In the early years of the Tongzhi reign, while serving as an official in Guangdong, I accidentally suffered from an eye disease. Guo Yulu, the Circuit Intendant (courtesy name: Xiangrui), bestowed upon me a volume of "Ophthalmology Manuscript," claiming it was passed down from Mr. Ye Tianshi. Recorded by Mei Qizhao (style name: Xiaoyan), Envoy of Zhejiang, in January of the 4th year of Guangxu.
The pupil belongs to the Kidney; the size and movement of the black pearl (iris/cornea area) relate to the Heart; the black pearl itself belongs to the Liver; the upper eyelid belongs to the Spleen; the white pearl (sclera) belongs to the Lung; the lower eyelid belongs to the Stomach.
For cases where the sclera has fine red vessels and slight pain, use heat-clearing and dispersing herbs. The suitable prescription is Zuo Jingfang Decoction: Schizonepetae Herba (Jingjie) 8 fen, Viticis Fructus (Manjingzi) 8 fen, Paeoniae Radix Rubra (Chishao) 8 fen, Chuanxiong Rhizoma (Chuanxiong) 8 fen, Saposhnikoviae Radix (Fangfeng) 8 fen, Plantaginis Semen (Cheqianzi) 1 qian, Bombyx Batryticatus (Chantui) 6 fen (wings and feet removed), Chrysanthemi Flos (Juhua) 1 qian, Rehmanniae Radix (Shengdi) 1.5 qian (sliced), Qingxiangzi [Qingqi]...
For cases where the canthus is red and swollen, it is due to Heart Fire. To the previous formula, add Scutellariae Radix (Huangqin) 8 fen (stir-fried with wine), Akebiae Caulis (Mutong) 8 fen, and use Lophatheri Herba (Dan Zhuye) 9 leaves as a guiding herb.
For cases where the entire sclera is red, accompanied by swelling, pain, discharge (discharge from eyes), tearing, and photophobia, it indicates excessive Fire. The suitable prescription is Liangxue Sanhuo Decoction: Rehmanniae Radix (Shengdi) 2 qian (sliced), Moutan Cortex (Danpi) 8 fen, Paeoniae Radix Rubra (Chishao) 8 fen, Scutellariae Radix (Huangqin) 8 fen, Saposhnikoviae Radix (Fangfeng) 8 fen, Schizonepetae Herba (Jingjie) 8 fen, Angelicae Sinensis Radix (Guiwei) 8 fen, Bombyx Batryticatus (Chantui) 6 fen, Bupleuri Radix (Chaihu) 8 fen, Plantaginis Semen (Cheqianzi) 1 qian. If there is headache, aversion to wind, or fever, add Notopterygii Rhizoma et Radix (Qianghuo) 8 fen. If eye pain is unbearable or there is thirst, add Coptidis Rhizoma (Chuanlian) 8 fen (stir-fried with wine). If swelling does not subside or redness does not recede, add Carthami Flos (Honghua) 4 fen.
For cases where the canthus is pale red or red and painful, it is due to Deficient Fire of the Heart. The suitable prescription is Yangxue Sanhuo Decoction: Rehmanniae Radix (Shengdi) 1 qian (sliced), Moutan Cortex (Danpi) 8 fen... If after taking this medicine the redness and pain both resolve, but vision remains unclear, remove Schizonepetae Herba (Jingjie) and Saposhnikoviae Radix (Fangfeng), add Astragali Complanati Semen (Shayuanjili) 1 qian (stir-fried with light salt water), and Cuscutae Semen (Tusizi)...
For cases where the periphery of the black pearl is red, it is due to Liver Fire. Whether the pain is extreme or slight, the suitable prescription is Xiegan Decoction: Bupleuri Radix (Chaihu) 8 fen, Saposhnikoviae Radix (Fangfeng) 6 fen, Schizonepetae Herba (Jingjie) 6 fen, Chuanxiong Rhizoma (Chuanxiong) 6 fen, Angelicae Sinensis Radix (Guiwei) 8 fen, Paeoniae Radix Rubra (Chishao) 8 fen, Chrysanthemi Flos (Juhua) 8 fen, Gardeniae Fructus (Zhiren) 8 fen (stir-fried with wine), Citri Reticulatae Pericarpium Viride (Qingpi) 8 fen, Plantaginis Semen (Cheqianzi)...
