Constant tidal fever never ceases. Inability to eat is also a cause for worry. Cough, thirst, and phlegm symptoms appear. The wonderful prescription of nourishing yin and retreating yang should be administered.
At the initial stage of measles, tidal fever is most desirable. During the eruption phase and after it has subsided, the absence of fever is desirable. If tidal fever persists from the beginning until the end of the eruption, or if there is an inability to eat, cough, thirst, or phlegm symptoms, all these belong to blood deficiency and blood heat. Since measles belongs to the yang category, blood is often depleted by deficiency. Nourishing yin and tonifying blood will naturally eliminate the heat. This is the meaning of "nourishing yin to retreat yang." It is appropriate to use Siwu Tang (Four Substance Decoction) (see Article 25), modified according to symptoms. For thirst, add Ophiopogonis Radix (Maidong), Trichosanthis Radix (Tianhuafen), or Rhinocerotis Cornu juice (Xijiao). For cough, add Benincasae Semen Exsiccatum cum Melle (Gualou Shuang) and Armeniacae Semen Amarum Exsiccatum (Xingren Shuang). For phlegm, add Fritillariae Bulbus (Beimu) and Citri Reticulatae Pericarpium (Chenpi). For wheezing, add Sinapis Semen (Jiezi) and Lepidii Semen (Tingli). Generally, it is advisable to add Scrophulariae Radix (Yuanshen), Phellodendri Cortex (Huangbai), and other herbs such as Bupleuri Radix (Chaihu - *Note: text says Guopi which usually refers to Cortex Lycii/Digupi*), Paeoniae Radix Alba (Baishao - *Note: text implies Digupi/Goubi*), Scutellariae Radix (Huangqin), and Phellodendri Cortex (Huangbai) for treatment. One must strictly avoid using Ginseng Radix et Rhizoma (Renshen), Pinelliae Rhizoma Preparatum (Banxia), and Atractylodis Macrocephalae Rhizoma (Baizhu).
Zhu stated: Constant tidal fever indicates either blood deficiency or blood heat. Nourishing yin and tonifying blood is the best treatment method. The modifications are also consistent with the principles.
Internal blazing of heart fire causes irritability and dryness; urgently one should purge fire and clear the heart source. If due to chronic illness and deficiency of primal qi, the method is to nourish yin, and the illness will recover.
Measles inherently involves fire pathogen, which is governed by the Hand-Shaoyin Heart Meridian. Internal blazing of heart fire causes irritability and restlessness. If the fire pathogen in measles is too severe, causing irritability, thirst, and constipation, leading to the failure of the rash to erupt, the condition is extremely critical. It is appropriate to clear the heart, purge fire, and clear the lung metal. Use Qingyang Tang (Clear Yang Decoction) (see Article 9), removing Cimicifugae Rhizoma (Shengma), Platycodonis Radix (Jiegeng), Glycyrrhizae Radix et Rhizoma (Gancao), and Zingiberis Rhizoma Recens (Shengjiang), while doubling the dosage of Ophiopogonis Radix (Maidong), Scutellariae Radix (Huangqin), and Scrophulariae Radix (Xuanshen). Administer three doses, and the rash will erupt. Alternatively, use Jiawei Renshen Baihu Tang (Modified Ginseng White Tiger Decoction) (see Article 29), removing Ginseng Radix et Rhizoma (Renshen), Glycyrrhizae Radix et Rhizoma (Gancao), and Coicis Semen (Jimi), adding Scutellariae Radix (Huangqin) and Scrophulariae Radix (Yuanshen) for treatment. If it is a person with chronic illness, deficient in primal qi, or developing irritability and thirst, with the rash failing to resolve and stagnating in the skin, this is due to blood deficiency. In this case, the previous two formulas should not be used; instead, Yangyin Tang (Nourish Yin Decoction) should be used. This embodies the principle of "nourishing yin to harmonize yang."
Zhu stated: Irritability, thirst, and constipation leading to the failure of the rash to erupt is also one of the most dangerous aspects of measles. Knowing the methods of purging fire and clearing the lungs ensures safety even in dangerous situations. Regarding the Qingyang Tang and Modified Ginseng White Tiger Decoction shown in this article, they both contain dispersing herbs, which makes them excellent.
