When the smallpox toxin has dissipated and the primordial qi (Yuan Qi) has not yet recovered, the spirit is fatigued, leading to drowsiness and a desire to sleep. Even if unresponsive to calls, this is generally harmless. However, if the patient remains unconscious for several days, muttering to themselves like a drunken person and often speaking deliriously, this indicates pathogenic heat invading the Pericardium (Xin Bao). It is appropriate to use Daochi Jiedu Tang (Red Guide Decoction for Detoxification) or Anshen Wan (Calming Spirit Pill) as the primary treatment.
Daochi Jiedu Tang: Tongcao (Akebia Stem), Plantaginis Semen (Cheqianzi), Rehmanniae Radix Recens (Shengdihuang), Ophiopogonis Radix (Maidong), Glycyrrhizae Radix et Rhizoma (Gancao), Poria (Fushen), Acori Tatarinowii Rhizoma (Shichangpu), Gardeniae Fructus (Zhizi), taken with decoction of Ginseng Radix et Rhizoma (Renshen) and Junci Medulla (Dengxincao).
Anshen Wan: Calculus Bovis (Niuhuang) 5 fen, Coptidis Rhizoma (Huanglian) 5 qian, Angelicae Sinensis Radix (Danggui), and Gardeniae Fructus stir-fried (Chao Zhizi) each 2.5 qian. All are ground into fine powder, mixed evenly with heart blood of pig, formed into pills, coated with Cinnabar (Zhusha), and taken with Dengxincao decoction. If genuine Niuhuang is unavailable, add Fritillariae Thunbergii Bulbus (Chuanbei), Arisaematis Cum Bile (Danning), and Succinum (Hupo).
Scholar's branch nodes (Shanshu jie), Terminalia chebula Retz. fruit (Hezi), Zingiberis Rhizoma (Shengjiang), and Difangxiatu (likely a textual error for a root herb, possibly Gouqi Gen or similar), if cold syndrome is present, add Piperis Fructum (Hujiao) and decoct in soup; mix with bamboo oil for administration.
Another method: Scallion (Cong), Ginger (Jiang), and clear oil are stir-fried together until hot, then rubbed over the entire body. Followed by moxibustion at one point below the throat, three points on the chest, and one point below both breasts using a lit lamp fire.
This condition arises from residual toxins returning to the Heart. Use Xiangru (Frankincense) 2 qian, decoct in water and take orally. Also, take 4 liang of white sugar dissolved in wine (water can also be used) or take Huxin San (Heart-Protecting Powder).
Phellodendri Cortex (Huangbai) and Sophorae Flavescentis Radix (Kushen) are ground into powder, mixed with clear oil, boiled into a paste, and applied topically.
Another method: Leaves of Toona sinensis (Tongzi ye) are softened in boiling water, coated with clear oil and Realgar (Xionghuang), and applied to the affected area for 3-5 times.
Another method: Dozens of lamp wick flowers (Denghua) are stir-fried with salt, mixed with lead powder (Qianfen) to form a cake, and applied to the umbilicus.
Treat according to the formulas for post-disease edema in the Drum Distension section, which is the simplest and safest approach. If ineffective, treat according to the following specific symptoms.
After smallpox, when pus and blood are exhausted, Spleen Earth is deficient, and both Qi and Blood are depleted. Pathogens easily invade, and food is easily damaged. Thus, whether due to Wind-Damp invasion, dietary injury, or lingering toxins, edema may occur. In cases of Wind-Cold invasion, swelling typically starts on the face, accompanied by fever and thirst. Use Shengma, Chaihu, Fuling, Mutong, Cheqian, Fangfeng, Dafupi, Gancao, with Jiang and Cong juice as guides, decoct and take orally. Mild sweating will cause the swelling to subside.
In cases of swelling due to Dampness, it usually starts on the feet, appearing shiny, leaving an indentation upon pressure, and developing slowly. The patient may have a preference for drinking, with dark and scanty urine, and washing with water may worsen the swelling; this is Edema (Shui Zhong). Treatment should focus on strengthening the Spleen and promoting urination. Use Cangzhu, Houpo, Mutong, Cheqian, Huashi, Qumai, Zexie, Zhuling, Fulingpi, Dafupi, to make urine clear and profuse, causing the swelling to resolve. Externally, smoke Cangzhu and Tanxiang, cover the patient with a blanket to induce sweating out dampness, and the swelling will recede.
In cases of Food Stagnation combined with lingering toxins, swelling often starts in the abdomen, accompanied by dull abdominal pain, loss of appetite, slight abdominal heat, yellow and foul-smelling stool, and dark, scanty urine; this is swelling due to stagnation. Treatment should focus on resolving stagnation and detoxifying. Use Cangzhu, Houpo, Chenpi, Dafupi, Xiangfu, Chuanxiong, Shanzha, Shenqu, Niubang, Lianqiao, Mutong, Cheqian, Fuling, Banxia, Gancao.
In cases where Spleen Qi fails to circulate, resulting in edema, the skin resembles a drum and is difficult to press down; this is Qi Edema. Treatment should focus on regulating Qi. Use Renshen Wupi San (Ginseng Five-Peel Powder): Renshen, Baizhu, Guigui, Maidong, Dafupi, Chenpi, Sangpi, Jiangpi, Fulingpi, Muxiang, Zexie, Cheqian, Mutong.
In cases where Blood circulation is impaired, resulting in swelling, there may be a hidden sensation of blood vessels under the skin; this is Blood Edema. Treatment should focus on invigorating Blood. Use Siwu Tang added with Renshen, Chenpi, Xiangfu, Mutong, and Fuling. Once Blood circulation is restored, the swelling will subside.
Swelling of the face, eyes, and limbs belongs to the Spleen, caused by exterior deficiency allowing Wind-Cold invasion. Sweating should be induced to resolve the swelling. Use Jiawei Wupi San (Modified Five-Peel Powder): Qianghuo, Fangfeng, Guizhi, Fangji, Cangzhu, Mutong, Wujiapi, Sangpi, Dafupi, Fulingpi, Jiangpi, Zhuling, Gancao.
Abdominal distension resembling a drum with slight swelling of the eyelids belongs to the Spleen, caused by inherent weakness of the Spleen and Stomach. Weak Earth cannot control Water, leading to accumulation and failure to circulate, thus causing edema. Alternatively, food stagnation prevents Spleen digestion, leading to internal accumulation of Damp-Heat and swelling. Both conditions are treated with Houpo Tang: Cangzhu, Houpo, Chenpi, Dafupi, Fulingpi, Zhuling, Muxiang. If the scrotum is also swollen (Edema type), add Zexie, Huashi, Cheqian, Mutong, Tingli. If due to stagnation, add Shanzha, Shenqu, Sanleng, Ezhu.
Laifu Wan: Treats all types of edema. Laofuzi (stir-fried), Hujiao, Houpo each 5 qian, Baizhu 1 liang. Ground into powder and made into pills... [Text cuts off].
In cases of weakness with failed Qi circulation, grind Rougui in wine and take orally; externally rub with burning wine. Recovery will follow.
