When the vulva protrudes due to excessive pushing during labor, administer Siwu Tang (Four Substances Decoction) with a small amount of Longgu (Fossilized Bones), taken for two doses. Externally, apply Rusheng Gao (Rusheng Paste) to the vertex; wipe it off immediately after slight retraction occurs.
Postpartum lactation failure has two causes: one is deficiency of both Qi and Blood, resulting in scanty milk; the other is Qi stagnation and Blood stasis, causing obstruction and non-flow. Women often suffer from worry, thought, anger, and indignation. Worry and thought cause Qi to knot and Blood to congeal; anger and indignation cause Qi to rebel and Blood to flow counter-currently. This may even lead to breast hardness, duct blockage, hypochondriac pain, and irritability with heat. Therefore, ancient formulas for promoting lactation often use Xiangfu (Cyperus Rhizome), Muxiang (Aucklandiae Radix), and Qingpi (Green Tangerine Peel) to move Qi. Wangbulexing (Vaccariae Semen) is used because it promotes lactation while also moving Blood. If the cause is solely Qi and Blood deficiency, there is no need to use Qi-moving herbs; instead, one should consume Pig Trotter Soup or Milk Drink.
Pig Trotter Soup: One pig trotter, five qian of Tongcao (Tetrapanacis Medulla), decocted in water and taken orally.
Zhongru Yin (Calcitriol Drink): Two qian of Zhongru (Ostreae Concha processed with calcium compounds), three qian of Loulu (Euphorbiae Humifusae Herba), decocted in water and taken orally. Another prescription: Crucian carp and Mutong (Akebiae Caulis), boiled into juice and consumed.
For a nursing mother who has no child to nurse and wishes to suppress lactation, use two liang of Maiya (Hordei Fructus Germinatus), stir-fried and ground into powder, mixed with Siwu Tang (Four Substances Decoction) and taken orally to eliminate milk.
Another prescription: Use two qian of Shenqu (Massa Medicata Fermentata), ground into powder, swallowed with wine, taken once daily.
A mass resembling an eggplant protrudes from the vagina postpartum. This is the amniotic sac (fetal membrane), known as "Qie Zheng" (Eggplant Syndrome). It is caused by excessive exertion during delivery leading to the sinking of Yuan Qi (Original Qi). Treatment requires Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) to lift and raise, allowing gradual retraction. The formula for Buzhong Yiqi Tang is listed under the section "Ziman" (Excessive Fullness During Pregnancy).
A mass resembling a bowl protrudes from the vagina postpartum. If it has two lobes, it is the uterus. Administer Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) without Chaihu (Bupleuri Radix), taken continuously for two to three doses, and it will retract quickly. Afterwards, regulate with Siwu Tang (Four Substances Decoction) plus Renshen (Ginseng Radix et Rhizoma).
Note: Since Shaoyao (Paeoniae Radix) is contained in Siwu Tang (Four Substances Decoction), it is ultimately better to use it sparingly; reconsider after seven days.
A piece of flesh resembling a mushroom or a rooster's comb, approximately one cun long, or even up to one chi long, protrudes from the vagina postpartum. This is called "Yinting Xiaduo" (Prolapse of the Vulva). It can also occur in ordinary women, generally due to the sinking of Yuan Qi (Original Qi). The treatment method is similar to that for Qie Zheng (Eggplant Syndrome); use Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction). In the case of postpartum conditions, remove Chaihu (Bupleuri Radix) from this formula, as Shengma (Cimicifugae Rhizoma) lifts Spleen Qi, leading to natural recovery.
