At the onset of the disease, if the Yuan Qi (Original Qi) is not yet deficient, purgative methods must be used. This is the principle of "treating obstruction with unblocking agents" (Tong Yin Tong Yong). A deep and fine pulse indicates easy treatment; a large and rapid pulse indicates difficult treatment. Furthermore, a slippery, large, floating, wiry, and urgent pulse indicates a fatal prognosis, whereas a deep and fine pulse is harmless.
Treat early-stage dysentery where accumulated qi is excessive, causing unbearable abdominal pain, bloating, tenesmus (urgent need to defecate but unable to pass stool), frequent trips to the toilet without successful evacuation, and extreme distress.
After five days, when Qi is deficient, or in the elderly and frail, purgative methods should not be used.
