朱曰 自微热至渴热。凡五条。辨证之细。最足增人识见。
The eruption of measles relies entirely on heat. If the body is not hot, the measles will not erupt. A tidal fever that is moderate and accompanied by a harmonious pulse is auspicious. However, if the condition presents with no heat, it is highly inauspicious.
"No heat" refers to a body temperature that is warm and cool without fever. In the initial stage, high fever is not suitable; but when the measles are about to erupt properly, there should not be an absence of heat internally. If no heat is present, it constitutes a *ni zheng* (adverse syndrome/reversal pattern). At this point, one should use Xuan Du Fa Biao Tang (See Clause 5), removing Shengma (Cimicifugae Rhizoma), Gancao (Glycyrrhizae Radix et Rhizoma), Jiegeng (Platycodonis Radix), and Dan Zhu Ye (Lophatheri Herba); or alternatively, use Gegen Jie Ji Tang (See Clause 5), removing Gancao (Glycyrrhizae Radix et Rhizoma), Dan Zhu Ye (Lophatheri Herba), and Chishao (Paeoniae Radix Rubra), to employ methods of dispersing and supporting the eruption. There are also cases where Yuebi Tang is used, but this should not be taken as a fixed rule. If there is no heat after the measles have fully erupted or subsided, it indicates that the pathogenic toxins have been exhausted, and no medication is required.
Zhu stated: The absence of heat constitutes an adverse syndrome. Parents often invite doctors to reduce fever because their children's fever persists, not realizing that during the proper eruption phase, the presence of internal heat is necessary.
Yuebi Tang treats Feng Shui (Wind-Edema) characterized by aversion to cold, generalized edema, floating pulse, thirstlessness, continuous spontaneous sweating, and absence of significant heat on the body surface. "Yuebi" means to vent the pathogenic qi of damp-earth. The qi of water and dampness, having invaded the middle and spread externally due to wind, agitates and knots together, making separation difficult. It is therefore necessary to borrow Mahuang (Ephedrae Herba) to dispel it, allowing it to vent from the exterior; and use Shigao (Gypsum Fibrosum) to clear it, allowing it to transform from the interior.
Ingredients: Mahuang (Ephedrae Herba) (remove nodes, 5 qian), Shigao (Gypsum Fibrosum) (8 qian), Zhi Gancao (Prepared Glycyrrhizae Radix et Rhizoma) (1 qian), Shengjiang (Zingiberis Rhizoma Recens) (3 slices), Hongzao (Jujubae Fructus) (3 pieces, as a guide). Decoct in water and divide into three doses for administration.
Since the emergence of measles makes one fearless of intense heat like fire burning the sky, one should not remain complacent if slight heat is encountered. In the initial stage, slight heat is still acceptable; but during the proper eruption phase, slight heat is undesirable.
"Slight heat" refers to mild intensity that is not vigorous. It is appropriate in the initial stage, but during the proper eruption phase, the heat should not be slight. If the heat is slight, the measles may emerge but fail to penetrate thoroughly. One should use dispersing and supporting formulas, primarily using Gegen Jie Ji Tang (See Clause 5) with Chishao (Paeoniae Radix Rubra), Gancao (Glycyrrhizae Radix et Rhizoma), and Dan Zhu Ye (Lophatheri Herba) removed. If there is slight heat before the measles subside or after they have subsided, it indicates that the toxins are mild and have been exhausted, so medication is unnecessary.
Zhu stated: From slight heat to thirsty heat, there are five clauses in total. The differentiation of syndromes is very detailed, which is most sufficient to enhance one's understanding.
