When the heat is severe, thirst arises, and the patient drinks copiously. At this time, no remedy can compare to those that clear toxicity and generate fluids. However, if drinking is indulged without restraint, water will accumulate in the middle jiao, giving rise to various pathological manifestations.
Measles is originally caused by fire pathogenic factors. Heart fire flares up internally, causing the lungs to become scorched and dry, the stomach to wither, and body fluids to be depleted. During fever, there is invariably thirst. Cold water should not be given for drinking; instead, it is appropriate to decoct mung beans, or sesame seeds, or fried aged rice in water for the patient to drink. The treatment method should focus on generating fluids and clearing toxicity. Use Jiawei Renshen Baihu Tang (Modified Ginseng and White Tiger Decoction), removing Renshen (Ginseng Root), Gancao (Licorice Root), and Nuomi (Glutinous Rice), and adding Lianqiao (Forsythia Fruit), Niubangzi (Arctium Fruit), and Yuanshen (Scrophularia Root) as the primary prescription. If one allows the patient to indulge in drinking cold water, it will inevitably lead to the disease of water accumulation. If water enters the Lung, it causes wheezing and coughing; Tingli (Descurainiae Semen) should be used to drain water from the Lung. If water enters the Spleen, it causes edema, distension, and white loose stools. If water enters the Stomach, it causes vomiting and hiccups; Zhuling (Polyporus) and Zexie (Alisma Rhizome) should be used to drain water from the Spleen and Stomach. If water enters the Heart, it causes fright and palpitations; Mutong (Akebia Caulis) and Chifuling (Red Poria) should be used to drain water from the Heart. If water enters the Liver, it causes hypochondriac pain; Chen Yuanhua (Old Genkwa Flower) should be used to drain water from the Liver. If water enters the Kidney and Bladder, it causes difficulty in urination; Cheqianzi (Plantago Seed) and Mutong (Akebia Caulis) should be used to drain water from the Kidney and Bladder. If the patient is thirsty but has abdominal distension and loss of appetite, this indicates deficiency in the root and excess/stagnation in the branch, constituting a critical condition that is difficult to treat.
Jiawei Renshen Baihu Tang (Modified Ginseng and White Tiger Decoction)
Renshen (Ginseng Root), Zhimu (Anemarrhenae Rhizoma), Shu Shigao (Prepared Gypsum Fibrosum), Gegen (Puerariae Lobatae Radix), Tianhuafen (Trichosanthis Radix), Maidong (Ophiopogonis Radix), Danzhuye (Lophatheri Herba), Gancao (Licorice Root), Nuomi (Glutinous Rice). Take a pinch of Nuomi as an adjuvant. Decoct in water until the rice is cooked.
After the measles rash subsides, it is favorable for the patient to have no fever. If recurrent fever occurs, it indicates that the toxin has not yet dissipated. Treatment should employ harmonizing and cooling methods. One must not neglect this simply because the rash has fully resolved.
Recurrent fever refers to the situation where fever has subsided but reappears. This symptom does not exist initially; it must wait until after the measles rash has receded, the fever breaks, and the body becomes cool. After six or seven days, the fever returns. This condition is not caused by a secondary contraction of wind pathogens, but rather by residual heat that has not been cleared and residual toxins returning. The treatment should ideally employ cooling and harmonizing methods. Use Chaihu Maimendong San (Bupleurum and Ophiopogon Powder), removing Renshen (Ginseng Root) and Gancao (Licorice Root); or use Shengdi Gupi Tang (Rehmannia and Dogwood Bark Decoction), removing Gancao (Licorice Root). However, during the initial stage of measles, warmth is preferred, whereas recurrence of fever after recovery is strictly contraindicated. Why? Because measles fever often lasts eleven or twelve days, and at least seven or eight days. Prolonged fever depletes primordial qi. Furthermore, after the eruption of the rash, food intake is poor. Now, with the return of high fever, yin and yang are severely exhausted. Death seems imminent. Urgent regulation and treatment are required. Therefore, for recurrent fever, it is necessary to greatly tonify qi and blood. Use Chaihu Siwu Tang (Bupleurum and Four Substances Decoction) as the primary prescription.
Zhu stated: Recurrent fever after recovery causes great worry for the patient's family. However, it generally stems from these three causes. With proper treatment, there will surely be no hindrance.
Chaihu Maimendong San (Bupleurum and Ophiopogon Powder) treats severe fever after the rash subsides.
Chaihu (Stir-fried with wine), Renshen (Ginseng Root), Yuanshen (Scrophularia Root) (each 5 fen), Longdancao (Gentian Root) (3 fen), Maidong (Ophiopogonis Radigo, heart removed) (8 fen), Gancao (Licorice Root). Decoct in water and take orally.
Shengdi Gupi Tang (Rehmannia and Dogwood Bark Decoction)
Digupi (Dogwood Bark), Shengdihuang (Rehmanniae Radix), Yuanshen (Scrophularia Root), Maidong (Ophiopogonis Radix), Longdancao (Gentian Root), Niubangzi (Arctium Fruit), Lianqiao (Forsythia Fruit), Huangqin (Scutellariae Radix, stir-fried with wine), Zhiren (Gardeniae Semen, fried), Chifuling (Red Poria), Mutong (Akebia Caulis), Gancaoshao (Licorice Root tip), Dengxincao (Junci Medulla) as an adjuvant.
If the tongue coating is white, add Jingjie (Schizonepetae Herba) and Fangfeng (Saposhnikoviae Radix). If the tongue coating is yellow, add Lianqiao (Forsythia Fruit, stir-fried with wine). If constipation occurs, add Zhike (Aurantii Fructus) and Huomaren (Cannabis Semen). If constipation is severe, add Chouniu (Pharbitidis Semen).
