Pain arises because the flow of Qi and blood in the meridians is ceaseless, circulating around the body without rest. When cold pathogen enters the meridians, it causes stagnation and delay; the blood becomes congealed and fails to circulate. If the cold pathogen lodges outside the vessels, there is a deficiency of blood; if it lodges inside the vessels, there is an obstruction of Qi. Thus, pain occurs suddenly. This pain may stop suddenly, or be severe and unceasing; it may be so severe that it cannot be pressed, or pressing may relieve it, or pressing may have no effect; the pulse may throb rapidly at the site of pain; there may be mutual traction causing pain between the heart and the back, or between the hypochondrium and ribs, or abdominal pain radiating to the inner thigh; pain may persist for days and form accumulations; sudden severe pain may cause loss of consciousness, followed by recovery after a short interval; or pain may be accompanied by constipation. The manifestations of various pains are different.
The Classics state: "When cold pathogen lodges outside the vessels, the vessels become cold; when cold, they contract; when contracted, the vessels become crimped (crimped implies tension). Vessel tension externally pulls on the small collaterals, preventing the free flow of Wei Qi, thus causing sudden pain, which ceases upon receiving warmth." When cold pathogen lodges within the meridians and contends with heat Qi, the vessels become full; fullness leads to chaotic Qi and blood, causing pain that cannot be pressed. When cold pathogen lodges between the stomach and intestines, below the membrane source, blood cannot disperse, and small collaterals contract and pull, causing pain. Pressing allows the Qi and blood to disperse, and the pain stops. Hypochondriac pain is caused by cold pathogen lodging in the Jueyin meridians. Mutual traction pain between the back and the heart is caused by cold pathogen lodging in the Back-Shu point meridians, connecting to the heart. Pain radiating from the lower abdomen to the inner thighs is caused by cold pathogen lodging in the lower abdomen with congealed blood. Sudden death from pain is caused by cold pathogen lodging in the Zang organs, leading to rebellious Qi blocking upwards, exhaustion of Yin Qi, and failure of Yang Qi to enter; thus, sudden pain and death occur. If Qi returns, life is preserved.
Observing the five colors of the face: yellow and red indicate heat; white and blue/green indicate cold; blue/green and black indicate pain. The Classics state: "Deficiency allows pathogens to sink and lodge; if they remain between the ribs, excessive cold causes tendon spasms and bone pain, while excessive heat causes flaccid tendons and emaciated flesh." All conditions involving pain, itching, sores, ulcers, carbuncles, and swelling belong to Heart Fire blazing and should not be uniformly treated as Cold Damage (Shanghan). In treating pain, one must first examine the root cause, then clarify the excess or deficiency of the meridians and skin regions; only then will medication be accurate.
