Similar to malaria, also known as "malaria-like syndrome," refers to symptoms that occur and cease at regular intervals, unlike the alternating chills and fever which have no fixed pattern.
In Taiyang disease presenting with a malaria-like syndrome, if the pulse is floating and surging, Guizhi Tang (Cinnamon Twig Decoction) is indicated; if urination and defecation are normal, there is no vomiting, and the episodes occur two or three times a day, Guizhi Mahuang Ge Ban Tang (Half-Half Decoction of Cinnamon Twig and Ephedra) is indicated.
In Yangming disease presenting with a malaria-like syndrome, if there is irritability, heat, sweating, afternoon tidal fever, and a floating pulse, Guizhi Tang (Cinnamon Twig Decoction) is indicated; if the pulse is excess/solid, Chengqi Tang (Gut-Draining Decoction) is indicated.
When pathogenic heat enters the Blood Chamber (uterus and related meridians) in women, the blood must congeal, resulting in symptoms similar to malaria; Xiaochaihu Tang (Minor Bupleurum Decoction) is indicated.
When heat predominates over cold, it indicates Yang pathogen invading Yin. If the Chi (cun-guan-chi) pulse is slow, it indicates blood deficiency; Huangqi Jianzhong Tang (Astragalus Middle-Jiao-Sustaining Decoction) should be administered first to nourish Ying (Nutrient) and Wei (Defensive) Qi. If the pulse is not slow, one may select from Xiaochaihu Tang (Minor Bupleurum Decoction), Guizhi Tang (Cinnamon Twig Decoction), or Yuespi Tang (Yue Bi Decoction).
In Jueyin disease, if the pulse is floating and moderate, the scrotum does not retract, and there is fever with aversion to cold resembling malaria, this indicates the disease is about to resolve; Guizhi Mahuang Ge Ban Tang (Half-Half Decoction of Cinnamon Twig and Ephedra) is indicated.
For Wenre (Warm Malaria), the pulse appears like that of a healthy person, the body has heat but no chills, the joints are painful and vexed, and there is occasional vomiting; Baihu Tang (White Tiger Decoction) added with Guizhi (Cinnamon Twig) is indicated. If there is thirst, Chaihu Jiaren Shen Gualou Tang (Bupleurum Plus Ginseng and Trichosanthes Root Decoction) is indicated.
Anhidrosis (absence of sweat) occurs when Cold pathogen penetrates the meridians, causing the Couli (skin pores) to close tightly, leading to body fluids seeping inward rather than being excreted as sweat. If Wind, Summerheat, or Dampness interfere, spontaneous sweating will occur. For conditions requiring diaphoresis, if three doses of Mahuang Tang (Ephedra Decoction) fail to induce sweating, the condition is considered incurable.
In Taiyang disease where fever is expected, anhidrosis accompanied by headache is treated with Mahuang Tang (Ephedra Decoction). Stiffness in the neck and back with persistent anhidrosis is treated with Gegen Tang (Kudzu Decoction). Gangjing (Rigid Spasm), characterized by fever, aversion to cold, and anhidrosis, is treated with Gegen Tang (Kudzu Decoction) or Xiao Xuming Tang (Minor Rescue Life Decoction).
If the pulse is floating, there is fever and anhidrosis, indicating the Exterior syndrome is unresolved, Mahuang Tang (Ephedra Decoction) is indicated; if sweating persists uncontrollably, Baihu Tang (White Tiger Decoction) is indicated. If there is thirst for water without Exterior signs, Baihu Jiaren Tang (White Tiger Plus Ginseng Decoction) is indicated.
Yangming disease presenting with anhidrosis, a floating pulse, and wheezing is treated with Mahuang Tang (Ephedra Decoction). If there is anhidrosis, difficulty in urination, and mental restlessness (Ao Nu), jaundice will likely develop; Wuling San (Five-Ingredient Powder with Poria) plus Yinchen (Artemisiae Scopariae Herba) is indicated.
Wei Qi (Defensive Qi) functions to fill the Couli (skin pores) and secure body fluids. When Wei Qi is disturbed by pathogens and fails to protect the exterior, sweating results. Spontaneous sweating must be differentiated by Exterior/Interior and Deficiency/Excess. If spontaneous sweating occurs with aversion to wind and cold, the Exterior is unresolved; muscle-releasing methods should be used to induce sweating and relieve wind aversion. Sweating followed by aversion to cold also indicates Exterior Deficiency, requiring warm tonifying agents. If sweating occurs without aversion to wind and cold, the Exterior is resolved but the Interior is disharmonious, warranting purgative treatment. However, if sweating causes hair to become moist like oil or beads that coagulate and do not flow, these are untreatable fatal signs.
In Taiyang Wind Strike, spontaneous sweating with a floating and moderate pulse is treated with Guizhi Tang (Cinnamon Twig Decoction). If sweating is accompanied by thirst and difficult urination, Wuling San (Five-Ingredient Powder with Poria) is indicated; if there is no thirst, Lingjian Gancao Tang (Poria and Licorice Decoction) is indicated. For spontaneous sweating with difficult urition using Guizhi Tang, Shaoyao (Peony Root) and Gancao (Licorice Root) should only be added. For spontaneous sweating, infrequent urination, mental irritability, slight aversion to cold, and leg cramps, Guizhi Fuzi Tang (Cinnamon Twig and Aconite Decoction) is indicated.
In Yangming disease, excessive sweating with thirst, fever, delirium, and hard stool is treated with Tiaowei Chengqi Tang (Stomach-Regulating Gut-Draining Decoction).
After treating Taiyang disease with diaphoresis, if sweating leaks continuously with aversion to wind and cold, Guizhi Fuzi Tang (Cinnamon Twig and Aconite Decoction) is indicated. If urination is normal but sweating occurs, it indicates fluid deficiency; purgatives or diuretics should not be used. Instead, honey suppositories (Mi Dao) should be used to promote bowel movement. If sweating is excessive, stomach fluids are dried up, requiring urgent purgation with Dachengqi Tang (Major Gut-Draining Decoction).
For excessive sweating with thirst, Wuling San (Five-Ingredient Powder with Poria) should not be used. For persistent sweating without other symptoms, apply warming powder externally.
After taking Guizhi Tang (Cinnamon Twig Decoction), if massive sweating occurs with unrelenting irritability and thirst, and the pulse is surging, Baihu Jiaren Tang (White Tiger Plus Ginseng Decoction) is indicated. After purgation, Guizhi Tang (Cinnamon Twig Decoction) must not be used again. If sweating is accompanied by wheezing without high fever, Mahuang Xingren Gancao Shigao Tang (Ephedra, Apricot Kernel, Licorice, and Gypsum Decoction) is indicated.
