Wherever the affected area presents with red skin, swelling, and pain, with a diameter of about one cun, this is termed carbuncle-toxin (Yong Du). If three to four days have passed since onset and pus has not yet formed, apply Dongtian Nen Gao externally, and internally administer Xingxiao Wan, taken with three qian of warm aged rice wine. This will immediately stop the pain, allow for sleep at night, and cause the skin to wrinkle the next day; subsequent doses will lead to complete resolution. If four or five days have passed and pus is about to form, Xingxiao Wan should also be administered to reduce swelling and relieve pain. If pus has formed causing swelling but the skin has not yet ruptured, Dai Dao San taken with wine will induce rupture; applying Dongtian Gao externally will result in healing within a few days. If the redness and swelling span several cun and are located in critical areas such as the mid-back, back of the head, waist, abdomen, axilla, or scrotum, Wutong Wan and Xingxiao Wan should be taken alternately morning and evening, delivered via Baidu Tang decoction; once the skin wrinkles and pain subsides, it indicates improvement. If the lesion has already ruptured, Tuodu San and Xingxiao Wan taken alternately morning and evening will yield miraculous effects. If the redness and swelling are less than one cun in diameter, it is named furuncle (Jie); Chan Su Wan and Meihua Dianshe Dan will effect a cure. These are empirical prescriptions by Linwu Shanshen.
(This formula is found in the Lung Abscess section.)
Intestinal abscess (Chang Yong) may originate inside the intestines, presenting with abdominal distension and urgency, pulling pain during urination and defecation resembling strangury (Lin), and a splashing sound upon turning or shaking the body; after ulceration, pus is discharged through the stool. It may also originate outside the intestines, presenting with swelling, pain, and distension around the umbilicus, flexion of one leg, foul breath, or pus discharging from the navel; in severe cases, intestinal perforation occurs with worms emerging from the navel, a condition that is difficult to manage. In the initial stage, Mudan Pi San should be used to dissipate the pathogen; after ulceration, tonifying internal support agents containing Ginseng (Renshen) should be employed.
Mudan Pi San: Radix Paeoniae Rubra (Danpi) 5 qian, Semen Coicis (Yiren) 1 liang, Fructus Trichosanthis (Gualou Ren) [de-oiled] 2 qian, Semen Persicae (Taoren) [skin and tip removed] 20 grains. Decoct in water and take orally.
Another prescription: Administer Liu Yi San frequently (see Formulas for Internal and External Use), allowing pus and blood to discharge through the stool; recovery occurs once the pus is exhausted. This is an extremely effective and stable formula.
Another prescription: Spina Gleditsiae Sinensis (Zaojiaoci) 1 liang, no less than 8 qian. Decoct in wine until reduced to seventy percent volume, and take warm. Recovery occurs when pus and blood discharge through urination and defecation. For those who do not consume alcohol, decocting in water is also acceptable.
Another prescription: Rhizoma et Radix Rhei (Dahuang) [stir-fried], Natrii Sulfas (Puxiao) each 1 qian; Radix Paeoniae Rubra (Danpi), Semen Mustardae Albae (Baijiezi), Semen Persicae (Taoren) each 2 qian. Decoct and take on an empty stomach. The lower abdomen is hard and hot.
