When a newborn infant has an abnormally large scrotum, it is called fetal hernia (Tai Shan). As the child grows, there is a fear that it may transform into hydrocele (Shui Shan). The ancient method waits until the infant is over one month old, then on the noon hour of the Dragon Boat Festival (Duanwu), fills a foot basin with hot water and places it in the center of the main hall. The infant is held so that the scrotum is immersed in the water for a moment, then placed on the middle of the threshold. The water marks from the scrotum are left on the threshold. Moxa fire is burned three times at the wet marks on the threshold, causing the scrotum to gradually shrink, with miraculous efficacy.
[Chen Shouzhen's Note] This method is indeed simple but should not be fully trusted. Instead, grind stone crab (Shi Xie) with vinegar and apply it frequently to the area until the scrotum shrinks.
Six or seven days after birth, if the infant's scrotum suddenly retracts into the abdomen accompanied by incessant crying, this is caused by cold invasion. The ancient method uses sulfur (Liu Huang) and Evodia fruit (Wu Zhu Yu) ground into powder, mixed with garlic paste and applied below the umbilicus. Additionally, lightly stir-fried Cnidium fruit (She Chuang Zi) wrapped in cloth is used to moxa-iron the umbilicus, which causes the scrotum to descend.
[Chen Shouzhen's Note] Sulfur is highly toxic and should not be used lightly. Use only Evodia fruit (Wu Zhu Yu) and Fennel seed (Xiao Hui Xiang) ground into fine powder, mashed together with garlic into a paste. When the temperature is neither too hot nor too cold, apply it below the umbilicus and bind with soft cloth. Externally, use a hot water bottle to warm the area until the scrotum descends.
If an infant presents with generalized erysipelas-like redness and swelling that migrates unpredictably, it is mostly due to toxic heat inherited from the fetus. It can also result from excessive warmth after birth, causing toxic heat to evaporate outward, leading to red, hot, and swollen skin resembling cinnabar paint. This condition is called wandering wind rash (Chi You Feng). It typically starts between the head, face, and limbs. If it moves inward to the heart and abdomen, it is incurable. This condition must not be treated with bloodletting via acupuncture. The ancient method uses Rhinoceros Horn Detoxification Decoction (Xi Jiao Jie Du San, containing Rhinoceros Horn [Xi Jiao], Burdock Seed [Niu Bang Zi], Schizonepeta Spike [Jing Jie Sui], Saposhnikovia Root [Fang Feng], Forsythia Fruit [Lian Qiao], Honeysuckle Flower [Yin Hua], Red Peony Root [Chi Shao], Raw Licorice Root [Sheng Gan Cao], Coptis Rhizome [Huang Lian], Raw Rehmannia Root [Sheng Di Di], referencing Ramulus Nymphaeae [Deng Xin]) or Orchid Leaf Powder (Lan Ye San, containing Orchid Leaf [Lan Ye], Scutellaria Root [Huang Qin], Rhinoceros Horn [Xi Jiao], Rhubarb Root [Da Huang], Bupleurum Root [Chai Hu], Gardenia Fruit [Zhi Zi], Cimicifuga Rhizome [Sheng Ma], Gypsum Fibrosum [Shi Gao], Licorice Root [Gan Cao], referencing Bamboo Sap [Zhu Li]). Externally, apply raw Adzuki Bean (Chi Xiao Dou) powder mixed with egg white to the affected area, or apply thin slices of lean pork to the lesion.
[Chen Shouzhen's Note] Erysipelas is caused by toxic heat qi battling with blood, with wind pathogens taking advantage, resulting in migrating redness and swelling. If the toxin originates internally from the abdomen and spreads to the limbs, it is easy to cure; if it enters the abdomen from the limbs, it is difficult to treat. One must act before the toxin enters the Kidney or abdomen. Internally, decoct Mulberry Leaf (Sang Ye), Honeysuckle Flower (Jin Yin Hua), Raw Rehmannia Root (Sheng Di), and Raw Licorice Root (Sheng Gan Cao), mix with a small amount of Bamboo Sap (Zhu Li), and administer slowly. Externally, grind Cold Water Stone (Han Shui Shi), Raw Phellodendron Bark (Sheng Huang Bai), and Raw Licorice Root (Sheng Gan Cao) into powder, mix with Banana Juice (Ba Jiao Zhi) for application. If banana juice is unavailable, fresh well water can be used. Furthermore, imitate the "grafting" method: slice yellow ox meat thinly, soak it in Coptis Rhizome (Huang Lian) water, and apply it to the affected area, replacing it when dry, to absorb the toxic heat. This is superior to using lean pork.
