This condition does not solely belong to the throat; it is caused by Yin deficiency with flaming fire, damaging both the Lung and Kidney. Symptoms may include coughing, hemoptysis (coughing up blood), tidal fever, dry throat, and dry tongue, with pain that comes and goes. The treatment is roughly the same as for sore throat due to Yin deficiency; one may combine Yinhua Sijunzi Tang (Silver Flower Four Gentlemen Decoction) with Shenmai San (Ginseng and Ophiopogon Powder), with the key being to simultaneously regulate the Spleen and Stomach. However, this is extremely difficult. It is essential that the patient completely abstains from alcohol and sexual activity, restrains anger and emotional agitation, and avoids consuming new chickens, ducklings, geese, mutton, fish, shrimp, fried or roasted foods with strong flavors, as well as pungent and stimulating foods like ginger and pepper. Only then can the treatment be effective!
Cold syndromes of the throat have always been rare, occurring only occasionally in one or two out of a hundred cases. The symptoms present as throat pain without redness or swelling, or perhaps slightly whitish; there is no thirst, though the patient prefers hot drinks but cannot drink much; urination is clear and copious; there is abdominal pain and coldness, diarrhea, cold limbs, heaviness of the body, aversion to cold, profuse flow of clear saliva from the throat, occasional dry heaving, a slippery and moist tongue coating, and a deep, faint, and fine pulse. This is a cold syndrome of the Foot Shaoyin Kidney Meridian. Because the person's true Yang within the Kidney is originally deficient, cold pathogenic factors invade the meridian directly through the deficiency, forcing the slight Yang to float upward, resulting in throat pain. This is a syndrome of pure Yin without Yang, hence named Yin Syndrome Throat Bi (Obstruction). One must understand that the pathological origins of Yang and Yin Throat Bi are as opposite as ice and charcoal, leading to a vast difference in medication. Moreover, since cold syndromes of the throat are inherently rare, if it is indeed the aforementioned Yin-cold syndrome, careful examination and accurate differentiation are required before administering medicine. Regardless of winter or summer, Si Ni Tang (Frigid Extremities Decoction) or Jiang Fu Li Zhong Tang (Ginger and Aconite Regulate the Middle Decoction) should be used, and the disease will heal spontaneously. One must strictly avoid formulas for releasing the exterior, clearing heat, descending, or purging with cold. If it is not a cold syndrome, the mistaken use of Ginger and Aconite will be incurable!
Hoarseness of the voice must be distinguished as either acute or chronic. Acute onset conditions are all treatable. For instance, if throat pain arises within four or five days, it is an acute condition; once the throat pain is cured, the voice will naturally recover. If accompanied by heavy coughing, expectoration of thick phlegm, aversion to cold, fever, headache, redness and swelling of the throat, and hoarseness, this indicates Heat binding in the Lung and Cold constraining the exterior. Cooling decoctions should be administered orally, and Yuanzhu Dan (Pearl Pill) should be insufflated into the throat. Once the throat improves slightly, Su Ye, Jing Jie, Fang Feng, and Zhiju Erchen Tang (Perilla Leaf, Schizonepeta, Saposhnikovia, and Bitter Orange and Two Old Decoction) should be used to disperse the pathogen. Alternatively, if hoarseness, mild pain, and loss of voice result from shouting loudly, Bu Zhong Yi Qi Tang (Tonify the Middle and Augment the Qi Decoction) should be used. These are established methods for treating acute loss of voice. As for chronic loss of voice, it belongs to Consumption (Xu Lao) syndrome, termed "Metal Broken Does Not Ring," and is therefore mostly incurable.
