The fever comes like the tide at its appointed time. When first observed, there are no other defects. If tidal fever persists after the rash has fully erupted, apply the excellent prescription for retreating yang and benefiting yin.
Although tidal fever varies in severity, there is no need to panic when it appears initially. If tidal fever occurs after the rash has fully erupted and receded, urgently use Siwu Tang (Four Substance Decoction) to benefit its yin blood.
Tidal fever refers to an episode occurring once daily towards evening, arriving punctually like the tide, never failing in its trust. This syndrome is commonly seen during the initial eruption of measles. If observed after the rash has fully erupted and receded, it is caused by deficiency of qi and blood. Treatment should primarily focus on retreating yang and benefiting yin, using Siwu Tang (see Article 25) added with Ren Shen (Ginseng Radix).
Measles eruption originates from pathogenic invasion of the Heart and Lung; excessive fire leads to dryness and depletion of body fluids, causing dry mouth. Clearing Heart fire and purging Lung heat often yields miraculous effects; if treatment does not violate the appropriate timing, the disease will be cured early.
Thirst belongs to the syndrome of excessive heat in the Lung and Stomach meridians. The lips and mouth appear red like cinnabar when thirst occurs. Treatment involves clearing Metal (Lung), purging fire, and generating fluids. Management must distinguish between excess and deficiency patterns.
In measles, the condition burns like fire, and patients drink copiously, yet the blazing pathogen cannot be extinguished by a single cup of water. The throat is dry, the Heart feels hot, body fluids are depleted, and the Stomach cavity feels scorched.
Thirst is a manifestation of excessive heat in both the Lung and Stomach meridians; the lips and mouth must be red. If the lips and mouth are red like cinnabar, this indicates thirst accompanying measles eruption. This is caused by internal heat; excessive fire depletes fluids, resulting in dry mouth. Treatment should involve purging fire, clearing Metal (Lung), and generating fluids. However, one must examine whether the pattern is excess or deficiency before treating. If stool and urine are constipated and difficult, heat is severe; suitable cooling and draining agents should be used, primarily Mendong Ganlu Yin (Asparagus Decoction for Sweet Dew), removing Gan Cao (Licorice Root). If stool and urine are clear and free-flowing, and the lips and mouth are pale without redness but still thirsty, this is likely due to overuse of cold-natured medicines damaging Middle Qi and qi-blood. Spleen deficiency fails to generate fluids, causing thirst. Use Qiwei Baizhu San (Seven-Ingredient Atractylodes Macrocephala Powder), removing Mu Xiang (Aucklandia Root), replacing Bai Zhu (Atractylodes Rhizome) with Shan Yao (Dioscorea Rhizome), and adding Jing Mi (Rice Grain). For thirst appearing during the initial heat stage, use Baihu Tang (White Tiger Decoction) (see Article 26), removing Gan Cao (Licorice Root) and Jing Mi (Rice Grain), adding Mai Dong (Ophiopogon Tuber), Tian Hua Fen (Trichosanthes Root), Niu Bang Zi (Arctium Fruit), and Lian Qiao (Forsythia Fruit). Alternatively, use Gegen Maidong San (Kudzu and Ophiopogon Powder), removing Ren Shen (Ginseng Radix), Sheng Ma (Cimicifuga Rhizome), Chi Shao (Red Peony Root), and Gan Cao (Licorice Root), adding Tian Hua Fen (Trichosanthes Root), Lian Qiao (Forsythia Fruit), and Niu Bang Zi (Arctium Fruit). For severe thirst, use Huanglian Jiedu Tang (Coptis Decoction for Resolving Toxicity) (see Article 2), removing Gan Cao (Licorice Root) and Jie Geng (Platycodon Root). For thirst appearing during the eruption phase, use Baihu Tang (White Tiger Decoction) (see Article 26), removing Gan Cao (Licorice Root) and Jing Mi (Rice Grain), adding Mai Dong (Ophiopogon Tuber), Tian Hua Fen (Trichosanthes Root), Niu Bang Zi (Arctium Fruit), and Lian Qiao (Forsythia Fruit). For severe thirst during eruption, use Baihu Jiedu Tang (White Tiger Decoction for Resolving Toxicity) (see Article 26). For thirst accompanied by restlessness, use Baihu Jiedu Tang (White Tiger Decoction for Resolving Toxicity) (see Article 26) added with Huang Bo (Phellodendron Bark). For thirst appearing during the exact receding phase of the rash, use Zhuye Shigao Tang (Bamboo Leaf and Gypsum Decoction) (see Article 26), removing Ren Shen (Ginseng Radix), Ban Xia (Pinellia Tuber), Zhi Gan Cao (Honey-fried Licorice Root), and Jing Mi (Rice Grain), adding Yuan Shen (Scrophularia Root), Niu Bang Zi (Arctium Fruit), Huang Qin (Scutellaria Root), Huang Lian (Coptis Rhizome), Sheng Di Huang (Rehmannia Root), and Di Gu Pi (Lycium Bark).
Master Zhu said: Thirst without redness of the lips indicates Spleen deficiency failing to generate fluids. Such precise differentiation is remarkable.
