Postpartum breast carbuncle and abscess, initially presenting with alternating chills and fever resembling malaria, should not be treated as malaria, nor should it be entirely managed with detoxifying and resolving abscess agents. It is necessary to greatly tonify Qi and Blood, assisted by Honeysuckle Flower (Jinyinhua) etc. to disperse pathogenic toxins. If pus has been discharged and the patient's weakness worsens daily, Shiquan Dabu Tang (Ten-Ingredient Great Tonifying Decoction) is appropriate.
Gualou Ruxiang San: One Gualou (Trichosanthes fruit, crushed with peel and seeds), Danggui (Angelica sinensis root) 3 qian, Jing Yinhua (Honeysuckle flower) 3 qian, Baizhi (Angelica dahurica root) 1 qian, Qingpi (Citrus reticulata green peel) 5 fen, Ruxiang (Frankincense resin) 1 qian, for draining pus and supporting interior.
Painouring Dabu Tang: Ren Shen (Ginseng root) 2 qian, Baizhu (Atractylodes macrocephala rhizome) 2 qian, Shengdi (Rehmannia glutinosa root) 2 qian, Yinhua (Honeysuckle flower) 2 qian, Danggui (Angelica sinensis root) 3 qian, Fuling (Poria sclerotium) 1 qian, Chuanlian (Coptis chinensis rhizome) 5 fen, Huangqin (Scutellaria baicalensis root) 1 qian, Qingpi (Citrus reticulata green peel) 3 fen, Wumei (Mume fruit) 1 piece, Yuanzao (Jujube fruit) 1 piece. This formula can include Baizhi (Angelica dahurica root) 8 fen, as it can drain pus and support the interior.
Modified Shiquan Dabu Tang: Ren Shen (Ginseng root) 2 qian, Baizhu (Atractylodes macrocephala rhizome) 2 qian, Danggui (Angelica sinensis root) 2 qian, Shengdi (Rehmannia glutinosa root) 2 qian, Huangqi (Astragalus membranaceus root) 2 qian, Fuling (Poria sclerotium) 8 fen, Chuanxiong (Ligusticum chuanxiong rhizome) 8 fen, Gancao (Licorice root) 5 fen, Yuanzhi (Polygala tenuifolia root) 1 qian, Yinhua (Honeysuckle flower) 3 qian, decocted in water. For diarrhea, add 14 Lianzi (Lotus seeds) and 1 Rougu (Nutmeg); for thirst, add Maidong (Ophiopogon japonicus tuber) and Wuweizi (Schisandra chinensis fruit); for non-healing wounds over time, apply a plaster containing Ginseng powder.
Postpartum lochia flows into the Kidney, waist, and joint areas, causing diffuse swelling or nodular pain. Over time, swelling increases, accompanied by limb fatigue. In urgent cases, use Scallion Moxibustion to dissipate external swelling, internally administer Shengu Huasheng Tang to resolve blood stasis. For both ulcerated and non-ulcerated conditions, refer to the treatment methods for Breast Abscess.
Scallion Moxibustion: Crush one handful of fresh scallions into a cake, apply to the affected area, cover with three to four layers of thick cloth, and iron with a hot fire container for immediate effect. This method can treat early-stage carbuncles and abscesses.
Shengu Huasheng Tang: Huangqi (Astragalus membranaceus root) 1.5 qian, Chuanxiong (Ligusticum chuanxiong rhizome) 1.5 qian, Danggui (Angelica sinensis root) 3 qian, Ren Shen (Ginseng root) 2 qian, Rougui (Cinnamon bark) 8 fen, Gancao (Licorice root) 5 fen, Mati Xiang (Costus root) 2 qian.
Excessive effort during childbirth leads to swelling and pain on both sides of the vulva, with inability to extend hands and feet. Use seasonal scallions mixed with Ruxiang (Frankincense resin) powder, crush into a cake, and apply to the swollen area; recovery occurs after a long period.
