Pulse: In internal injury due to overwork, the pulse is wide and uncontrolled; if the stomach qi is damaged, it is hidden and difficult to find; in internal injury due to diet, it is slippery, rapid, floating, and sinking; in internal injury due to food stagnation, it is rapid, large, and obstructed. The right Guan pulse being slow and tight indicates cold-dampness intertwining; the right Guan pulse being rapid and slow indicates damp-heat approaching together; if rapid and also showing a intermittent pulse (Dai Mai), it indicates food injury combined with pathogenic influence.
Differentiation of External and Internal Injury Symptoms:
Li Dongyuan said: When the Renying pulse is larger than the Qikou pulse, it is external injury; when the Qikou pulse is larger than the Renying pulse, it is internal injury. In external injury, aversion to cold and fever occur simultaneously without interruption; in internal injury, they alternate and are irregular. External injury's aversion to cold cannot be relieved even by proximity to fierce fire; internal injury's aversion to cold is resolved by warmth. External injury's aversion to wind is intolerance to all winds; internal injury's aversion to wind is only sensitivity to slight drafts. External injury symptoms manifest in the nose, so nasal qi is obstructed and forceful; internal injury does not work this way, its symptoms manifest in the mouth, so taste is lost and the abdomen is disharmonious. External injury differs thus: external injury has excess pathogenic qi, so speech is loud and vigorous, and severity increases from light to heavy; internal injury has deficient yuan qi, so speech is lazy and timid, and severity decreases from heavy to light. External injury presents with heat on the back of the hand and no heat in the palm; internal injury presents with heat in the palm and no heat on the back of the hand. Internal injury headache comes and goes; external injury headache is constant until it transmits to the interior. Internal injury leads to lethargy, desire to lie down, and flaccidity of the four limbs; external injury causes bedridden state from day one, requiring support to move, with muscle spasms and bone pain. External injury prevents eating, yet taste remains and there is no aversion to food; internal injury causes food aversion and loss of taste. After three days of external injury, grains are digested and fluids metabolized, pathogenic qi transmits inward causing thirst; in internal injury, pathogenic qi remains in the blood vessels as excess, so there is no thirst.
Internal injury belongs to deficiency syndromes, mainly caused by diet and fatigue; treatment involves warming, tonifying, regulating, and nourishing, all belonging to tonification methods. External injury belongs to excess syndromes, mainly caused by wind-cold, summer-heat, and dampness; treatment involves purging, vomiting, sweating, and diuresis, all belonging to draining methods.

They guide clear qi from the spleen-stomach into the yang pathways and meridians to generate yin-yang qi, nourishing the harmony of spring qi; they also guide the sweet-warm aroma of Huangqi (Astragali Radix), Renshen (Ginseng Radix et Rhizoma), and Gancao (Glycyrrhizae Radix et Rhizoma) upward to fill the cou li (pores), allowing yang qi to defend the exterior and consolidate. Generally, after major illness, grains digest, fluids go, essence scatters, and defensive qi perishes, often leading to dryness and constipation. Tonifying middle and augmenting qi is essential. The spleen is the central pivot, irrigating the four sides, transporting fluids with the stomach. Moreover, the Large Intestine governs fluids, and Small Intestine governs liquids, all receiving qi from the stomach. Once stomach qi is filled, fluids flow naturally. For severe dryness, use acrid to moisten and bitter to drain.
Official Liu of Taifu Shuixian suffered from excessive overwork, fever with aversion to cold, headache and body pain, dry mouth and thirst, nausea and vexation. One doctor used Qianghuo Tang (Notopterygi Decoction), another Huoxiang Zhengqi San (Patchouli Regulating Qi Powder), neither was effective. Acidic fatigue increased, hands and feet felt unsupported, heart panic and spirit chaos, insomnia day and night, restless sitting and lying, unable to take soup/water, vomiting upon smelling medicine. I diagnosed six pulses as large and rapid, Qikou pulse tight and full, indicating internal injury to yuan qi. Used Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) plus Yuanzhi (Polygalae Radix), Suanzaoren (Ziziphi Spinosae Semen), Zhuru (Bambusae Caulis in Taeniam), and Maidong (Ophiopogonis Radix).
