) to this formula.
The Kidney in women is connected to the Bao Mai (Uterine Vessel). Excessive blood loss postpartum leads to emptiness of the Bao Mai; vessel emptiness leads to Kidney Qi deficiency. Since the Kidney governs the waist, this causes lower back pain. The symptom is a dull, lingering pain. Use Bushen Dihuang Decoction: Shudi (Prepared Rehmannia), Guishen, Duzhong (Eucommia Bark, stir-fried with light salt water until fibers are removed), Duohuo (Notopterygium), Rougui (Cinnamon Bark), Xuduan (Dipsacus) each 1 qian.
Alternatively, residual blood flows into the Kidney Meridian, blocking the Dai Mai (Girdle Vessel), causing lower back pain. The symptom is distending pain like stabbing, intermittent, and intolerable to touch. Use Jiawei Fuyuan Tongqi Powder: Guishen, Chuanxiong, Xiaohui (Fennel, stir-fried), Huzi (Pepper, stir-fried and crushed), Yuanhu (Corydalis), Niuxi (Achyranthes), Rougui, Danpi (Moutan) each 1 qian. Decoct in water. Additionally, take Muxiang (Aucklandia) powder, Ruxiang (Frankincense) powder, and Moyao (Myrrh) powder, each 5 fen, mixed evenly, taken on an empty stomach.
Injury to Kidney Qi and Dai Mai due to rising, falling, twisting, or spraining during childbirth can also cause lower back pain. Use the aforementioned Jiawei Fuyuan Tongqi Powder for administration.
