Vomiting caused by measles is due to the accumulation of toxic fire in the stomach that cannot be vented outward. The stagnation rises and attacks upward. One should use Zhuru Shigao Tang (Bamboo Shavings and Gypsum Decoction) to harmonize the middle jiao and clear heat, after which the vomiting will naturally cease. If clear saliva and white foam are vomited, this is also due to internal stomach fire forcing fluids to rise upward. In cases where vomitus resembles medicinal herbs or forms clumps, it is appropriate to employ methods to descend fire and resolve phlegm. Herbs such as Tianhuafen (Trichosanthis Radix), Beimu (Fritillariae Bulbus), Gancao (Licorice Root) combined with Jiegeng (Platycodonis Radix), Huangqin (Scutellariae Radix) combined with Huanglian (Coptidis Rhizoma), Xingren (Prunus Semen), Sangbaipi (Mori Cortex), and Maidong (Ophiopogonis Radix) may be used. However, drying agents such as Nanting (Arisaematis Tuber) and Banxia (Pinelliae Rhizoma) must not be used recklessly.
Vomiting and diarrhea caused by intestinal parasites often occur between the onset and eruption phases of measles. This is because heat in the stomach and phlegm above the diaphragm disturb the parasites, causing them to move restlessly, leading to vomiting and purging. If this occurs after the measles rash has subsided, it indicates severe heat preventing food intake; lacking nourishment, the parasites seek food and exit. If more than ten worms are expelled, there is a risk of "worm convulsion" (Chong Jue). Vomiting worms during the initial fever stage suggests deficiency of the spleen and stomach, making it difficult for the rash to erupt, which is highly unfavorable. Parasites passing down after the rash subsides indicate residual stomach heat; this will resolve spontaneously once the heat recedes.
Restlessness and thirst in measles patients signify severe obstruction by toxic fire. The heart is disturbed by heat, preventing the spirit from remaining tranquil, resulting in epigastric discomfort and irritability. Heat stagnation in the stomach scorches and consumes body fluids, causing dry mouth and thirst. When the rash first appears, use Shengma Gegen Tang (Ephedra and Pueraria Decoction) added with Maidong (Ophiopogonis Radix) and Tianhuafen (Trichosanthis Radix). After the rash has fully erupted, use Baihu Tang (White Tiger Decoction). After the rash subsides, use Zhuye Shigao Tang (Bamboo Leaf and Gypsum Decoction). If restlessness becomes extreme with dryness and unquenchable thirst, the condition becomes difficult to treat.
Delirium is caused by the dimming of true qi and unclear consciousness. The Heart stores the spirit and governs fire; when diseased, heat energy converges upon it. In measles, excessive heat in the stomach ascends to invade the Heart. When the Heart is obscured by heat, the spirit becomes chaotic, speech becomes incoherent, and delirious speech ensues. Before the rash appears, Sanhuang Shigao Tang (Three Yellows and Gypsum Decoction) is suitable for treatment. After the rash appears, Huanglian Jiedu Tang (Coptis Decoction for Resolving Toxicity) should be used.
Convulsions occurring in measles should not be equated with acute wind-stroke (Ji Jing Feng). During the initial fever stage when the rash begins to show, intense heat agitates the qi and disturbs the sinews and collaterals throughout the body, leading to convulsions. This usually poses no serious harm and requires only dispersal and cooling measures. However, if high fever persists after the rash subsides and sudden convulsions occur, use Daochi San (Guide Out Red Powder) added with Renshen (Ginseng Radix et Rhizoma) and Maidong (Ophiopogonis Radix), administered with Qiwei Anshen Wan (Seven-Ingredient Calming Spirit Pill). If urine is clear, the condition is treatable; if urine is scanty, it is difficult to treat. If excessive phlegm is observed, Baolong Wan (Embrace Dragon Pill) may be used, or Siwu Tang (Four Substances Decoction) can be modified with Maidong (Ophiopogonis Radix), Zaoren (Ziziphi Spinosae Semen), Danzhuyeh (Lophatheri Herba), Longdancao (Gentianae Radix), Gancao (Licorice Root), Huanglian (Coptidis Rhizoma), Fuling (Poria), Chensha (Cinnabar), and Shichangpu (Acori Tatarinowii Rhizoma).
