Pulse: In cases of Fanwei (Revolving Stomach) and Yige (Choked Esophagus), if the Cun position is tight and the Chi position is choppy, or if a Kou (Hollow) or Xian (Wiry) pulse is present, it indicates a deficiency-cold condition. A Chen (Deep) pulse at the Guan position suggests phlegm; a Fu (Floating) and Choppy pulse indicates Spleen accumulation; a Fu and weak pulse signifies Qi deficiency; a Choppy and small pulse denotes Blood deficiency. If the pulse is both Choppy and Deep, it is caused by obstruction from the Seven Emotions.
Generally, the conditions of Ge (Diaphragm obstruction), Ye (Choking), and Fanwei (Revolving Stomach) arise from excessive emotional agitation stirring up Fire in the Five Zang organs. This heat steams body fluids, causing Phlegm to increase, while the Spleen and Stomach gradually weaken. Consequently, food and drink cannot circulate smoothly, leading to Ge, Ye, or Fanwei. Zhu Danxi stated: "Elderly patients are difficult to cure." This is because young people have not yet depleted their Qi and Blood. Although medication can temporarily expel Phlegm and provide relief, the disease will immediately recur. The reason is that Qi deficiency fails to transform fluids, generating Phlegm, while Blood deficiency fails to moisten, generating Fire. He further stated: "This condition must absolutely not be treated with aromatic-drying herbs or rich, greasy foods." Since the pathology involves Heat and Dryness, aromatic-drying herbs are contraindicated. Aromatic substances disperse Qi, drying substances consume Blood, and rich flavors assist Fire and generate Phlegm. Unskilled physicians, unaware of the disease's origin, seeing such ailments, use harsh agents to force immediate effects for profit. They fail to consider that when the patient is critically ill, even treatment may not save them. Is this not lamentable?
In general, for Ge, Ye, and Fanwei, one must not administer acrid-hot aromatic-drying medicines, as these most easily consume Yin Blood. Stool resembling sheep dung is an untreatable sign, due to the Large Intestine lacking Blood nourishment. Vomiting white foam is also untreatable, indicating exhaustion of both Qi and Blood.
Fanwei refers to the Stomach being deficient, vomiting food without accepting new intake.
