Whenever treating diseases and administering medication, if the treatment is appropriate, there is no case where it fails to be effective. Sha disease (a condition caused by pathogenic factors) arises from contracting the abnormal Qi of the four seasons; therefore, the primary approach should be to expel pathogens, while nourishing righteousness (Zheng Qi) is not the priority. Treatment should favor dispersing and diffusing rather than heavy sweating; descending and purging rather than lifting and raising; cooling and resolving rather than acrid and hot; clearing and unblocking rather than stagnating and astringent; digesting and guiding out rather than tonifying; opening and unblocking rather than consolidating; moving Qi rather than supplementing Qi; invigorating blood rather than nourishing blood. Assist with detoxification, combine with clearing fire, transform Qi to eliminate distension, move blood to drive out pathogens. This is the general principle of medication. Furthermore, one must tailor the treatment to the specific disease. Using herbs like Jingjie (Schizonepetae Herba) and Fangfeng (Saposhnikoviae Radix) is for dispersing from the exterior. Using herbs like Chenpi (Citri Reticulatae Pericarpium) and Qingpi (Citri Reticulatae Pericarpium Viride) is for eliminating from the middle jiao. Using herbs like Zhishi (Aurantii Fructus Immaturus) and Dahuang (Rhei Radix et Rhizoma) is for downward expulsion via the large intestine. Using herbs like Mutong (Akebiae Caulis) and Zexie (Alismatis Rhizoma) is for downward expulsion via the small intestine. Shanzha (Crataegi Fructus) and Laifuzi (Radish Seed) are used to treat food obstruction; Binglang (Arecae Semen) and Ezhu (Curcumae Rhizoma) are used to drive out accumulation stagnation; Xiangfu (Cyperi Rhizoma) and Sharen (Amomi Fructus) are used to open Qi closure; Honghua (Carthami Flos) and Yinhua (Lonicerae Japonicae Flos) are used to invigorate stagnant blood. This is the general principle of using medication according to the disease.
If medication is administered incorrectly, danger and危重 conditions will appear immediately. Knowing the contraindications in advance means half the battle is won. Renshen (Ginseng Radix), [text missing], Baizhu (Atractylodis Macrocephalae Rhizoma), and Shanyao (Dioscoreae Rhizoma) may tonify toxic Qi and are strictly forbidden in Sha disease. Shudihuang (Rehmanniae Radix Praeparata) and Baishao (Paeoniae Radix Alba) nourish blood and astringe blood, which are strictly taboo for Sha disease. Gancao (Glycyrrhizae Radix et Rhizoma) should be avoided as it may cause Sha masses, making treatment difficult. All sweet-tasting substances are unsuitable. Fuling (Poria) and Zhuling (Polyporus) may promote fluid metabolism but could solidify Sha Qi, so they are included in the prohibitions. Banxia (Pinelliae Rhizoma), Baizhi (Angelicae Dahuricae Radix), and Cangzhu (Atractylodis Rhizoma) have drying properties and are contraindicated. Shengma (Cimicifugae Rhizoma) is contraindicated, as it may lift Sha Qi upward, making it difficult to restrain. Mahuang (Ephedrae Herba) causes excessive sweating and is prohibited. Rougui (Cinnamomi Cortex), Fuzi (Aconiti Lateralis Radix Praeparata), and Wuzhuyu (Evodiae Fructus) are prohibited, as they may assist toxicity and cause immediate changes. Ganjiang (Zingiberis Rhizoma) can assist heat toxicity and should be avoided (if caused by excessive consumption of cold water, it may be used sparingly to disperse cold). Wuweizi (Schisandrae Fructus) and Mugua (Chaenomelis Fructus) have sour and astringent properties and are contraindicated. Zhuli (Bambusa Textilis Catillus Juice) is cold in nature and contraindicated; if used, Ginger Juice must be added to guide it through the meridians, but it is better not to use it at all. Duzhong (Eucommiae Cortex), Buguzhi (Bastard's Cistanche Fruit), and Gouqizi (Lycii Fructus) should not be used even for lower back pain. Fushen (Poria with Hostwood), Baiziren (Biota Seed), and Suanzaoren (Ziziphi Spinosae Semen) should not be used even for deficiency-related irritability and insomnia. Congrongu (Cistanches Herba) and Bajitian (Morinda Officinalis Root) are especially taboo.
Commentary: Banxia and Huoxiang can stop vomiting. In general vomiting cases, Banxia and Huoxiang are used; however, for Sha disease-induced vomiting, Banxia's drying nature must be guarded against as it may exacerbate fire pathogens, so it is absolutely contraindicated. Huoxiang (Patchouliae Leaves) is only used for its pure Qi to treat turbid pathogenic factors. If there is food accumulation or blood stasis in the intestines and stomach blocking Sha toxins, suddenly using these herbs to stop vomiting carries the risk of "locking the door to chase away the thief" (trapping the pathogen inside).