For cases where the eyelid margin is itchy and ulcerated, it is due to Wind. Internally take the Sousong San (Wind-Searching Powder); externally apply Ruiren Ointment. Saposhnikoviae Radix (Fangfeng) 6 fen, Schizonepetae Herba (Jingjie) 6 fen, Pruni Semen (Ruiren) 8 fen, Tribuli Fructus (Cijili) 1 qian, Chrysanthemi Flos (Juhua) 1 qian, Bombyx Batryticatus (Chantui) 6 fen, Glycyrrhizae Radix et Rhizoma (Gancao) 4 fen, Sparganii Rhizoma (Gujingcao) 6 fen, Paeoniae Radix Rubra (Chishao) 8 fen, Plantaginis Semen (Cheqianzi) 1 qian, plus one thin slice of old ginger as a guiding herb.
External wash preparation: Notopterygii Rhizoma et Radix (Qianghuo) 1.5 qian, Saposhnikoviae Radix (Fangfeng) 1.5 qian, Alum (Danfan) 4 fen, Mori Folium (Dongsangye) 7 leaves. Boil in water for fumigation and washing. Alternatively, dissolve Alum in hot water and gently rub frequently on the itchy and ulcerated areas, which is also effective.
Ruiren Ointment Formula: Pruni Semen (Ruiren) (soaked in water and peeled) 1 liang, pounded into a paste, boiled in two bowls of water until reduced to the volume of one wine cup, filtered to remove dregs, heated again, then mixed with ground... [Text incomplete].
For cases where corneal opacity covers the entire black pearl, but is slightly thinner at the pupil, causing somewhat unclear vision, it is still treatable. Use Bo Yun San (Cloud-Dispersing Powder) with modifications.
For cases where corneal opacity covers the entire eyeball, the pupil area appears green with no visible pupil, and the opacity is protruding, named "Green Water Flooding the Pupil," it is incurable.
For cases where a single red vessel runs through the pupil, it indicates Heart Fire attacking the Kidney. Use Jiawei Daochi San (Modified Guide Out the Heat Powder): Rehmanniae Radix (Shengdi) 1.5 qian (sliced), Akebiae Caulis (Mutong) 8 fen, Glycyrrhizae Radix et Rhizoma (Gancao)... Paeoniae Radix Rubra (Chishao)...
For cases where flesh grows from the canthus onto the black pearl, named "Pterygium" (Meat Climbing over the Eye), internally take Jiawei Daochi San (Modified Guide Out the Heat Powder); externally apply Xiaolu San (Nitre and Furnace Powder). Rehmanniae Radix (Shengdi) 1.5 qian (sliced), Akebiae Caulis (Mutong) 8 fen, Carthami Flos (Honghua) 4 fen, Paeoniae Radix Rubra (Chishao) 8 fen, Saposhnikoviae Radix (Fangfeng) 6 fen, Schizonepetae Herba (Jingjie) 6 fen, Bombyx Batryticatus (Chantui) 8 fen, Glycyrrhizae Radix et Rhizoma (Gancao) 4 fen, Angelicae Sinensis Radix (Guiwei) 8 fen, one thin slice of ginger as a guiding herb. If painful, add Scutellariae Radix (Huangqin) 8 fen (stir-fried with wine). If itchy, add Pruni Semen (Ruiren) 8 fen, Tribuli Fructus (Cijili) 8 fen.
Xiaolu San (Nitres and Furnace Powder): Float-water Gypsum (Fushui Ganshi) as needed, calcined until red, quenched seven times in child's urine, dried, and ground to an extremely fine powder, washed with water three times. Then mix with equal parts of Notopterygii Rhizoma et Radix (Qianghuo), Saposhnikoviae Radix (Fangfeng), Viticis Fructus (Manjingzi), Chuanxiong Rhizoma (Chuanxiong), Angelicae Dahuricae Radix (Baizhi), Coptidis Rhizoma (Chuanlian), Scutellariae Radix (Huangqin), and Chrysanthemi Flos (Juhua). Decoct into a thick liquid, mix thoroughly with the Gypsum powder, dry, and set aside. Take 1 qian of prepared Gypsum, 3 fen of Nitrum (Huoxiao), [ratio/text unclear], and 1 fen of Borax (Bingpian), grind uniformly, and apply to the eyes.
For cases where the eyes are not red, swollen, or painful, it is due to Qi Stagnation. For cases where the eyes are not red, swollen, or painful, but the eyelids droop and vision is unclear, it is due to Qi Deficiency. Equiseti Herba (Muzei) 1 liang, Saposhnikoviae Radix (Fangfeng) 6 qian, Bupleuri Radix (Chaihu) 6 qian, Qingxiangzi [Qingqi] 8 qian, Bombyx Batryticatus (Chantui) 1 liang, Scutellariae Radix (Huangqin) 6 qian, Chrysanthemi Flos (Juhua) 8 qian, Plantaginis Semen (Cheqianzi) 8 qian.
