<Pulse>: In various conditions presenting with bleeding, a Kou pulse (Kong pulse, hollow like a scallion stalk) is observed. By examining the location of this pulse, one can verify the source of the bleeding. Generally, for blood loss, a deep and fine pulse is desirable; if a surging and large pulse appears, the condition will be difficult to treat later.
Hematemesis (vomiting blood), epistaxis (nosebleed), hemoptysis (coughing up blood), spitting blood, sputum blood, hematuria (blood in urine), hematochezia (blood in stool), Changfeng (bleeding before stool), Zangdu (bleeding after stool).
Blood disorders refer to pathological changes in the body's blood. Blood belongs to Ying (Nutritive Qi), and Qi belongs to Wei (Defensive Qi); the Heart governs blood, the Liver stores blood, and the Spleen controls blood; blood follows the movement of Qi, and when Qi rebels, blood also rebels; organs and bowels require blood to perform their functions of moistening and nourishing; the eyes require blood to see, the tongue requires blood to speak, the hands require blood to grasp, and the feet require blood to walk. Ying and Wei circulate day and night without ceasing. If injury from overwork leads to internal fire agitation, it causes blood loss. All blood disorders belong to Heat syndromes. Medications should be cooling. These are caused by excess Yang and deficiency Yin, where Fire carries blood upward, causing blood to flow recklessly and rebel. Use Xijiao Dihuang Tang (Rhinoceros Horn and Rehmannia Decoction) with modifications based on symptoms. Fresh red blood indicates new blood, which should be stopped with hemostatic agents; purple-black clotted blood indicates stasis, which should be removed with blood-invigorating agents; afterwards, use Burong Tang (Nourish Ying Decoction) for regulation. The pulse after blood loss should be deep, fine, and gentle, not floating, large, or solid. Blood flows when hot and coagulates when cold; red color belongs to Fire, and black color belongs to Water. Seeing black blood necessitates stopping the bleeding, which is natural. If sudden massive vomiting of purple blood occurs, it is harmless because dead blood damaged by heat previously stagnated in the stomach region, and vomiting it out is beneficial. If bleeding is stopped too early, leaving blood behind, it may later form painful blood masses that are difficult to treat; use Huoxue Tang (Invigorate Blood Decoction) with modifications (see Abdominal Pain chapter); if hematemesis is followed by phlegm, it indicates Yin deficiency with Fire agitation, treated with Ziyin Jianghuo Tang (Nourish Yin and Descend Fire Decoction) with modifications; if phlegm precedes blood, it indicates accumulated Heat, treated with Qingfei Tang (Clear Lung Decoction) with modifications.
Hematemesis originates from the Stomach, and what is vomited is pure blood.
Xijiao Dihuang Tang (Rhinoceros Horn and Rehmannia Decoction): Treats all types of hematemesis, epistaxis, hemoptysis, spitting blood, and sputum blood.
